Book excerpt: Introduction : Cephalometrics is a radiographic technique used to analyze craniofacial structures for diagnostic or analytic purposes. However, conventional cephalometrics has many limitations, including the fact that it is entirely landmark-dependent and based on a 2-dimensional image. In previous studies, Fourier descriptors have been used to describe part of the craniofacial structure. However, these studies are insufficient providing a complete set of information to fully analyze craniofacial morphology. The aim of this study is to develop a true 3-dimensional description of the craniofacial structure that will provide the basis of morphologic analysis in a more comprehensive and effective way. Materials and Methods : CBCT images taken at UCLA School of Dentistry by the Newtom 3G CBCT scanner (Image Works) were collected. Samples with significant craniofacial defects were excluded. Using 10 samples without morphologic abnormality, the curved outline of the craniofacial structure was defined by collecting the coordinates of the points along the border of the shape. The superimposition and averaging was done using procrustes analysis. Geometric algebraic methods were used to combine all the curved outlines to construct an average shape of the whole craniofacial structure allowing for superimposition and comparison. Results : The outline of the skull was successfully aligned in 3-dimensional space to represent the craniofacial structure. The normalized form provided a basis for the comparison of an individual sample to the group average. The new method showed advantages over the conventional cephalometrics by eliminating the constraint factors of 2-dimensional images. This allows us to analyze difference and irregularities of craniofacial morphology in a more accurate and effective way. Conclusion : The curve and surface information extracted from CBCT image data could be used to find a normalization of the population, which is a basis for a 3-dimensional cephalometric analysis to overcome the limitations of conventional cephalometry.

With the introduction of Cone-beam Computed Tomography (CBCT), an accurate three-dimensional (3-D) representation of the craniofacial form could be generated. Objectives: The overall objective of this pilot study was to overcome limitations of traditional 2-D cephalometry by developing and testing a protocol for generating an average 3-D human skull using CBCT images. Specific goals were 1) To identify landmarks and regions on the skull appropriate for geometric morphometric analysis especially in relation to orthognathic surgery patients, 2) Develop protocol for applying geometric morphometric techniques to generate an accurate 3-D average skull model suitable for surgical orthodontic applications, 3) To compare the gender differences between the male and female average skull models, and 4) To explore the usefulness of the normalized skull models for diagnosis and evaluation of treatment outcomes. Methods: Thirty CBCT scans obtained from normodivergent adults with Class I skeletal pattern were used. Geometric morphometric analysis of these scans was performed with the Checkpoint software (Stratovan Corporation, Davis, CA). Eighteen landmarks and 8 patches with semi-landmarks encompassing a total of 666 points were identified on each 3-D surface model generated from the CBCT scan. Once homologs points were collected statistical shape analysis was performed using absolute averaging and Procrustes superimposition technique. Once population averages were computed, normalization was performed on two specimens with Class II and III skeletal malocclusions. Results: Simple linear measurements computed from 3D coordinate data revealed that males had a significantly larger anterior cranial base length, transverse dimensions and ramus height. Two 3-D skull models were generated for each gender based on the aforesaid absolute and Procrustes averaging techniques. Normalization of 3-D skulls with Class II and III deformities enabled the identification of most affected regions in the skull that needs to be corrected by orthognathic surgery. Conclusion: The constructed average 3-D skull models were a viable alternative to the traditional 2-D cephalometry. These models can be used for diagnosis, treatment planning and evaluating treatment outcomes.

In previous studies, the UCLA Section of Orthodontics, in a collaborative effort with the Laboratory of Neuro Imaging (LONI), developed a method for surface mapping a skull in three dimensions and expressing the curved outline of the structure using geometric algebraic methods. The long-term objective of the study is to establish a true three-dimensional (3D) skeletal analysis involving quantifiable 3D subnorms with respect to age, gender, and ethnicity. The specific aim of this study is to create a quantifiable 3D Korean adult normative skull that allows superimposition and comparison. Materials and Methods: Quantifying samples followed the inclusion criteria of Korean adult male/female of ages greater than 18 and skeletal class I relationship based on cephalometric analyses. Patients with craniofacial defects, significant skeletal asymmetry, and/or previous history of orthodontic treatment were excluded. Using 11 male and 13 female normative CBCT images, the surface map and curved outline of each skull were generated by projective mapping and harmonic interpolation, and acquisition of points along the surface, respectively. The Procrustes superimposition method was used for normalization of samples. Results: This normalized form provided a foundation for the comparison of an individual to the average, allowing sample differences to be visualized and quantified. The establishment of a linear distance map, directional vector map, and statistical map effectively showed clear deviation of individual skulls to the norm. Conclusion: The establishment of subnorms is a difficult task due to limited sample size from stringent inclusion and exclusion criteria, but can be successfully done utilizing the methods we developed. Further studies accounting for ethnicity, gender, and age will strengthen the diagnostic power of CBCT images in orthodontics.

Certain aspects of craniofacial development are unique developmental processes in higher vertebrates. First, in higher vertebrates the cranial neural crest, in contrast to the trunk neural crest, gives rise to the skeletal structures. These skeletal elements include those comprising mem brane bone and secondary cartilage, which with the exception of the clavicle are tissue types found exclusively in the head in higher vertebrates. Second, with the exception of the tongue, the origin of the musculature is distinct from other regions of the body. The body and tongue muscles are formed from the segmented epithelial somites whilst the head musculature is formed from unsegmented paraxial and prechordal mesoderm. Furthermore, the signalling cascades that control myogenic differentia tion appear to be distinct as determined by gene expression and the response of myogenic cells to growth factors. Finally, the neurogenic placodes, which give rise to the sensory organs and some cranial ganglia, are only found in the head. Over recent years, there have been significant advances in our knowledge of the molecular proc esses that control craniofacial development in a number of animal models. This has given insight into the genes that control many aspects of head development from the initial induction of the head to the final stages of differentiation.
