Book excerpt: Excerpt from A Treatise on Rheumatism and Rheumatoid Arthritis Although the literature of Rheumatism is very extensive, and although the great advances which have been made in our knowledge of the disease of recent years have greatly modified the views formerly held concerning it, yet the various contributions to the subject are, for the most part, scattered through the Transactions of Societies and the Medical Journals, and have not often been brought together in the Treatise form. I therefore felt, in accepting the invitation of my publishers to write a work upon the subject, that I might be doing a service to medical science if I endeavoured to present a consistent picture of Rheumatism as a systemic disease, of which the articular lesions are, at the same time, the most frequent and conspicuous manifestations, but which also implicates many other structures and tissues of the body, and assumes widely different forms in different cases. How far the outcome of this attempt falls short of its aim, I am only too conscious; but I trust that the work will nevertheless prove useful to those who are anxious to form a notion of the present position of our knowledge of the subject. It has been my endeavour to restrict the denotation of the words "Rheumatism" and "Rheumatic," which are often so loosely employed that they have almost forfeited all claim to be regarded as scientific terms, to one definite set of phenomena, which I believe to depend upon one single and specific morbid process, of the nature of which it is not yet possible to speak with any degree of certainty. The latter portion of the book treats of Rheumatoid Arthritis, which I believe to be, unlike Rheumatism, an essentially articular affection, the lesions of which are dependent upon trophic disturbances.
