Material and Methods: Fifty-eight patients received dental implant therapy to restore missing teeth in one area of the maxilla or mandible at the UAB Department of Graduate Periodontology. Residents in the program placed microtextured implants with a microtextured collar with microgrooves (test group) and microtextured implants with a machined collar (control group). Sites were randomized to the test group or control group. Study visit protocol was followed and implants were restored by off-site dentists. Sixty-nine (69) implants were assessed at one year for a final study visit. The primary outcome was to determine the difference between crestal bone levels in the test versus the control group implants as measured by differences in radiographic crestal bone heights at baseline and one year. Data collected consisted of periapical radiographs and clinical photographs that were taken at specific time points throughout the study period. Statistical analyses using individual t-tests and ANOVA were performed to analyze the data. Although nano-surface modifications increase osseointegration, initial implant stability at time of placement is an important clinical factor in implant survival and is considered a prerequisite for early or immediate loading protocols.Aim:This RCT evaluates the 3 years outcome of modifications of the OsseoSpeed implant (Dentsply Sirona Implants, Mu00f6lndal, Sweden) in terms of survival, bone level and peri-implant health. Alterations in the OsseoSpeed EV consisted of changes in dimensions of body shape, drilling procedures, drill design and abutment connection compared to the original OsseoSpeed TX.Material and methods:120 partially edentulous patients, enrolled in five clinics, were randomized to test (EV) and control (TX). 59 subjects received 79 EV and 61 subjects received 87 TX implants respectively. Cemented permanent restorations were placed on average 9 weeks later on titanium stock abutments. Peri-implant bone level changes, plaque, probing pocket depths and bleeding indices were assessed at restoration placement (baseline), and thereafter at 6, 12 and 36 months.Statistical comparison between EV and TX were done with Mann Whitney U-test with p0.05 as significance level.Results:104 subjects (50 EV, 54 TX) were assessed (66 and 75 implants respectively) after 3 years. Three patients discontinued due to failure, 7dropped out and 6 were not available. The groups were comparable (p0.05) regarding age, gender, race, smoking habits, tooth loss reason, periodontal history, implant location, length and diameter. Mean implant insertion torque was 30u00b113 Ncm and 22u00b19 Ncm for EV and TX, indicating improved stability for the modified protocol. Five of 79 EV implants were lost (93.6%, 4 prior and 1 post-loading). One of 87 TX implants failed prior to loading (98.9%). This difference was not statistically significant. Regarding crestal bone change, a mean gain was observed from baseline to 3 years of 0.29mm (SD 0.72) and 0.21mm (SD 0.48) for EV and TX respectively. Probing pocket depth at 3 years was 2.72mm (SD 0.73) vs. 2.82mm (SD 0.73). Plaque was present on 8.3% and 9.0% of surfaces, and bleeding on probing remained stable with a mean of 17.4% and 18.0%. None of the parameters were statistically significantly different between groups (p>0.05).Conclusion:Peri-implant health did not differ between OsseoSpeed EV and TX implants. A gain in crestal bone levels was observed in both groups but EV implants indicated an improved primary stability.Although not possible to determine, it cannot be excluded that in three of the four early EV failures, a deviation in the cortical bone preparation may have contributed to the outcome.Experiences from this study led to modifications of the suggested drilling protocol including widening of the spongious bone preparation to facilitate the implant placement within recommended torque value.

More specifically, we will discuss randomized clinical trial designs that investigate the improvements provided by conventional and immediate occlusal loading protocols using mandibular overdentures retained by narrow dental implants in populations of completely edentulous patients with prolonged time since edentulism. Most clinical studies available present different methodological characteristics, and many do not show aspects aligned with current research practices, especially where immediate loading is concerned. Immediate loading requires high primary stability, which can be clinically assessed by insertion torque values provided by the surgical motor and by the implant quotient stability value through resonance frequency analysis. Controversies are being observed in regard to this clinical concept, both related to decisions based on the insertion torque obtained during implant installation and by the implant quotient stability. Furthermore, immediate loading induces more strain to the peri-implant bone earlier, but allows faster transition to implant-retained overdentures, improving the oral-health related quality of life. In addition, the use of the CONSORT Statement is discussed to facilitate randomized clinical trial planning in such target population. Finally, some clinical comparisons in this research field are presented alongside common logistical problems encountered during this type of randomized clinical trial development.

