Bone resorption was accepted as a physiologic response to second-stage implant uncovering An average of 1.2 mm of bone loss around submerged two-piece implants in the first year following restoration was noted. The concept of platform switching was proposed to reduce crestal bone loss around implants. Purpose: This study was attempted to study the influence of platform switching on the marginal bone level alterations around implantsMaterials and Methods: 60 implant sites were divided into two groups i.e a test group and a control group. The test group comprised of platform switched implants and the control group comprised of platform matched implants Radiologic assessment was done at varying time intervals after uncovering of the implant.Results: A Comparison of mean bone loss between platform matched and platform switched abutment as noted on the mesial side showed a non-significant difference (p value=0.92) while on the distal side statistically significant difference was noted. Comparison of the results between 2 groups at 1,2 4,6 months of post abutment placement on the distal side showed a statistically significant difference. (p value=0.024).Conclusion: It was noted that bone loss is more in platform matched abutments as compared to platform switched abutments. It can thus be concluded that platform switching results in peri-implant bone preservation and also has esthetic benefits.

The aim of this study was to systematically review the combined effect of platform switching with implant placement depth on marginal bone loss.An electronic search was conducted up to March 2017. One-piece implants and immediate post-extractive implants were excluded. The mean differences between groups were calculated using the inverse variance method with a random effects model.Twenty studies were included in the meta-analysis. More marginal bone loss was observed around platform-matched implants at 12 months after prosthetic loading (-0.38mm (-0.49; -0.27)). Among platform-switched implants, more marginal bone loss was observed around implants placed subcrestally (-0.67mm (-1.31; -0.03)).Further studies are needed to compare marginal bone loss around (1) platform-matched implants placed at different depths and (2) platform-switched implants placed at crestal versus supracrestal levels.

Platform matched abutments elicits statistically insignificant change in mesial and distal papilla level and buccal peri-implant mucosa level at different time interval.(p u2265 0.05). Platform switched abutment elicits statistically significant change in mesial and distal papilla level and buccal peri-implant mucosa level at different time interval. Conclusion: This study suggests that platform switching can provide increase in mesial and distal papilla and buccal peri-implant mucosa level along with bone remodeling around implant.

Dental implants may be prone to tribocorrosion leading to the release of metal ions and particles which may disrupt bone homeostasis Objectives: The aim of this project was to compare the amount of corrosion products released from different implant-abutment couplings with different connection geometries under accelerated corrosion tests, and to evaluate different osteoblastic cell responses to the released products. Materials and methods: Part I: Titanium cylinders were connected to titanium alloy (Ti-6Al-4V) and cobalt"†chrome alloy abutments forming either platform-switched or platform-matched groups. Samples were subjected to static immersion in 1% lactic acid for 1 week. Metal ions released were measured and microscopic analyses were performed pre- and post- immersion to assess corrosion at the interface. Part II: Osteoblasts were treated with culture media containing different concentrations of metal ions obtained from the results of part I. Osteoblasts treated with ion-free culture medium served as the controls. Osteoblastic viability, apoptosis and expression of genes related to bone resorption were analysed. Part III: The samples used in part I were incubated with osteoblasts. Viability, apoptosis and expression of the genes tested in part II were investigated. Part IV: Titanium implants were coupled with either titanium, gold, cobalt-chrome, or zirconia abutments forming either platform-switched or platform-matched groups. The specimens were subjected to cyclic loading under wet acidic environment. Metal ions released were measured and microscopic evaluations were performed pre- and post- immersion for tribocorrosion assessment and wear particle characterization. Results: Part I: The implant"â€abutment couplings underwent an active corrosion process resulting in metal ions released into the surrounding environment. The platform- matched groups demonstrated higher amount of metal ion release. Part II and Part III: Osteoblastic cell viability, apoptosis, and regulation of bone resorbing mediators were significantly altered in the presence of both the metal ions and the different implant-abutment couplings. The observed changes in some of the biological responses tested were more pronounced in groups representing the platform-matched implant-abutment couplings. Part IV: The implant"â€abutment couplings underwent an active tribocorrosion processes resulting in metal ions and wear particles released into the surrounding environment. The platform-matched groups demonstrated higher amount of metal ion release and more surface damage Conclusion: Platform-switching concept had a positive effect in reducing metal ion release and wear features from dental implants which in turn minimised the adverse osteoblastic biologic responses related to peri-implant bone loss, therefore highlighting the possible role of corrosion products in the mediation of crestal bone loss around dental implants.

