Book excerpt: Over the last 10 years the syndrome of severe acute renal failure has progressively changed in its epidemiology. It is now most frequently seen in critically ill patients, typically in the context of sepsis and multiorgan failure. This epidemiologic change has meant that intensive care physicians and nephrologists must now work in close cooperation at all times and must take many com plex issues of prevention, pathogenesis, and management into account that they did not previously have to tackle. Simultaneously, the last 10 years have seen the development of major technical and conceptual changes in the field of renal replacement therapy. There are now previously unavailable therapeutic options that provide physicians with a flexible and rapidly evolving armamentarium. The nutrition of these patients, previously limited by the par tial efficacy of renal replacement therapies, has also become more aggressive and more in tune with the needs of critically ill patients. Increased understanding of the pathogenesis of the multi organ failure syndrome has focused on the role of many soluble "mediators of injury" (cytokines, leukotrienes, prostanoids etc.). These molecules are likely to participate in the pathogenesis of acute renal failure. Their generation and disposal is also affected by different techniques of artificial renal support.

Book excerpt: The most common reason for a renal consult in a hospitalized patient is acute renal failure (ARF). This new companion book to Brenner and Rector's The Kidney provides the reader with the complete information necessary to understand, diagnose, and treat this serious and challenging condition. The first part of the book describes recent progress in the understanding of the pathophysiology of ARF, explaining the tremendous strides in the molecular and cellular aspects of cell injury and recovery. The remainder (and majority) of the text emphasizes the clinical spectrum and management of the different disease processes causing ARF. In these chapters, ARF is discussed in terms of its many associated disease processes such as pigment nephropathy, burns, infectious disease, and gastrointestinal and liver disease. This format allows for rapid referral and an in-depth understanding of all clinical aspects of ARF, including its most important aspect of management: to prevent the disease from becoming chronic, ultimately leading to dialysis and then transplantation. Provides the reader with a single-volume, authoritative, clinically oriented resource on ARF. Offers an in-depth analysis of recent advances in scientific and clinical aspects of acute renal failure (ARF). Covers the most recent advances in therapy, and emphasizes diagnosis, management, and prognosis. Integrates basic science with clinical diagnosis and treatment throughout. Describes in detail the mechanisms underlying the development of ARF and highlights the factors which promote recovery. Emphasizes the clinical impact of ARF according to the areas where it is most likely to be seen.

Available in PDF, EPUB and Kindle. Book excerpt: Historically, 20% of all injured combatants die on the battlefield before they can be evacuated to a field hospital. Blood lossâ€"hemorrhageâ€"is the single major cause of death among those killed in action whose lives might otherwise be saved. Fluid resuscitation and the treatment of hypovolemia (the abnormally decreased volume of circulating fluid in the body) offer the greatest opportunity for reducing mortality and morbidity associated with battlefield casualties. In Fluid Resuscitation, a committee of experts assess current resuscitation fluids and protocols for the treatment of combat casualties and make recommendations for future research. Chapters focus on the pathophysiology of acute hemorrhagic shock, experience with and complications of fluid resuscitation, novel approaches to the treatment of shock, protocols of care at the site of injury, and future directions for research. The committee explicitly describes the similarities and differences between acute medical care during combat and civilian emergency trauma care. Fluid Resuscitation should help energize and focus research in both civilian and military emergency care and help save the lives of citizens and soldiers alike.

The importance of the toxicity of drugs and other substances for the kidneys is increasingly recognized. The book consists of themes such as clinical relevance, renal handling and cellular mechanisms of nephrotoxicity, as well as animal and cell culture models. In addition, the volume highlights specific types of drugs, such as anti-infectious agents and anti-inflammatory, cardiovascular and anti-cancer drugs. The last section deals with prevention and focuses on urinary biomarkers, pharmacological aspects and drug dosage in renal failure. In this third and completely revised edition, several new topics were added and additional figures and tables will be included. The book is of interest for the nephrologist, internist, general practitioner, toxicologist, pharmacologist, anesthesiologist, epidemiologist, public health official, pharmaceutical industry, and national drug safety committees, among others.

Available in PDF, EPUB and Kindle. Book excerpt: Internists, surgeons, critical care physicians and nephrologists all treat critically ill patients with renal failure and the multiple system organ dysfunction syndrome. A comprehensive review of the state of the art of this topic is definitely needed both in academic and clinical medicine, and Critical Care Nephrology fulfils this need. It is a useful reference tool for both nephrologists and intensive care specialists and it is therefore no coincidence that the editors of the book are themselves specialists in these particular fields. The book addresses the following: definitions of critical illness, epidemiology, monitoring and diagnostic procedures, pathophysiology of organ systems in relation to kidney function, concepts of renal physiologic and pathologic responses to various derangements, oxygen transport and cardiovascular adaptations, hemodynamic parameters, respiratory parameters, mechanical ventilation and cardiac support, and severity score parameters. The book is also devoted to all forms of acute renal failure with specific reference to intensive care patients. The nature of the multiple organ dysfunction syndrome is discussed with special emphasis on the impact of different organs dysfunction and kidney failure. Kidney function and acute renal failure in patients with kidney, liver and heart transplants is also considered, as well as acute illness occurring in chronic hemodialysis patients. Special emphasis is placed on therapeutic interventions and treatment procedures. Different forms of organ support are discussed including liver, lung and cardiac therapy.

Book excerpt: Physiological, pharmacological and molecular biological data generated over the past three decades have demonstrated the existence of two major families of extracellular receptors, the P1, a family of four G-protein coupled receptors and the P2, a family of at least 12 receptors responsive to purine (ATP, ADP) and pyrimidine (UTP) nucleotides through which adenosine and ATP can function as extracellular messengers. The present two-part volume represents an integrated compendium of invited chapters by leading researchers in the area focusing on advances in the understanding of purinergic and pyrimidinergic signaling systems, their role(s) in tissue function and pathophysiology and advances in developing potential new medications based on the modulation of P1 and P2 receptor signaling processes. The volumes will thus provide the reader with a topical, comprehensive and integrated overview of this important area.

Available in PDF, EPUB and Kindle. Book excerpt: Dr. Goldstein has authored more than 50 publications and co-edited two books. Her latest contribution to the field of nephrology, Mechanisms of Injury in Renal Disease and Toxicity, promotes an understanding of the pathophysiologic mechanisms mediating renal dysfunction in disease. It provides an important perspective in understanding mechanisms of chemically induced renal injury. Over the past decade, understanding of the pathophysiologic and molecular basis of renal disease has grown tremendously. New and evolving concepts on the pathophysiology of glomerulonephritis, chronic glomerular injury, diabetic nephropathy, and acute renal failure are changing the clinical management of these disease states.