However, the use of short implant in this treatment option has not been performed. The purpose of the study was to compare the clinical outcomes and survival rates of immediately-loaded short implants and conventional length dental implants (Pw Plus dental implant system, Thailand) in replacing mandibular molar teeth.Materials and methodsThe clinical study was approved by the Human Experimentation Committee. Fifty implants in 46 patients were included in the study. Twenty five short dental implants (6 mm.) and 25 conventional dental implants (10 mm.) were randomly placed as single tooth replacements in the edentulous molar areas of the mandibles. Provisional, CAD-CAM, ceramic block (Shofu HC) crowns were cemented to the abutments of the implants, and immediately loaded. The clinical parameters, such as insertion torque (IT), resonance frequency analysis (RFA), marginal bone level changes, and complications, were recorded and statistically analysed.ResultsTwo short implants and two conventional implants were excluded from the study due to insufficient insertion torque (less than 35 Ncm). Two short implants disintegrated, and one conventional implant failed. The accumulate survival rate of short implants was 91.30%, compared to 95.65% for conventional implants. However, there was no significant difference between the two implant types (P (Fisheru2019s exact) = 1.00). Only minor complications were found (three provisional crown fractures with short implants and two with conventional implants).The differences in IT among the two implant types were not significant (P =.264). There was no significant difference in ISQ value for short or conventional implants between baseline (short: 73.86 u00b1 2.38 mm, conventional: 75.05 u00b1 3.26mm, P=.088) and four months after loading (short: 72.37 u00b1 1.35 mm, conventional: 72.89 u00b1 1.87 mm, P=.165). The mean change in marginal bone level four months post-loading was 0.24 u00b1 0.29 mm for short implants and 0.26 u00b1 0.29 mm for conventional implants ; there was no statistical difference between the two implant types.ConclusionsWithin the limitations of this study, the findings seem to confirm that the immediate loading of short implants is a viable option, comparable to conventional length implants in terms of implant survival, marginal bone loss and ISQ value.

Heterogenous results were published with respect to the influence of a conical versus straight implant collars design on marginal peri-implant bone loss (MBL). The first observations on conical implants with a machined neck reported higher values of MBL compared to straight implant fixtures. However, further investigations reported controversial outcomes, supporting no difference in MBL between conical and straight implant collars on one hand, and significant differences on the other.u2028The purpose of this randomized split-mouth clinical trial was to assess whether two implant fixtures with identical surfaces, prosthetic connections, coronal diameters and lengths, exhibit any significant difference in: a. MBL; b. implant success and survival rates; c. peri-implant probing depth (PD) and bleeding on probing (BOP), after a 3 year follow-up period. Methods and Materials: Ten patients presenting with single tooth symmetric posterior edentulous spaces, received a straight collar implant in one site (OsseoSpeed TX 5.0 S Astra Tech Implant System), and a conical collar implant in the symmetric contralateral one (OsseoSpeed TX 5.0, Astra Tech Implant System). All edentulous sites did not require bone regeneration in order to place 5mm implants with a minimum length of 9mm, as the inclusion criteria required a minimum residual bone height of 12mm and a minimum residual bone width of 7mm per site. All patients received implants with matching lengths in the two symmetric sites. Patients were followed-up for 36 months to evaluate: a) MBL; b) implant survival and success rates according to Albrektsson et al. criteria (1986); c) peri-implant probing depth (PD) and bleeding on probing (BOP).Results: All patients were evaluated at their last recall with a clinical evaluation and a periapical radiograph, standardized through the customization of a Rinnu2019s film holder. No implant failed to osseointegrate or required to be removed. All patients maintained adequate plaque and bleeding on probing indexes during the follow-up period. The evaluation conducted on the periapical radiographs through an image processing and analysis software (Image Ju00ae) revealed a marginal peri-implant bone loss ranged from 0 to 0.5 mm in 80% of cases and from 0.5 to 1 mm in 15%. In a single implant (control group), a 1.9 mm vertical bone resorption was observed. Thus, the survival and success rates according to Albrektsson et al. criteria were 100% and 95%, in the test and control group, respectively. No significant differences were observed between the two sites, with respect to MBL, peri-implant probing depth (PD) and bleeding on probing (BOP).Conclusions: Despite the short follow-up period and the limited external validity of our results, this study suggest that large diameter implants with a straight or conical implant collar macrostructure are equally reliable in the rehabilitation of single tooth posterior edentulous spaces.

