The purpose of this Doctor of Nursing Practice (DNP) project was to reeducate the nurses on the effective use of Teach-back on a cardiac unit at a community hospital in Maryland. The aim of promoting Teach-back (TB) is to assist nurses in becoming comfortable and competent in using Teach-back effectively. The goal is to improve patient education documentation, increase the use of Teach-back for patient education, close the loop on nurse-patient communication, encourage patient's engagement, and enhance health literacy, and improve patient outcomes. This quality improvement (QI) project took place over a four-week period. Pre-and post-intervention questionnaires were given to cardiac nurses at the hospital before and after the implementation of the educational intervention. A pre- and -post-test design was used to evaluate the increase use of Teach-back. Descriptive analysis was performed to determine if the Teach-back reeducation of the nurses would improve the use of Teach-back. The findings indicated a considerable improvement in the nurses' competence and confidence in using Teach-back for patient education after the intervention. The findings can contribute to positive change by improving the quality of cardiac care, preventing readmissions, and reducing healthcare costs in cardiac facilities. Book excerpt: Cardio-oncology is a medical subspecialty concerned with the diagnosis and treatment of cardiovascular disease (CVD) and organ failure mediated by micro- to macro-circulation defects in cancer patients and survivors. The risk of CVD in cancer survivors is eight times higher than the general population, and the relative risk of coronary artery disease and heart failure (HF) is 10 times and 15 times higher, respectively, compared to their siblings without cancer. It is important to note that cancer treatments including chemotherapy and radiation can lead to both short- (< 1 year) and long-term (> 5 years) cardiovascular complications. Previously, Dr. Edward T.H. Yeh initiated the MD Anderson Practice (MAP) project, and published 16 MAPs to provide the current best practice we considered at UT MD Anderson. As shown in these MAPs, one of the key issues of cardio-oncology practice and science is how we can allow cancer patients to receive maximum and uninterrupted treatments for cancer while protecting them from cancer therapy-mediated cardiovascular complications. Therefore, it is crucial to understand not only pathophysiology of CVD, but also mechanistic insights of how each cancer treatment can control cancer growth and metastasis. Without having the profound knowledge in the pathophysiological, clinical and epidemiologic aspects of cardiovascular complications of existing cancer therapy, it is impossible to establish the strong evidence-based strategies and approaches for both short- and long-term cardiovascular complications after cancer treatment. In addition, we believe that cardio-oncology science should not be restricted to deal with the side effects of each specific cancer drug, because many common cardiovascular phenotypes among the different cancer treatments are also observed. This Research Topic provides a comprehensive overview from both of the current research and practice, and a platform for obtaining the logical and up-to-date treatment and prevention of CVD in cancer patients and survivors.

While many elements may contribute to patient satisfaction with discharge education, use of the teach-back method has been found to improve retention of discharge instruction, as well as increase patient confidence in chronic disease management and self-care at the time of discharge. Patient satisfaction with discharge education is a notable component of hospital reimbursement by national standards, as measured by the Hospital Consumer Assessment of Healthcare Providers and Systems survey (HCAHPS) results. Following education of nursing staff participating in use of discharge teaching, patient satisfaction with discharge instructions taught using the teach-back method was measured via HCAHPS for a period of two months. Results suggested potential correlation to improved HCAHPS scores for the question “When I left the hospital, I had a good understanding of the things I was responsible for in managing my health,” associated with use of teach-back at time of discharge.

Methods: Electronic searches of CINAHL and PubMed were performed through James Madison University libraries. Articles selected for inclusion were evaluated for study design, relevance, and sample size. Results: Three studies met eligibility criteria, these studies showed statistically significant evidence that the teach-back method did reduce readmission rates in heart failure patients. Discussion: Hospital education programs should include the teach-back method in their existing education program in order to reduce readmission rates. Further studies should be done on a more diverse population and the long term results of the teach-back method. The impact on the critical care environment and the health care system, as a whole, is significant from both a cost and burden to the system perspective. There are 6.5 million hospital days a year and nearly $40 billion dollars in yearly health care costs attributed to heart failure in the United States. There are more Medicare monies spent for diagnosing and treating heart failure than any other Diagnosis Related Group. There is a 24% hospital re-admission rate for this diagnosis which leads to financial implications for health care systems.The human cost is also significant. Less than half of Americans diagnosed with heart failure survive greater than 5 years. The ongoing health care needs and cost of this chronic disease takes a significant toll on patients' finances, time and quality of life. Over $2.9 billion dollars is spent annually on the pharmaceutical management of heart failure in the United States. This diagnosis is the leading cause of hospitalization for patients who are 65 years of age and older.
