Book excerpt: This study evaluated and compared the reproducibility and accuracy of cephalometric landmarks and measurements between conventional radiographic film and the corresponding indirect, JPEG compressed or direct digital images (DI). Twenty-one conventional films were indirectly digitized using a scanner, and TIFF images were converted to JPEG files using 2 levels of compression (12:1 and 25:1). Simultaneous acquisition of conventional and direct DI (stimulable-storage phosphor (SPP)) was obtained for thirty patients. Three observers for the indirect and one observer for the direct DI identified 17 and 22 landmarks respectively on the computer monitor. X and y-coordinates were recorded and cephalometric measurements calculated. Statistical analysis involved use of the paired samples t-tests and ANOVA models. The results showed that indirect DI have equivalent landmark reproducibility with conventional film. JPEG compression of indirect DI at 25:1 level results in loss in accuracy of cephalometric measurement when compared to non-compressed DI, whereas, JPEG at 12:1 did not. Direct DI were less accurate for 8 out of 21 landmarks but clinical accuracy of cephalometric measurement was equivalent to conventional film.

Twelve orthodontic residents analyzed the radiographs to derive measurements from both hard and soft tissue landmarks using three methods: (1) Manual - measurements were derived using tracing paper, a pencil, and protractor; (2) Digitization - landmarks were identified using a digitizer tablet, and measurements computed by a software program, and; (3) On Screen - radiographs were scanned into a software program where landmarks were identified using a mouse driven cursor on the monitor, and measurements automatically computed. The resulting measurements from both computer-aided systems were then compared to the clinicians' manually traced measurements which acted as a control. No statistically significant differences were found between measurements obtained from the manual method and either the digitization or on screen methods. Comparisons between the clinical efficacy of manual and computer assisted methods are presented and discussed.

Book excerpt: The purpose of this study was to compare examiner reliability of cephalometric measurements from cone beam computed tomography (CBCT) and conventional digital cephalograms. Three imaging modalities were compared: CBCT using maximum intensity projection and ray-sum algorithms, and conventional digital cephalograms. Blinded examiners were calibrated, and intra- and inter-examiner reliability were assessed. Data were collected on a convenience sample of seven subjects. Examiners obtained linear and angular measures on each of the subjects' images, and rater consistency among the three methods were assessed using repeated measures analysis of variance of intraclass correlation coefficient ratios. A statistically significantly greater level of consistency for linear measurements was seen with the CBCT images. For the conventional digital images, linear measurements showed less consistency than angular measurements. This statistical significance unlikely correlates to a clinical significance. For orthodontic diagnosis, treatment planning, and treatment evaluation, CBCT generated cephalograms are equally effective to conventional digital cephalograms.