Materials and Methods In total, 158 patients were randomly assigned either to the immediate loading group (n = 81) or to the delayed loading group (n = 77). All implants were loaded with an overdenture retained by a ball attachment. Results After 5 years, 102 patients attended the follow-up investigation. Immediately loaded single implants in the midline of the edentulous mandible revealed a statistically significant lower survival rate than implants loaded conventionally over an observation period of 5 years. In the immediate loading group, 9 implants failed within the first three months of implant loading. No further implant loss was recorded for this group. Two implants failed in the delayed loading group, whereas one implant had to be removed during second-stage surgery and the second five years after implant loading. Non-inferiority of the survival rate of the midline implant of the immediate loading group, compared with the delayed loading group, could not be shown (p = .79, CI immediate loading: 74.9%; 100.0%, CI delayed loading: 73.0%; 100.0%). The observed difference in implant survival between the two treatment groups over time was statistically significant. Conclusions The results of the present study indicate that immediate loading of a single mandibular implant in the edentulous mandible should be considered only in exceptional cases

However, the extensive use of antibiotics in health care has been debated due to adverse effects and bacterial resistance. Furthermore, the impact of postoperative antibiotics on peri-implant bone level is still not clear. Objectives: The primary objective of this pilot study was to produce preliminary data and to assess whether giving postoperative antibiotics after implant placement over seven days would influence peri-implant crestal bone levels after four months in healthy patients undergoing platform-switched implant placement. The secondary objectives were to evaluate postoperative pain severity, surgery-associated morbidities, and one-year implant survival rate. Methods: Thirty-eight individuals were enrolled in a double-masked two-arm randomized clinical trial. Participants in the intervention group received 2 g of amoxicillin one hour before implant placement followed by a seven-day post-operative course of 500 mg of amoxicillin. Participants in the control group took only 2 g of amoxicillin before surgery and an identical placebo postoperatively. The changes in mesial and distal crestal bone level (primary outcome) were measured at baseline and four-month follow-up using standardized periapical radiographs. Pain severity and surgery-associated morbidities (secondary outcomes) were evaluated by clinical examinations and self-administered questionnaires. Implant survival rate was assessed at the one-year follow-up. Descriptive and bivariate analyses were used to analyze the data. A P value ≤ 0.05 was considered statistically significant. Results: Thirty-seven participants completed the study (mean age: 57.4 ± 11.3 years). The mean combined peri-implant crestal bone level change for the intervention and control group was -0.29±0.36mm and -0.11±0.35mm, respectively (n=37 participants). The differences between groups for mean combined crestal bone level change and pain severity were not statistically significant (P> 0.05). Due to small width of MDIs, patients do not need demanding surgical procedures and hence are well accepted by elderly patients or those with deteriorating general health status.Aim: Aim was to test the behavior of dental implants being both, slim and short (short MDIs) for complete overdenture (OD) retention by digital image correlation (DIC), and afterward, in clinical trial, to rehabilitate patients with less than 10 mm of mandibular height by means short MDIs OD retention. Materials and Methods: Based on CBCT scans of patients with extremely atrophied mandible (interforaminal height 10 mm), 6 models of an average extremely atrophied mandible were virtually built and 3D printed. In each model 4 short MDIs (length 6, 8 or 10 mm, width 2 or 2.5 mm; Dentium, South Korea) were inserted interforaminally and CoCr reinforced ODs were loaded with 150 N on denture molar region bilaterally,unilaterally and the on anterior region. Precise measurement of strain and displacement was conducted using the Digital Image Correlation Method (ARAMIS software v6.2.0; GOM-Optical Measuring Techniques), which approved the protocol. Patients (D or E classification according to Leckholm and Zarb) received 4 short MDIs by a flapless surgical technique. MDIs were early loaded using matrices with O-rings mounted in mandibular ODs. Marginal bone loss (MBL), technical complications, OHIP-EDENT and the chewing function questionnaire(CFQ) were assessed prior treatment, after OD delivery and after 2-years in function.Results: Strain values on all models ranged from 0.45% to 0,92% with the highest displacement and strain values observed around MDIs and on the zone of bone-denture contact. Analysis of the major strain fields demonstrated that the strains of the mandibular bone were the result of vertical displacements of the ODs. DIC showed in all loading stages identical spreading direction with the maximum strain value within the denture-bone complex. In clinical trial 20 patients received 4 short MDIs and respective ODs. Two implants fractured during insertion and 4 failed after placement. All other implants were stable, without biological complications. MBL was 0.26 u00b1 0.35 mm with 95% survival rate and 8 O-ring replacements. The OHIP-EDENT and the CFQ scores significantly decreased after treatment (p.05) and did not change significantly through the 2-years of observation (p>.05).Conclusions: DIC is a novel method in dentistry providing high precision and accurate repeatability. Measurements on 3D printed models demonstrated low strain and displacement values around short MDIs and in mandible, which enabled proceeding with clinical trials. Within the limitations of this study, insertion of 4 short MDIs (length 6 or 8 mm) in extremely atrophied mandibles for retention of mandibular ODs showed encouraging results in the first two years of function.

Recent short-term studies have shown how narrow diameter implants are useful in patients with type-2 diabetes mellitus patients (DM2). Aim/Hypothesis: To evaluate prospectively the long-term (2-years) of the performance of the Titanium-Zirconium alloy implant (Roxolidu00ae Straumannu00ae) with a narrow diameter (3.3 mm) and with hydrophilic surface (SLActiveu00ae) in DM2 in unitary restorations, compared to a control group of patients without diabetes mellitus, and to analyse the success rate and survival, the marginal bone loss and the influence of the glycemia level of the patients in the osseointegration, by measuring the glycosylated hemoglobin (HbA1c). Materials and Methods: 28 patients, 14 DM2 (test group) and 14 without diabetes mellitus (control group). With a unitary tooth absence was rehabilitated in incisors, canines or premolars with a titanium-zirconium alloy implant of narrow diameter (3.3 mm) Standard RN (2.8 mm high platform) SLActiveu00ae Roxolidu00ae, (Institut Straumann AG, Basel, Switzerland). Implants were placed in healed bone (more than 8 weeks post-exodontia), non-submerged healing and loaded 2 months after surgery. The success and survival rate was assessed according to the criteria of Buser et al., 1991. Implant follow-up was performed at 2 years, with standardized radiographs evaluating marginal bone loss and by measurement of glycemic control of patients over time (glycosylated hemoglobin -HbA1c-). Results: The study power is 87%. The success and survival rate of the implants was 100% in both groups. The mean marginal bone loss from implant placement to 2 years was -0.53 u00b1 0.54 mm. There were no significant differences between the two groups (-0.48 u00b1 0.50 mm, in the test group (DM2) and -0.43 u00b1 0.47 mm in the control group). No marginal bone loss appeared in any of the groups measured from the implant platform to the bone tissue at two years. The values of HbA1c were maintained on average in 7.10% in patients with DM2. Moreover, no significant correlation between HbA1c levels and obtained results was observed. Conclusions/Clinical Implications: Straumannu00ae Roxolidu00ae SLActiveu00ae narrow diameter (3.3 mm) titanium-zirconium alloy implants, placed in both well-controlled DM2 and healthy patients, have the same behavior and outcome. Given the lower surgical trauma to DM2, the indications for this type of narrow-diameter implants could be widened. Although short- and long-term results are already known, more controlled clinical trials would be necessary to corroborate these results. The results demonstrate that with good glycemic control, narrow implants with a hydrophilic surface could be useful in the treatment of DM2 patients.

