Regardless of the point of entry, new nurses can struggle with role responsibilities, heavy patient loads, incivility, and staffing shortages. Preceptorship experiences before entering the professional nursing practice have been shown to facilitate an easier transition for new nurses and can help with socialization, communication, skill acquisition, and role modeling. The literature review reveals that the use of preceptors and preceptorship experiences are useful in assisting new nurses transition into professional practice. However, little research has been performed using preceptorship experiences with Associate Degree (AD) nursing students before graduation in the United States. The purpose of the study is to explore whether a 48-hour preceptorship experience with a practicing registered nurse aids in the transition of senior AD nursing students into the role of registered nurse. The Transformative Learning Theory (TLT) was used to guide the research project, and a descriptive qualitative design was utilized for data attainment. Following the conduction of in-depth interviews, of 13 participants, the information was transcribed and coded for themes. The themes were then correlated with the study's research questions. The data obtained from the participants revealed that preceptorships experienced as a nursing student are useful in the transition from student to registered nurse"--Abstract from thesis.

Book excerpt: The aim of this qualitative, phenomenological study was to explore the lived experience of new graduate nurses who completed a Student Nurse Associate Program during the summer between junior and senior year of their baccalaureate nursing program. This pre-licensure program was designed to support transition to professional practice by fostering confidence and competence, thus reducing fear and anxiety. Purposive sampling with the use of the snowball technique yielded data saturation with a of sample of 12 nurses. Data were generated by asking the participants to respond to four research questions during individual interviews. The naturalistic inquiry of Lincoln and Guba and the data analysis method of Colaizzi were germane to the study. This study revealed eight themes which were significant for new graduates to facilitate a smooth transition to professional practice. These included: 1. Confidence: Growth during the program; 2. Delegation: A difficult challenge; 3. Communication: Learning how to interact with patients, families, and other healthcare professionals; 4. Competence: Perfecting basic skills; 5. Time Management: Developing organizational systems; 6. Work Environment: Learning practices, policies, and people; 7. Prioritization: Critical thinking in action; and 8. Collaboration: Becoming a team player. The participants reported improved abilities in all areas with the exception of delegation and prioritization; these two areas were not mastered to the same extent. This study has contributed to knowledge development regarding transition to practice although more work is needed with a larger and more diverse sample. Pre-licensure and post-licensure programs should be compared in terms of cost, efficacy, and outcomes.

Instruments: The Casey-Fink Tool measured perceptions of student nurse practice readiness and their preceptors’ perceptions of student readiness following the final clinical capstone experience. The survey instrument is a three-factor set of correlated subscales: demographics/clinical experience, competency skill performance, and professional identity of the student nurse. Procedure: This STEP study utilized collected data from the Casey-Fink Tool survey data obtained during the spring 2018 clinical capstone course from 100 students and preceptors. Analysis: A descriptive analysis of the Casey-Fink Tool survey data, including exploratory factor analysis, was used to identify subscales and study findings. Nursing Implications: This STEP study advances the future of nursing education by exploring curricular methodologies to aid in the preparedness and practice readiness of the student nurse upon graduation.

Book excerpt: Preceptors are expected to facilitate students' learning and the transition of new graduate nurses in the clinical area but may not be fully prepared for the role. The literature abounds with evidence supportive of considerable preceptor development yet researchers have been slow to explore preceptors' perceptions of their capabilities and how they influence their willingness and behaviors. This qualitative descriptive study explores nurse preceptors' perceptions of their self-efficacy when precepting senior practicum students. Albert Bandura's self-efficacy theory guided this study. Individual semi-structured interviews were conducted with eight participants. Qualitative content analysis was used to analyze the data. Findings included two major themes, experiencing self-efficacy as a nurse preceptor and developing self-efficacy as a nurse preceptor. Previous preceptor experience, years of clinical practice experience, practicum experience as students and workplace supports were identified as salient sources of preceptor self-efficacy. Findings provided insight into the current state of self-efficacy in the preceptor role and contributed to addressing a gap in the literature. Findings have implications for strategies for development of self-efficacy and provided insight into other potential resources for supporting nurse preceptors in the development of their knowledge and behavior. The findings can inform the groundwork for research on further consideration for the implementation of structured transition programs in Canada.

