Book excerpt: Alcohol misuse, which includes the full spectrum from drinking above recommended limits (i.e., risky/hazardous drinking) to alcohol dependence, is associated with numerous health and social problems and more than 85,000 deaths per year in the United States and an estimated annual cost to society of more than $220 billion. Alcohol misuse is estimated to be the third leading cause of preventable mortality in the United States following tobacco use and being overweight. Alcohol misuse contributes to a variety of conditions, including hypertension, cirrhosis, gastritis and gastric ulcers, pancreatitis, breast cancer, neuropathy, cardiomyopathy, anemia, osteoporosis, cognitive impairment, depression, insomnia, anxiety, and suicide. Excessive alcohol consumption is a major factor in injury and violence. Definitions of the spectrum of alcohol misuse (i.e., unhealthy alcohol use) continue to evolve. The National Institute on Alcohol Abuse and Alcoholism (NIAAA) has proposed epidemiologically based alcohol-use guidelines to limit risks for drinking-related consequences by establishing age- and sex-specific recommended consumption thresholds. This report's main objective is to conduct a systematic review of the effectiveness of screening followed by behavioral counseling, with or without referral, for alcohol misuse in primary care settings, addressing seven questions. This new review differs from the report on which the USPSTF 2004 recommendations were based in the following ways: We allowed inclusion of screening and behavioral interventions for the full spectrum of alcohol misuse, as long as subjects were identified by screening in a primary care or primary care-like setting; we added referral as an intervention of interest and changed the title to reflect this; we expanded the eligible settings from traditional primary care to also include settings with primary care-like relationships (e.g., infectious disease clinics for people with HIV); and we added additional outcomes of interest to our inclusion/exclusion criteria and analytic framework. Key Questions addressed by this review include: KQ 1: What is the direct evidence that screening for alcohol misuse followed by a behavioral counseling intervention, with or without referral, leads to reduced morbidity, reduced mortality, or changes in other long-term (6 months or longer) outcomes (e.g., health care utilization, sick days, costs, legal issues, employment stability)? KQ 2: How do specific screening modalities compare with one another for detecting alcohol misuse? KQ 3: What adverse effects are associated with screening for alcohol misuse and screening-related assessment? KQ 4a: How do behavioral counseling interventions, with or without referral, compare with usual care for improving intermediate outcomes for people with alcohol misuse as identified by screening? KQ 4b: How do specific behavioral counseling approaches, with or without referral, compare with one another for improving intermediate outcomes for people with alcohol misuse as identified by screening? KQ 5: What adverse effects are associated with behavioral counseling interventions, with or without referral, for people with alcohol misuse as identified by screening? KQ 6: How do behavioral counseling interventions, with or without referral, compare with one another and with usual care for reducing morbidity, reducing mortality, or changing other long-term (6 months or longer) outcomes (e.g., health care utilization, sick days, costs, legal issues, employment stability) for people with alcohol misuse as identified by screening? KQ 7: To what extent do health care system influences promote or hinder effective screening and interventions for alcohol misuse?

Book excerpt: Preventative care in free clinic settings aims to mitigate health risks for vulnerable populations. Heavy alcohol intake is a major threat to physical and mental health. Wellness care for free clinic patients must include regular screening for alcohol use in accordance with national guidelines. The purpose of this project was to implement and evaluate an evidence- based practice change to improve alcohol screening and intervention in a free clinic setting. The NIH/NIAAA screening, brief intervention, and referral for treatment (SBiRT) process was implemented. A convenience sample of de-identified patient charts was reviewed to assess alcohol screening before (n = 38 charts) and after (n = 30) a staff education intervention; data collected (n = 68) did not include demographic or patient identifiers to protect privacy. Comparison of HEDIS scores before and after intervention showed improved alcohol screening and intervention/referral for treatment rates.

The estimated 12-month and lifetime prevalence values for AUD are 13.9% and 29.1%, respectively, with approximately half of individuals with lifetime AUD having a severe disorder. AUD and its sequelae also account for significant excess mortality and cost the United States more than $200 billion annually. Despite its high prevalence and numerous negative consequences, AUD remains undertreated. In fact, fewer than 1 in 10 individuals in the United States with a 12-month diagnosis of AUD receive any treatment. Nevertheless, effective and evidence-based interventions are available, and treatment is associated with reductions in the risk of relapse and AUD-associated mortality. The American Psychiatric Association Practice Guideline for the Pharmacological Treatment of Patients With Alcohol Use Disorder seeks to reduce these substantial psychosocial and public health consequences of AUD for millions of affected individuals. The guideline focuses specifically on evidence-based pharmacological treatments for AUD in outpatient settings and includes additional information on assessment and treatment planning, which are an integral part of using pharmacotherapy to treat AUD. In addition to reviewing the available evidence on the use of AUD pharmacotherapy, the guideline offers clear, concise, and actionable recommendation statements, each of which is given a rating that reflects the level of confidence that potential benefits of an intervention outweigh potential harms. The guideline provides guidance on implementing these recommendations into clinical practice, with the goal of improving quality of care and treatment outcomes of AUD.

