Book excerpt: Today in the United States, the professional health workforce is not consistently prepared to provide high quality health care and assure patient safety, even as the nation spends more per capita on health care than any other country. The absence of a comprehensive and well-integrated system of continuing education (CE) in the health professions is an important contributing factor to knowledge and performance deficiencies at the individual and system levels. To be most effective, health professionals at every stage of their careers must continue learning about advances in research and treatment in their fields (and related fields) in order to obtain and maintain up-to-date knowledge and skills in caring for their patients. Many health professionals regularly undertake a variety of efforts to stay up to date, but on a larger scale, the nation's approach to CE for health professionals fails to support the professions in their efforts to achieve and maintain proficiency. Redesigning Continuing Education in the Health Professions illustrates a vision for a better system through a comprehensive approach of continuing professional development, and posits a framework upon which to develop a new, more effective system. The book also offers principles to guide the creation of a national continuing education institute.

Over the years, educators concerned with continuing education (CE) for health professionals have either heard or voiced that question in one form or another more than once. But because of the difficulty in measuring the specific effects of a given course, program, or conference, the question has not been answered satisfactorily. Since CE is costly, since CE is now mandated in some states for re-registration, and since its worth has not been proven in for mal evaluation research, the pressure to evaluate remains strong. The question can be partially answered by a more careful definition of continuing education, particularly the goals to be achieved by CEo Another part of the answer depends on the development of a stronger commitment to evaluation of CE by its providers. But a significant part of the answer might be provided through the improvement of methods used in evaluation of continuing education for health professionals. To address this last concern, the Development and Demonstration Center in Continuing Education for the Health Professions of the Univer sity of Southern California organized and conducted a meeting of academi cians and practitioners in evaluation of continuing education. During a three-day period, participants heard formal presentations by five invited speakers and then discussed the application of the state of the art of educa tional evaluation to problems of evaluation of continuing education for health professionals.

Book excerpt: This study explored the perceptions of civil engineers regarding continuing professional competency requirements for license renewal. In this study, continuing professional competency is assumed to have a direct relationship to public welfare and safety. The policy of mandatory continuing education was adopted primarily for this reason. Currently, mandatory continuing education is adopted by 29 states as the method of assuring continued professional competency. Many professionals, however, have not welcomed this mandatory continuing education (MCE) policy. The reason for this opposition is the claimed ineffectiveness of this policy in fostering continued competency. In addition, the regulatory boards and policy advocates have not produced data to support that continued professional competency have resulted from mandatory continuing education. This study is a cross-sectional quantitative type. Primary data was collected using an electronic questionnaire that was e-mailed to participants. The populationof this study consisted mainly of civil engineers and land surveyors. The sampling frame was those engineers who subscribed to the CE News magazine newsletter "CivilConnections." The findings of this study are: (1) The majority of the participating professionals viewed MCE positively. (2) The attitude towards mandatory continuing education was mostly positive. However, the attitude towards MCE requirements being a condition for licensing renewal was not as decisive. The participating professionals were polarized about the issue with 41% against and 43% for, with 16% neutral or undecided. (3) Regarding the issue of public safety and protection being assured by MCE, the results show that participating civil engineering professionals do not consider the impact of MCE as positive. (4) The majority of professionals preferred the voluntary continuing education method as a reassessment approach to assuring continued competency. Periodic re-examination on the other hand was the least preferred and was strongly opposed. (5) The mandating policy clearly impacted the participation level in continuing education. (6) Professional characteristics such as level of education, level of responsibility, field of practice, years of licensing, and age of professionals do not influence participation in continuing education. (7) The majority of employers support continuing education. (8) Finally, 77% of the participants attendedcontinuing education on a voluntary basis.

Book excerpt: This book assembles research findings accumulated over the span of half a century from the Jefferson Longitudinal Study (JLS). This study, initiated in 1970, is the most comprehensive, extensive, and uninterrupted longitudinal study of medical students and graduates maintained in a single medical school. The study was based on the conviction that medical schools have a social responsibility and ethical obligation to monitor the quality of their educational programs, to assess their educational outcomes, and to ensure that their educational goals have been achieved for the purposes of public safety. The JLS has resulted in a large number of publications in professional peer-reviewed journals and presentations in national and international meetings. Some medical schools have expressed interest in learning more about the JLS, requesting copies of the instruments we used in the study, information about how to set up a longitudinal study of medical education, and other needed resources. In response to a request from Academic Medicine [2011, 86(3), p. 404], we prepared and published in that journal a schematic snapshot of the JLS for those interested in a model for the development of a longitudinal study of medical students and graduates. The JLS is well-known to the medical education research communities. A recent Google search using keywords “Jefferson Longitudinal Study” resulted in 1,550,000 hits, an indication of its broad popularity among researchers. At the present time, the JLS database contains academic information, assessments, and educational and career outcomes for 13,343 medical students and graduates of Sidney Kimmel (formerly Jefferson) Medical College of Thomas Jefferson University. There are presently 502 variables in the JLS analytic database. This book presents a collection of 207 abstracts of major publications from peerreviewed journals, books, and book chapters in which data and information from the JLS were used. In this book, we classified the abstracts, based on their primary contents, into the following categories: Admissions of the Applicants to Medical School (e.g., standardized tests, academic preparation, other admission variables). Demographic Composition (e.g., gender, age, race/ethnicity). Performance Evaluations in Medical School (e.g., preclinical and clinical phases). Postgraduate and Career (e.g., assessment of clinical competence in residency training, career choice, specialization, professional activities). Psychosocial Attributes (e.g., personal qualities, indicators of physical and mental well-being). Professionalism (e.g., assessment of elements of professionalism in medicine, such as clinical empathy, attitudes toward interprofesssional collaboration, and orientation.

Book excerpt: Health professionals grapple with a critical challenge: the traditional Continuous Medical Education (CME) model falls short of fostering the unique skills and self-directed learning required for a dynamic career. As medical practitioners navigate a world of new epidemiological models, technologies, and strategies, the need for a transformative solution becomes evident. The Lifelong Learning Journey of Health Professionals: Continuing Education and Professional Development is a book that not only identifies the limitations of existing education models but also provides a comprehensive solution for ushering in a new era of lifelong learning. This compelling book advocates for a paradigm shift towards Continuous Professional Development (CPD), a contemporary concept that embraces non-traditional learning formats. It dismantles the inadequacies of credit-based training by emphasizing the importance of self-direction and self-assessment for adult learners. From core principles for designing a robust CPD system to exploring successful models, alternative credentials, and the role of learning communities, the book offers a holistic approach to reshaping medical education.
