Book excerpt: This knowledge of basic sciences coupled with a keen clinical sense, attention to detail and good decision-making skills is the defining characteristic of Critical Care. However, medical knowledge of the nature needed for an exacting practice of Critical Care also requires renewal as technology in the ICU and understanding of disease processes evolve with the passage of time. This monograph has been created to fill this gap. While Critical Care is an established specialty in many parts of the world, only in recent years it has received some attention in India. It is therefore appropriate that this monograph has been planned, keeping in view the growing interest in this specialty. Some of the chapters are detailed while others provide an overview of the subject, but each one has been written in keeping with the challenges of our present times. Many of the important issues related to the practice of Critical Care in our country have been covered in this monograph. This publication has been made possible through the untiring efforts of all faculty members and with the help of a grant from one of the companies. Critical Care Update will hopefully serve consultants, junior doctors, nurses and paramedical staff as a handy book to guide practice and decision-making in the future.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides a detailed review of state of the art knowledge on critical care topics as well as the latest research findings. It covers the core aspects in excellent detail, but is not so comprehensive as to make its daily use unfeasible. For each condition considered, discussion of the pathophysiology is integrated with observations on diagnosis and treatment in order to allow a deeper understanding. The book is scientifically based, with extensive references to published research. This will allow readers to investigate their individual interests further and will enable physicians to justify measures by providing a coherent, evidence-based strategy and relevant citations where needed. Core Knowledge in Critical Care Medicine will appeal to experienced practitioners as an aide-mémoire, but will also be of great value to a wide range of more junior staff wishing to complement their background knowledge with important facts applicable to everyday practice.

Available in PDF, EPUB and Kindle. Book excerpt: We are honored to present the second edition of Surgical Intensive Care Medicine. Our first edition was considered to be an important contribution to the critical care literature and received excellent reviews from Critical Care Medicine, Chest, and Anesthesiology. In the second edition, the basic organization of the book remains unchanged, being composed of 60 carefully selected chapters divided into 11 sections. The book begins with general topics in primary intensive care, such as airway management and vascular cannulation, followed by categories based on medical and surgical subspecialties. While the chapters discuss definitions, pathophysiology, clinical course, complications, and prognosis, the primary emphasis is devoted to patient management. The contents of the current edition have been comprehensively upgraded and the chapters retained from the first edition have been thoroughly updated, revised, or rewritten. In this second edition, some new topics have been added including Postoperative Care of the Obese Patient, Postoperative Care of the Pancreas Transplant Patient, Optimization of High-Risk Surgical Patients, Post- erative Alcohol Withdrawal Syndrome, Ethics and End of Life Issues, Improving the ICU, and Continuous Medical Education in Intensive Care Medicine. We are extremely fortunate to have high-quality contributors, many of whom are nationally and internationally recognized researchers, speakers, and practitioners in Cri- cal Care Medicine. An important feature of this latest edition is the geographical diversity of its authors. Most are based in the United States, but colleagues from Canada, England, Ireland, Germany, Belgium, Holland, France, Italy, Portugal, and Australia have also made notable contributions.

In combination with the principles of beneficence, autonomy and justice, and whilst keeping in mind the confidence and dignity of the patient, it should constitute the basis of our behaviours as physicians and nurses. Since diagnostic and therapeutic interventions have become more complex and their risk/benefit ratios more difficult to assess, the importance of safety and quality of care rises. Avoiding the infliction of harm on our patients has moved into the focus of clinical medicine"--Publisher's description.

Available in PDF, EPUB and Kindle. Book excerpt: In recent years, many clinical interventions in intensive care medicine have been based on clear scientific evidence. However, at least as many clinical interventions still remain the subject of controversy, either due to a lack of rigorous data or due to the existence of conflicting data. In this book, these controversies are discussed by experts of international renown in their respective fields. The goal is twofold: To provide us with a balanced and unbiased presentation of the subject, explaining the different 'schools of thought' relevant to the controversy; To summarise the data with a clinically useful and valid recommendation for our practice. Virtually all fields of intensive care medicine are covered in over 50 chapters dealing with controversies over treatment options in acute illness states, organising and providing care for acutely ill patients, as well as how to answer ethical questions arising in critical care medicine every day.

Physicians, nurses, and many others associated with the field of Intensive and Critical Care Medicine will be coming from all corners of the world to Florence, Italy in August, 2009 to celebrate the 10th quadrennial congress. Every 4 years for the last 36 years, congresses in the magnificent venues of London (1973), Paris (1977), Washington (1981), Jerusalem (1985), Kyoto (1989), Madrid (1993), Ottawa (1997), Sydney (2001), and Buenos Aires (2005) have sig- fied an ever-developing process which has resulted in the four pillars of the field of Intensive and Critical Care Medicine, namely partnership, ethics, professionalism, and competence. The first pillar is based on a stronger interdisciplinary collaboration and a mul- professional partnership in the field of Intensive and Critical Care Medicine. In recent decades, professional activity in medicine has been regulated by well-defined, universal principles, such as the welfare of the patient, autonomy, social justice, and the patient–physician relationship. The second pillar, ethics, has offered welcomed assistance to all these principles in establishing an ethics curriculum.

