Book excerpt: This Review sets out to propose a structure for the funding arrangements for the whole spectrum of health research, with the objective of obtaining the maximum benefit from research success and, where possible, eliminating duplication of effort. The Review found, however, that the UK is at risk of failing to reap the full economic, health and social benefits that the UK's public investment in health research should generate. There is no overarching UK health research strategy to ensure UK health priorities are considered through all types of research and there are two key gaps in the translation of health research: (i) translating ideas from basic and clinical research into the development of new products and approaches to treatment of disease and illness; (ii) implementing those new products and approaches into clinical practice.The Review also found that the wider funding arrangements for supporting translation of ideas from conception to practice could be more coherent or comprehensive and, where arrangements exist, they do not function well. The Review identified cultural, institutional and financial barriers to translating research into practice in the publicly funded research arena. But it also found that, in the private sector, the pharmaceuticals industry is facing increasing challenges in translating research into health and economic benefit. The Review has sought to make recommendations that will increase the translation of R&D into health and economic benefit for the UK, both in the public and private sectors. The Review recommends that the Government should seek to achieve better coordination of health research and more coherent funding arrangements to support translation by establishing an Office for Strategic Coordination of Health Research (OSCHR).

We are bombarded with thousands of potential resources and c- tradictory advice. Should we seek wisdom in a self-help book, an online site, a 12-step group, an engaging autobiography, a treatment manual, an inspiring movie, or distance writing? Should we just do it, or just say no? Work toward change or accept what is? Love your inner child or grow out of your Peter Pan? I become confused and discouraged just contemplating the choices.

Book excerpt: Conversely, those adults with high panic-fear (anxiety) reported lower quality of life, lower self-efficacy, lower optimism and higher psychological stigma. Those participants with medium panic-fear also had middle range scores in the other variables. This defined three distinct patterns of behaviour. The participants at either extreme of panic-fear are most at risk of future rehospitalisation. Participants were randomised to receive 6 telephone-based asthma management sessions (intervention) or usual care (control). Weekly measures of morbidity and healthcare utilisation were recorded for 12 months. At 1 year, one intervention participant versus six controls had been readmitted to hospital, on a total of one and 20 occasions respectively (p=0.07). Intervention participants were less likely to be woken on more than half the nights of the week (p=0.03). Readmitted participants were most likely to be in the control group (p=0.05), report fewer weeknights undisturbed by asthma (p0.05), more days lost from work or study (p=0.05) and started or increased oral steroids more often (p=0.03). There were sufficient clinically important and statistically significant gains in healthcare utilisation and morbidity that resulted from the telephone-based intervention to merit further research into this mode of asthma management.
