This process is even more complicated and impacting when it must be formulated by health practitioners who work in a hospice setting. It has been found that there exists a significant percentage (33%) of hospice nurses which found difficulty in knowing who controlled the overall responsibility of hospice patient care. Such confusion has led to an overwhelming desire by both community nurses and general practitioners for additional educational input from domiciliary services (Seamark, Thorne, Jones, Gray, & Searle, 1993, p. 57). There is little doubt that the health care professional finds themselves in a complex organizational system and must find a way to form their own outlook within that system. Nurses want to simultaneously maintain fidelity to patients and their family members, follow physician colleague orders, work in interdisciplinary family-centered teams, and yet follow their consciences when order care or treatments appear harmful to patients (Catlin et al., 2008, p. 106). The complexities of the situation in which hospice professionals find themselves in makes the defining of personal attitudes toward hospice care even more difficult. There were an estimated 1.5 million patients who received hospice services in 2012 (NHPCO, 2013, p. 4). Therefore, a large patient population is directly effected by the outlooks which hospice clinicians take towards end of life care. Since the main focus of palliative care should be maximizing the quality of care of the hospice patient, health practitioners must adopt an attitude towards patients which maximizes the probability of the highest quality of life.

Book excerpt: Two separate studies were conducted to investigate hospice palliative care (HPC)volunteers' attitudes towards medical assistance in dying (MAiD), their thoughts aboutcurrent controversies surrounding MAiD, and their perceived need for training regardingMAiD. In Study One, a focus group was held with three HPC volunteers. All threevolunteers were in favour of MAiD and supported their patients' right to request MAiD,as long as they were mentally competent. None of the volunteers knew whether theirHPC program had a policy for volunteers about how to manage inquiries about MAiDfrom patients, patients' family members or caregivers. In Study Two, 27 HPC volunteersfrom two different programs responded to an in-depth survey. Their responses indicatedthat they were supportive of MAiD and in favour of some of the proposed changes to thecurrent MAiD legislation (e.g., advanced requests for MAiD for patients in the earlystages of dementia). More than two-thirds (68%) of the volunteers did not know if theirHPC program had a MAiD policy specifically for volunteers. The majority (83%) of thevolunteers reported that if one of their patients decided to pursue MAiD and requestedtheir presence when MAiD was being administered, they would feel comfortableattending. Limitations of this research, suggestions for future research are discussed, aswell as recommendations for HPC volunteer policy and MAiD training.

Book excerpt: The levels of stress and coping experienced by hospice and palliative care nursing staff was investigated by the use of the Draw-a-Person-in-the-Rain Test (Verinis, Lichtenberg and Henrich, 1974). Thirty-five volunteer female hospice and palliative nurses (PN's, LPN's and CNA's) from Middlesex Hospital, Conecticut and the Hospice of Southeastern Conecticut participated in this study. Participants had an average work record of 6.07 years in the field and had experienced an average of 2.37 patient deaths in one month. Sixty packets were delivered to each site. Packets included a consent form, a demographic questionnaire, the Coping With Death Scale (Bugen, 1981, cited by Robbins, 1994), the Impact of Events Scale (Horowitz, Wilner and Alvarez, 1979) and the Draw-a-Person-in-the-Rain Test. The results suggest there is a correlation between the length of work experience and the number of deaths one can effectively cope within a short period of time.