Book excerpt: A wave of new health care innovation and growing demand for health care, coupled with uncertain productivity improvements, could severely challenge efforts to control future health care costs. A committee of the National Research Council and the Institute of Medicine organized a conference to examine key health care trends and their impact on medical innovation. The conference addressed the following question: In an environment of renewed concern about rising health care costs, where can public policy stimulate or remove disincentives to the development, adoption and diffusion of high-value innovation in diagnostics, therapeutics, and devices?

Available in PDF, EPUB and Kindle. Book excerpt: In 1772 in Uppsala the Swedish chemist Karl Wilhelm Scheele discovered the element Oxygen. Two hundred and one years later, in 1973, the International Society on Oxygen Transport to Tissue (ISOTT) was founded. Since then there has been an annual ISOTT meeting. After 24 years of international ISOTT meetings it was decided, at the 2005 summit in Bary, Italy, that the 2007 meeting was to be held in Uppsala, Sweden. Thus, after the Louisville meeting we, in the Uppsala group, withdrew to the Edgewater Resort at Taylorsville Lake outside Louisville and prepared the Uppsala ISOTT meeting by tasting Kentucky Bourbons, smoking cigars while bathing in a jacuzzi in the hot dark Kentucky night full of fire flies and a sky full of stars. The ISOTT program should include different aspects of oxygen - however, it is accepted that each meeting has its own local “touch”. We decided to focus the Uppsala ISOTT meeting on the theme of "Imaging and measuring oxygen changes". With this in mind we invited scientists within and outside the ISOTT society. We then also received lots of good abstracts from ISOTT members that were included in the program. Lars-Olof Sundelöf introduction speech “AIR AND FIRE” concerned how oxygen was discovered in Uppsala in 1772 by Karl Wilhelm Scheele. After the introduction speech a get together event took place in the magnificent and spacious foyer of Uppsala University main building. The vice chancellor Ulf Pettersson welcomed all delegates to Sweden and Uppsala.

Book excerpt: Over 25 years ago, a group of intensive care physicians felt that a European Society of Intensive Care Medicine (ESICM) would be helpful as a framework to facilitate research, training and teaching throughout the different countries of Europe and also from further afield. Accordingly, they founded our Society on 13 May 1982 in the headquarters of the World Health Organization in Geneva, Switzerland. Today we have a large society of active members from all the continents in the world and are looking forward to our 20th Annual Congress to be held in Berlin, Germany in October 2007. Thousands of intensive care providers, many of whom are our members, will meet under the motto '25 years of progress and innovation'. On the occasion of these anniversaries it is an honour and a genuine pleasure for us to present this book under the same title.

Available in PDF, EPUB and Kindle. Book excerpt: This book explains the basic concepts of Selective Decontamination of the Digestive tract (SDD) to help those involved in treating critically ill patients to improve outcomes and the quality of care. SDD has led to major changes in our understanding, the treatment and prevention of infections in critically ill patients over the past 40 years. It is the most studied intervention in intensive care medicine and is the subject of 73 randomized controlled trials, including over 15000 patients and 15 meta-analyses. SDD reduces morbidity and mortality, is cost-effective and safe as SDD does not increase antimicrobial resistance. Correct application of the SDD strategy enables ICU teams to control infections – even in ICUs with endemic antibiotic resistant microorganisms such as methicillin resistant S. aureus (MRSA). Describing the concept and application of SDD, and presenting case studies and microbiological flow charts, this practical guide will appeal to intensivists, critical care practitioners, junior doctors, microbiologists and ICU-nurses as well as infection control specialists and pharmacists.

Available in PDF, EPUB and Kindle. Book excerpt: Second in a series of publications from the Institute of Medicine's Quality of Health Care in America project Today's health care providers have more research findings and more technology available to them than ever before. Yet recent reports have raised serious doubts about the quality of health care in America. Crossing the Quality Chasm makes an urgent call for fundamental change to close the quality gap. This book recommends a sweeping redesign of the American health care system and provides overarching principles for specific direction for policymakers, health care leaders, clinicians, regulators, purchasers, and others. In this comprehensive volume the committee offers: A set of performance expectations for the 21st century health care system. A set of 10 new rules to guide patient-clinician relationships. A suggested organizing framework to better align the incentives inherent in payment and accountability with improvements in quality. Key steps to promote evidence-based practice and strengthen clinical information systems. Analyzing health care organizations as complex systems, Crossing the Quality Chasm also documents the causes of the quality gap, identifies current practices that impede quality care, and explores how systems approaches can be used to implement change.
