The ESP block has been shown to provide effective postoperative analgesia after various laparoscopic abdominal surgeries. We therefore sought to determine the analgesic effects of ESP block in living liver donors undergoing laparoscopic right hepatectomy. We hypothesized that it would provide noninferior postoperative analgesia compared to ITM and reduce postoperative nausea/vomiting and pruritus.Methods: Fifty-four patients scheduled for pure laparoscopic right donor hepatectomy were randomly assigned to one of two groups: an ESP group (n=27) and an ITM group (n=27). Patients in the ESP group received preoperative bilateral single-injection ESP blocks at the level of the T8 transverse process with an injection of 20 mL 0.5% ropivacaine with 5mcg/mL epinephrine on each side). Patients in the ITM group received a preoperative intrathecal injection of 400 u00b5g morphine sulfate. The primary outcome was the pain score at rest 24 hours after surgery as assessed by an 11-point numeric rating scale. The predetermined noninferiority limit was 1. Eighty-eight patients scheduled for VATS were randomly allocated to either an ESPB or a TPVB group. Patients in both groups received continuous infusion of 0.2% levobupivacaine (8 mL/hour) after 20 mL of 0.2% levobupivacaine bolus injection. The primary outcome was postoperative numerical pain rating score (NRS) at rest 24 hours postoperatively, with a maximum acceptable difference (non-inferiority margin) between the groups as 1. We also evaluated NRS during movement, amount of rescue fentanyl used, and anesthetized dermatome number.Results: Eighty-one patients completed the study. NRS at rest was significantly lower in the TPVB group at 1, 2, and 24 hours postoperatively (respective p values = 0.018, 0.008, and 0.030). There were no significant differences in NRS during movement. The median difference in NRS at rest 24 hours postoperatively was over 1, which failed to demonstrate non-inferiority. Various regional and systemic analgesic approaches have been used to tackle postoperative pain in these patients. The objectives of this study were to evaluate the efficacy and feasibility of subcostal Transverse Abdominis Plane (TAP) block and bilateral rectus sheath blocks as an adjunct to Patient controlled Analgesia (PCA) using Morphine.Methods: The study was planned as a prospective randomized double blind study. 50 consecutive liver donors belonging to age group 18-50 years were randomized into two groups of 25 each. Patients in both the groups received a standard general anesthetic. All patients were given right subcostal TAP block and bilateral rectus sheath blocks under ultrasound guidance at the end of surgery. Group R patients received the blocks with 50 ml of 0.3% Ropivacaine; with 10 ml each for rectus sheath blocks and 30ml for subcostal TAP block using under ultrasound guidance. Group S patients received the same blocks with 50 ml of Normal Saline at the end of surgery. Patients in both the groups had access to morphine PCA.Results: Median morphine consumption was found to be significantly less (28 mg vs. 43 mg, p=0.020) in Group I patients. The Numerical Rating Scores (NRS) were comparable in both the groups. No major block related complications were observed. Conclusions: We found subcostal TAP block in combination with bilateral rectus sheath blocks a safe and effective adjunct for pain management in patients undergoing donor hepatectomy.

Main purpose of this study was to evaluate the analgesic effect of ultrasound guided ESB in breast surgery.MethodsFifty ASA I-II female patients, aged 25-65, who were scheduled to go under elective breast surgery were included to the study. Patients were randomized into two group as ESB and Control group. Single shot ultrasound (US) guided ESB with 20ml 0.25 % bupivacaine was done preoperatively to all patients in ESB group. Patients in both groups were provided with iv patient controlled analgesia device containing morphine for the postoperative analgesia. Morphine consumptions and visual analogue scale (VAS) scores for pain were recorded at 1st,6th, 12th and 24th hours postoperatively. ResultsThere were no significant differences between ESB and control groups for VAS scores at 1st, 6th, 12th and 24th hour (Median VAS values were 2, 2, 0, 0, and 2, 2, 1, 1 respectively).