Book excerpt: This qualitative instrumental case study investigates the process of tacit knowledge sharing that occurs within a nurse preceptor program. Tacit knowledge is implicit knowledge that is based on lived experience and cannot be codified. Many organizations have struggled with how to successfully manage tacit knowledge that is important and valuable to their firm. The field of nursing has developed a professional practice called preceptorship: When a nurse progresses in their career, their duties expand from solely providing patient care to include the mentoring of junior, less experienced nurses. This preceptorship model is built upon the foundation of sharing tacit knowledge in the organization, yet little research can be found in the literature about why this activity provides value as a learning tool. This instrumental case study was driven by the research questions of how preceptors determine the most appropriate organizational "know-how" to be shared; how less experienced nurses value or not value the knowledge they receive from their preceptor; and how the junior nurses choose to accept, modify, or not accept the knowledge to add to their own existing lived experience. This research includes an investigation of the literature via the conceptual framework: expert research on the topic of tacit knowledge, research into the organizational value of tacit knowledge, and a study of various educational methods for tacit knowledge management. Findings from this research demonstrate that while the preceptorship process is a standard learning process within nursing, the sharing of tacit knowledge is still relatively informal. The study suggests methods in which learning and development can incorporate simple tools, such as checklists and reflection guides, that can be leveraged within both clinical and non-clinical organizations as a way to ensure critical tacit knowledge is shared appropriately. This research will add to the limited existing literature on tacit knowledge sharing in organizations to investigate ways in which this phenomenon can be incorporated into employee educational practices that focus on managing critical organizational tacit knowledge. The limited number of research studies completed to date have primarily focused on student learning and end-of-program outcomes. Additionally, there is a dearth of research on the experience of nurse educators who have transitioned from traditional to concept-based teaching. The aims of this study were to: (1) describe the experience of nurse educators who transition to concept-based teaching, (2) identify whether a common process of transitioning to concept-based teaching exists amongst study participants, (3) identify facilitators and barriers faculty encounter in the process of transitioning to concept-based teaching, and (4) compare study findings to the managing transitions model to determine ares of congruence and the need to refine or expand the model. A qualitative description design with responsive interviewing as the principle method was used to elicit data in this study. the purposive sample of 19 participants consisted of faculty who taught in associate degree programs in the U.S. that deliver a concept-based curriculum. Participants were recruited from an Internet nurse educator email discussion group as well as an e-mail invitation sent to deans/directors of associate degree programs that deliver a concept-based curriculum. Email interviews were conducted between March and April, 2016. Content analysis was used to derive initial codification of interview data, as well as within and between interview comparison of codes to arrive at theme and sub-themes. Five major themes with associated sub-themes emerged from the data analysis that represented the participant's description of the transition experience. The major themes were: recognizing fears, facing conflict, working out of one's comfort zone, seeing successes, and self-talk and questioning self. A purpose of participant transition from traditional to concept-based teaching emerged consisting of five phases that included: responding to the decision to adopt a CBC, developing the curriculum, implementing the curriculum, seeing results, and reflecting back-a continuing journey. Self-talk and questioning self transcended all the phases and provided a framework for reflection that assisted the individual with moving forward wit the transition process. Participants described emotional reactions such as fear of failure and, questioned whether they could be successful in working in a team which were generally analyzed through self-talk. Seeing positive student outcomes helped to convince faculty of the decision to move to a CBC while others continues to question the effectiveness of the change. Four major themes arose that best represented facilitators that assisted the participant throughout the transition process. These facilitators included: identifying inner support, building team cohesiveness, learning from and sharing knowledge with others, and seeing positive student outcomes. Three major themes emerged that represented barriers throughout the participant's transition to concept-based teaching. These major themes included: dealing with personal struggles, lacking support within the school nursing, and lacking support outside the school of nursing. Similarities arose that were consistent with the three stages of managing transitions model. future refinement of the managing transitions model may include reflective question cues to assist individuals as they transition through the phases of change as well as guidance to assist those who continue to resist change. Implications for theory, education, policy, practice, and research are also discussion.

Book excerpt: The purpose of this study was to investigate the transition experience of new graduate nurses during their first work experience after graduating from a baccalaureate undergraduate nursing program. A case study approach was used to examine a community hospital in Connecticut. A random sample of baccalaureate prepared new graduate nurses who graduated in May 2007, and were in the first year of working as registered nurses, was chosen. The research question examined how new graduate nurses perceived the supports provided within their transition experience. The study examined the supports through Benner's (1982) developmental theory of nursing. The four supports included: support during orientation, support from experienced nurses, supports to develop time management skills and support from their undergraduate nursing education. The four research questions addressed each of these supports. The study used three methods of data collection. Twelve new graduate nurses completed the quantitative survey, eight participated in the individual interview and four new graduate nurses participated in the focus group interview. The conclusions included the following: new graduate nurses were supported by the learning environment of the orientation; new graduate nurses perceived a supportive network was available to them within the hospital environment; time management skills are essential for the transition to entry nursing practice; new graduate nurses were assisted by their undergraduate school experiences during the transition period. Recommendations for practice have implications for making new graduates successful in a nursing career. Hospital orientations should provide preceptors who encourage autonomy and are consistent and provide emotional support and need to provide support in the transition period by mentoring and encouraging guidance from experienced nurses. Hospitals also need to provide clinician guidance and organizational resources to assist with time management, and offer intern programs to students prior to graduation. Undergraduate nursing programs should incorporate more clinical time to help the new graduate nurse transition into nursing practice. Recommendations for further research might involve a qualitative in-depth study regarding time management as well as an in depth case study examining new graduate nurses perceived level of support during the orientation period.
