Book excerpt: Excerpt from A New Synopsis of Nosology, Founded on the Principles of Pathological Anatomy, and of the Natural Affinities of Diseases The word Nosology, according to its derivation, simply means a discourse on diseases; but, by conventional acceptation, it is used to signify their systematic classi fication. From the earliest periods in the history of medicine, when diseases came to be treated of in books, irregular vestiges of this department of medical science are to be found; and hence we observe the father of our art, Hippocrates, arranging diseases in groups, founded on some real or supposed analogy between them, or treating of them collectively, as they affect particular organs or parts of the body. As a know ledge of the nature of diseases became more correct and extended, we find, in later successive writers, greater precision and method in their classification but Nosology, as a distinct branch of medical science, takes its origin from a period comparatively modern. Plater, who wrote in the beginning of the seventeenth century, was the first to sketch the penumbra of what is now understood by a synoptical classification of dis eases. In his work on the Practice of Medicine, pub lished at Basle in 1602, this author succeeded in making a first step towards a systematic arrangement of dis eases, especially those affecting the nerves of sense and motion. In the case of abortion, for example, the laws have changed radically, and the widely pub licized recent conviction of Dr. Edelin in Boston has done little to foster a moral consensus or even render the exact status of the law beyond reasonable question.

In the second part of each chapter, the case study is discussed within the context of relevant literature and a detailed picture of the state of the art concerning the psychopathological understanding of the phenomenon at issue is provided. An Experiential Approach to Psychopathology, and the method it proposes, may be considered the result of convergence of classic phenomenological psychopathological concepts and updated clinical insights into patients’ lived experiences. It endorses three key principles: subjective phenomena are the quintessential feature of mental disorders; their qualitative study is mandatory; phenomenology has developed a rigorous method to grasp “what it is like” to be a person experiencing psychopathological phenomena. Several reasons can be cited for this increasing interest. Primary among these has been the rise of the chronic, non-infectious diseases as important causes of morbidity and mortality within Western populations during the 20th century. Generally speaking, the chronic, non infectious diseases are strongly influenced by lifestyle variables, which are themselves strongly influenced by social and cultural forces. The under standing of the effects of the behavioral factors in, say, hypertension, thus requires an understanding of the social and cultural factors which encourage obesity, a sedentary lifestyle, non-compliance with anti-hypertensive medica tions (or other prescribed regimens), and stress. Equally, there is a growing awareness that considerations of human behavior and its social and cultural determinants are important for understanding the distribution and control of infectious diseases. Related to this expansion of epidemiologic interest into the behavioral realm 'has been the development of etiological models which focus on the psychological, biological and socio-cultural characteristics of hosts, rather than exclusive concern with exposure to a particular agent or even behavioral risk. Also during this period advances in statistical and computing techniques have made accessible the ready testing of multivariate causal models, and so have encouraged the measurement of the effects of social and cultural factors on disease occurrence.