The primary aim of this randomized, controlled, blinded clinical trial was to compare the hard and soft tissue response of a standard vs. a laser grooved implant collar using objective and subjective clinical parameters. Methods. Ten patients received a single tooth implant in the maxillary anterior that was bordered by at least one tooth. Seven test patients received a laser grooved collar implant while three positive control patients received a standard smooth collar implant in a healed edentulous space. A fixture level impression was taken at implant placement to facilitate provisional placement at the 2 month uncovering visit. At 4 months a second fixture level impression was taken and a final restoration was fabricated. Final measurements were taken following crown placement at 6 months. Results. Implant interproximal crestal bone loss was similar for the laser-grooved collar and the standard collar implants from placement to 6 months. For the laser-grooved group, 0.3 +/- 0.6 mm for the mesial and 0.1 +/- 0.6 mm for the distal was lost. 0.0 +/- 0.9 mm for the mesial and 0.3 +/- 0.6 mm for the distal was lost, for the standard group. Using the Jemt Papilla index, the laser group had > 50% papilla present in 71% of sites (10 of 14) vs. 83% (5 of 6) for the standard group. Facial recession was minimal for both groups, 0.3 +/- 0.5 mm for the laser-grooved group and 0.3 +/- 0.6 mm for the standard collar group. Gingival margin harmony was present 71% (5/7) of the time in the laser group 67% (2/3) of the time for the standard group. Papilla harmony was achieved in one case for the laser-grooved group (1/7) and not achieved in any of the standard cases (0/3). The Pink Esthetic Score, an index of soft tissue esthetics, was 10.4 +/- 1.5 for the laser-grooved implants and 11.7 +/- 0.6 for the standard collar implants. The mean osseous crest to contact distance was 4.5 +/- 1.0 mm on the mesial and 4.0 +/- 1.6 mm on the distal for the laser-grooved group and 4.2 +/- 0.6 mm on the mesial and 3.5 +/- 1.0 on the distal for the standard group. Conclusions. Interproximal bone loss was similar for both the laser grooved and the standard implant collar. Soft tissue healing in terms of papilla formation and recession was similar for both groups. Subjective patient assessment of esthetics using the Visual Analog Scale was higher for the laser grooved collar group.

Bone and soft tissue remodeling can also be influenced by dental implant materials, designs and surface modifications1. Several studies on implants with the same design but with different surfaces such as hybrid implants are lacking at the present time2. Aim: The purpose of this preliminary prospective study was to investigate the influence of soft and hard tissue response to a hybrid dental implant with a platform switch inserted at 6 months after implantation in the posterior mandible.MATERIALS AND METHODS:From August 2016 to January 2017, 16 consecutive patients were enrolled (8 men and 8 women; mean age 51.2 years with 32 implants, needing two implant-supported restoration in the posterior mandible. An impression was taken 6 weeks later, and a finall restoration was positioned. Follow-up examinations, clinical parameters, photographs, and intraoral digital radiographs were made at baseline and six month after definitive restorations to evaluate the marginal bone level changes. Cast analysis were performed to detect soft tissue vertical and horizontal changes. Implant stability was measured with an (Osstellu00ae) ISQ implant stability meter.RESULTS: Postoperative healing was uneventful. All implants were clinically osseointegrated. Changes of marginal bone level and soft tissue dimensions were recorded at implant installation and at 6 months. No bone defect around implants and there were no signs of inflammation. Soft tissue levels significantly improved between baseline and 6 month follow up. Periodontal parameters never exceeded the physiological levels. The ISQ readings obtained the high stability at 6 month follow up.CONCLUSIONS: Within the limitations of this prospective study, the analyzed hybrid implants obtained favorable results in the posterior mandible. Long term randomized controlled clinical trials on hybrid implants are necessary to confirm these results.
