A number of psychosocial interventions exist, but the most effective components of interventions are unclear. Mechanisms research could organize and advance this field, but few mechanisms studies have been completed in the field of psycho-oncology. This study utilized archival data from a randomized controlled trial of a comprehensive, biobehavioral intervention for newly diagnosed breast cancer patients (N=227) to investigate mechanisms and mediators of change in sexual and body image outcomes. Hierarchical linear models were constructed separately for sexual behavior, sexual satisfaction, and body image. The intervention was not associated with improvements in sexuality and body image outcomes. Relationship satisfaction was a significant predictor of all outcomes and interacted with time and group assignment such that those with lower relationship satisfaction had more negative body image at baseline and more positive body image at 12 months. Reported use of intervention techniques was not associated with improvements in outcomes. This study confirms the importance of relationship satisfaction in predicting sexual and body image outcomes. In terms of intervention mechanisms, this study provides evidence that exposure for body image is not an effective component for comprehensive interventions such as the one in this study, and that further research is needed to determine effective approaches to these problems in the breast cancer population.
