Malaysia health system has been well developed and was said to have achieved universal health coverage based on the easy access to primary and secondary health care services in the public sector. In this book, we present our effort to assess if Malaysia provides UHC for preventive, curative and palliative to breast cancer patients . At the inception of the study, we embarked on the development of a composite index to assess the overall status of UHC. This is followed by an extensive research to explore the extent of financial coverage for management of breast cancer. Over 300 patients in various stages of the breast cancer attending the selected public tertiary hospitals were interviewed and their medical records reviewed to assess their health spending and to estimate the incidence of catastrophic health expenditure. The book is the first of its kind that provides comprehensive real-world data on universal coverage for breast cancer patients in the world.

This book also illustrates how the 5G core and EPC is integrated into the radio access network, how virtualization and edge computer fundamentally change the way users interact with the network, as well as 5G spectrum issues. This book is structured into six chapters. The first chapter reviews the use cases, requirements, and standardization organization and activities for 5G. These are 5G requirements and not NR specifically, as technology that meets the requirements, may be submitted to the ITU as 5G technology. This includes a set of Radio Access Technologies (RATs), consisting of NR and LTE; with each RAT meeting different aspects of the requirements. The second chapter describes the air interface of NR and LTE side by side. The basic aspects of LTE that NR builds upon are first described, followed by sections on the NR specific technologies, such as carrier/channel, spectrum/duplexing (including SUL), LTE/NR co-existence and new physical layer technologies (including waveform, Polar/LDPC codes, MIMO, and URLLC/mMTC). In all cases the enhancements made relative to LTE are made apparent. The third chapter contains descriptions of NR procedures (IAM/Beam Management/Power control/HARQ), protocols (CP/UP/mobility, including grant-free), and RAN architecture. The fourth chapter includes a detailed discussion related to end-to-end system architecture, and the 5G Core (5GC), network slicing, service continuity, relation to EPC, network virtualization, and edge computing. The fifth and major chapter describes the ITU submission and how NR and LTE meet the 5G requirements in significant detail, from the rapporteur responsible for leading the preparation and evaluation, as well as some field trial results. Engineers, computer scientists and professionals with a passing knowledge of 4G LTE and a comprehensive understanding of the end to end 5G commercial wireless system will find this book to be a valuable asset. The urban local self-government (ULSG) is struggling to provide efficient, effective, inclusive and responsive urban services. Most ULSGs are too fragile to perform the mandated functions for enhancing the quality of life and making cities and towns livable. The book traces evolution of ULSGs, its decline after Independence, and steps taken to strengthen them, especially through the big-bang decentralization initiative of 1992 for their empowerment, by enacting the 74th Constitution Amendment Act (74th CAA). Analyzing facets of the decentralization initiative, views of two review Commissions, policy responses to it and processes for implementation of constitutional provisions, it alludes to conspicuous gaps at three levels such as (i) gaps and deficiencies in the 74th CAA (ii) gaps in the post-74th CAA municipal Acts that were required to conform to the constitutional provisions, and (iii) gaps due to half-hearted implementation of even the mandatory constitutional provisions. Empowerment and strengthening of ULSG being in the nature of an imperative, it explores plausible options within the constitutional autonomy of states. Empowerment denotes authority, power and clarity in municipal functional and fiscal domain. Therefore it also specifies experiential based rational framework and a strategy for strengthening ULSGs that must look beyond the existing predilection for mere training.
