The challenge is to provide care to more people and do it in the most efficient manner possible. Several military health care initiatives are planned, or in some cases have been recently implemented, to meet this challenge. According to the Patient Administration Division, Martin Army Community Hospital (MACH) in FY 1986 there was a total of 85,318 outpatient visits to the MACH Outpatient Clinic (OPC) and Emergency room (ER). ER visits accounted for 54,792 or 64% of the total ER and OPC outpatient visits counted. Through July FY 87, there were 68,501 OPC and ER visits counted. During this time, ER visits accounted for 44,996 or 66% of the total visits counted. According to these figures, the reported FY 87 ER utilization was higher than the FY 86 utilization even though the FY 87 utilization data represented only nine months. Higher ER utilization rates' translate to greater consumption of costly ER resources. Are these increases consistent with a true ER need? In an environment of ever increasing resource constraints, health care planners must examine their organizations to determine how resources are being consumed. Book excerpt: The object of this study was to determine the percentage of patients seen in the WBAMCER for conditions triaged as nonurgent during a seven-month period from June to December in 1998. Additionally, the study, utilizing pre-recorded data, desired to determine if 12 independent variables were associated with nonurgent utilization of the ER. The registered nurses on duty triaged a total of 2,173 patients that met inclusion criteria. A large number (n=1260, 58% of sample) were triage to level 4 (nonurgent). Patients triaged to level 3 (treat within 24 hours) accounted for 37.8% (n=821). The remaining level two (urgent) and level one (emergent) accounted for nearly equal percentages of the population at 2.3% (n=49) and 2.0% (n=43) respectively. The typical profile of ER utilization by frequency showed the greatest usage by female active duty dependents between the ages of 18-35 who are CHAMPUS eligible and enrolled in TRICARE Prime. This profile also depicts the typical user as someone who presents with a sprain or strain on a weekday during the hours of 0700-1500. This profile is further typified by a person or sponsor between the rank of E5 and E9 with no third party insurance, lived in zip code 79924 (6 miles from WBAMC) and released without limitation after being treated in the ER. This study strengthens and quantifies the long-standing belief among practitioners and administrators regarding the prevalence of nonurgent use in the ER at WBAMC. These nonurgent patients may be candidates for referral to a more appropriate level of care, typically primary care, without a significant risk for deterioration of their condition. Identifying and quantifying this population is the first step in shaping important organizational initiative aimed at self-care education, utilization of telephone advice and triage, and other out patient medical management programs.

EDs in urban areas are overcrowded and contend with large amounts of patients with non-urgent complaints. To identify reasons for seeking care, a convenience sample of 100 non-urgent patients (ESI triage acuity 4 and 5) presenting to one Midwestern, urban ED was used. This quantitative, descriptive study utilized a 14 item self-administered questionnaire to collect demographic data and to ask a series of closed and open-ended questions about ED use. Specifically, the questions inquired about a patient's usual sources of care, decision-making, and perceived urgency of illness. The Andersen framework of health services utilization was employed to analyze patients' predisposing factors, enabling/impeding factors, and needs for care. Understanding reasons for non-urgent ED use is important to future hospital planning for optimal patient outcomes and effective resource utilization. The results of this study showed patient's seeking care in the ED for non-urgent complaints did so because of a perceived sense of urgency about their condition, a desire for the best care, and the convenience/accessibility of the facility. While a large portion of the sample did not have a primary care provider (PCP), insurance/PCP status were not found to be a significant factor.

Concerns existed among the senior leadership that services in the Emergency Room were not being delivered as efficiently as possible. Freed resources could then be applied toward primary care. The Status Quo model represented a valid model of the KACH ER with simulation patient service times statistically no different than the empirical data (t = - 0.38, df(13369), p=.70). The results show that the KACH ER may be appropriately staffed with one nurse on duty without significantly increasing patient services times. While qualitative arguments must still be considered by the hospital executives in their decision to further transform the KACH ER, the quantitative information gathered from this study will provide a solid foundation for future decisions.