However, PS has been a generic term used for different implant-abutment mismatch sizes, among the studies. It is yet not well understood to which extent different mismatch size may influence the extension of marginal bone preservation for PS implants. Aim: To evaluate the effect of different implant-abutment mismatch sizes on marginal periimplant bone loss. Materials and methods: Twelve patients received four u00d8 4.5 x 13 mm external hex implants, and abutments diameters of 4.5 mm (conventional matching implant-abutment design u2013 CMD), 4.1 mm (PS04), 3.8 mm (PS07) and 3.3 (PS12), randomly placed based on a split-mouth design. Clinical parameters were evaluated at 6 sites around the implants, at a 12, 36, and 60 months follow-up. The distance from the top of the implant to the first bone-to-implant contact u2013 IT-FBIC was evaluated on standardized digital peri-apical radiographs acquired at 1, 3, 6, 12, 36 months follow-up. Comparison among multiple periods of observation were performed using repeated-measures Analysis of Variance (ANOVA), followed by a Tukey post-hoc test, while two-period based comparisons were made using paired t-test. Further, 12 computer-tomographic based finite element (FE) models were accomplished and the results for the peak EQV strain in periimplant bone were interpreted by means of ANOVA. Book excerpt: It was proved that vertically thin soft thickness might be an important factor in development of early crestal bone loss, therefore several methods were advanced how to increase soft tissue thickness. Some authors claim, that subcrestal placement of implant and subsequent controlled bone remodelling can accommodate the problem. Another method suggests to increase soft tissue thickness by placing implant at the bone level with 2 mm height healing abutment and suture soft tissues over.The aim of the study was fourfold - to register crestal bone stability around (1) 1.5 mm subcrestally placed implants; and (2) epicrestally placed implants with 2 mm healing abutments; to (3) register bone remodelling in subcrestal group; (4) determine how vertical soft tissue has increased in epicrestal group, compared to initial situation.24 patients with vertically thin tissues of 2 mm or less received 24 submerged bone level platform-switched implants (Conelog, Camlog, Basel, Switzerland). Implants were divided into 2 equal groups u2013 (1) 1.5 mm subcrestally placed implants and (2) epicrestal implants with 2 mm healing abutments. At II stage surgery, all implants received 4 mm healing screws and later restored with Zr screw-retained single crowns, radiological images taken at II stage surgery and after 1 y follow-up. Bone loss was calculated as a distance between implant neck and first radiographically visible bone-to-implant contact and measured in both groups. Additionally, in subcrestal group bone remodelling was calculated as a distance between bone crest and implant neck. Mann-Whitney U-test was used for statistical analysis, significance set to 0.05.After 1 y follow-up, 1 (subcrestal) group had 0.06 (0.07 SD) mm of bone loss, while 2 group (epicrestal with 2 mm healing abutment) had 0.11 (0.14 SD) mm of bone loss, with statistically significant difference (P=0.005). Bone remodelling in group 2 (subcrestal) was 0.55 (0.3 SD) and was significantly larger compared to bone loss of subcrestal (P=0.001) and epicrestal groups (P=0.003). Vertical tissue thickness in epicrestal group before intervention was 2.30 (0.26 SD) and 3.65 (0.41 SD) after use of 2 mm healing abutment, with statistical difference (P=0.005).The novelty of the study is that crestal bone loss and bone remodelling as discussed as separate outcomes. Bone loss was calculated as a distance between implant neck and first radiographically visible bone-to-implant contact, while bone remodelling was calculated as a distance between bone crest and implant neck. Both groups had only minor crestal bone loss and could be used as a method to reduce bone loss in vertically thin tissues. However, it is interesting to note that bone remodelling in subcrestal group was larger than bone loss in both groups. In addition, the use of 2 mm healing abutment, as a method to increase soft tissue thickness and reduce bone loss was established.
