Modern technology has made the access to information so much easier and faster, and the Internet sometimes gives its users the false impression that all information can be obtained without any human intervention. If this were true, libraries would be reduced from intellectual laboratories to museums, where visitors only come to look at those strange paper format precursors of the digital information carriers. Even if such an extreme futuristic view cannot be completely excluded, it is still very far away. In the meantime, libraries and librarians continue to play an important role, especially in the digital environment where the traditional skills of librarians have found a new importance and new applications. Their devotion to the preservation of historically significant documents may counterbalance the tendency of the Internet to become a dull collection of knowledge facts and to forget the logical processes through which this knowledge was obtained. The long-established experience of librarians as organizers of information and as facilitators of the access to this organised knowledge collection has already proven to be of immeasurable value for the use of all kinds of digital information sources.

Physical illness resulted from pathology which could be identified and studied in detail and the cause could often be deduced. Mental illness, on the other hand, could not, it was believed, be based upon identifiable changes in basic science. It was literally 'all in the mind' and, occurring in the absence of causes that could be identified, analysed and remedied, had no treatment that could in any way be called specific. N ow it is all too easy, in Medicine, for a doctor to become so immersed in a specialty that he fails to notice how his colleagues in other specialties are faring and, through laziness, to miss advances even of fundamental importance in fields other than those in which he himself works. As a consultant surgeon, after W orId War II, although I realized the importance of keeping in touch with those medical specialties which appeared to have a common interface with my own surgical interests, such as general medi cine, gastro-enterology, hepatology and endocrinology, it did not occur to me that there was any pressing need to ask what was new in Psychological Medicine.

There are several factors that have contributed to this empirical influx, including improved diagnostic cri teria for many psychiatric disorders, increased nosological attention to childhood psychopathology, and development and standardization of several structured diagnostic interview schedules for both adult and child populations. With the advent of DSM-III-R, and in anticipation of DSM-IV, diagnostic labels and their definitions have been in a state of change, as evinced by the many refinements and modifications currently taking place. However, the basic purpose or raison d'etre of the nosological scheme has not been altered. Psychiatric diagnosis is the means by which we classify or categorize human psychopathology. And, as is the case in the medical arena, psychiatric diagnosis serves three central functions: classification. communication. and prediction. As research accumulates, our understanding of psychiatric disorders increases, and we are in a much better position to classify reliably and with validity, as well as to com municate and predict Despite periodic changes in the diagnostic system, the basic strategies for conducting diagnostic research (e. g. , genetic-family studies, biological markers, follow-up studies, etc. ) do not vary appreciably over time. But in over one decade no scholarly book has appeared that tackles the essential research issues involved in upgrading the diagnostic endeavor.