Book excerpt: Unhealthy alcohol use is a common, undertreated, and growing public health problem among U.S. adults. Community-level factors, such as stressors within neighborhoods and workplaces, in addition to elements within the health care system such as the quality of primary care, may influence the prevalence of unhealthy alcohol use as well as receipt of evidence-based treatment. This dissertation describes associations between neighborhood disadvantage and patterns of unhealthy alcohol use (Chapter 2), examines relationships between job-related stressors and unhealthy alcohol use (Chapter 3), and explores whether patient-centered medical home (PCMH) implementation was related to receipt of evidence-based care among people with unhealthy alcohol use (Chapter 4). Chapters 2 and 4 examine these relationships using electronic health record (EHR) data from the Veteran Health Administration outpatients, while Chapter 3 uses a national sample of working adults from the National Health Interview Survey. Overall, findings suggest that unhealthy alcohol use and alcohol-related conditions are more prevalent among VA patients living in disadvantaged areas (Chapter 2). Specifically, we found that neighborhood disadvantage was positively associated with all patterns of unhealthy alcohol use and alcohol-related harm. Findings also suggested consistent moderation by race/ethnicity and urbanicity. Association sizes were largest for people of Black/African American and American Indian/Alaska Native race/ethnicities and for those residing in more urban neighborhoods. Among VA patients with unhealthy alcohol use, those served by primary care clinics with stronger PCMH implementation had higher probabilities of brief intervention and medications for alcohol use disorders. These findings indicate that – likely due to increased patient-centeredness of care – PCMH implementation may encourage evidence-based care for unhealthy alcohol use (Chapter 4). Yet, descriptive findings suggested that clinics serving more disadvantaged VA patients tend to have lower PCMH rankings. Finally, relationships between job stress and unhealthy alcohol use require additional investigation (Chapter 3). We found few statistically significant relationships between job-related stressors (job insecurity, financial stress, and job strain) and three patterns of unhealthy alcohol use in gender-stratified models. For females, while financial stress was positively associated with heavy episodic drinking on heavy drinking days, job strain was negatively associated with drinking above recommended limits. Overall, findings from these analyses highlight the need to expand preventive strategies and high-quality primary care for those living in structurally disadvantaged communities.

In 1993, the first version of these guidelines, titled: ‘An outline for the management of alcohol problems: Quality assurance in the treatment of drug dependence project’ was published (Mattick & Jarvis 1993). The Australian Government commissioned an update a decade later (Shand et al. 2003) and a further edition in 2009 to integrate the Guidelines with the Australian Guidelines to Reduce Health Risks from Drinking Alcohol (National Health and Medical Research Council, NHMRC 2009; Haber et al., 2009). The present version of the Guidelines was also commissioned by the Commonwealth of Australia to remain current and integrated with the updated NHMRC consumption guidelines (2020). In order to ensure that guidelines remain relevant, the next set of guidelines should be updated in 2025, consistent with NHMRC recommendation that guidelines be updated every five years. These guidelines aim to provide up-to-date, evidence-based information to clinicians on available treatments for people with alcohol problems and are largely directed towards individual clinicians in practice, such as primary care physicians (general practitioners, nursing staff), specialist medical practitioners, psychologists and other counsellors, and other health professionals. Some chapters highlight service or system level issues that impact on clinicians and their patients. These include recommendations concerning Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, stigma, and discrimination. Elsewhere, organisation capacity is implied, such as medical resources for withdrawal management where recommendations indicate use of medications. As all forms of treatment will not be readily available or suitable for all populations or settings, these guidelines may require interpretation and adaptation.

Book excerpt: Behavioral health conditions, which include mental health and substance use disorders, affect approximately 20 percent of Americans. Of those with a substance use disorder, approximately 60 percent also have a mental health disorder. As many as 80 percent of patients with behavioral health conditions seek treatment in emergency rooms and primary care clinics, and between 60 and 70 percent of them are discharged without receiving behavioral health care services. More than two-thirds of primary care providers report that they are unable to connect patients with behavioral health providers because of a shortage of mental health providers and health insurance barriers. Part of the explanation for the lack of access to care lies in a historical legacy of discrimination and stigma that makes people reluctant to seek help and also led to segregated and inhumane services for those facing mental health and substance use disorders. In an effort to understanding the challenges and opportunities of providing essential components of care for people with mental health and substance use disorders in primary care settings, the National Academies of Sciences, Engineering, and Medicine's Forum on Mental Health and Substance Use Disorders convened three webinars held on June 3, July 29, and August 26, 2020. The webinars addressed efforts to define essential components of care for people with mental health and substance use disorders in the primary care setting for depression, alcohol use disorders, and opioid use disorders; opportunities to build the health care workforce and delivery models that incorporate those essential components of care; and financial incentives and payment structures to support the implementation of those care models, including value-based payment strategies and practice-level incentives. This publication summarizes the presentations and discussion of the webinars.
