Book excerpt: Contexte : les médicaments potentiellement inappropriés (MPI) sont définis comme des médicaments ayant un rapport bénéfice/risque défavorable. Ils présentent un risque d'évènement indésirable médicamenteux supérieur au bénéfice attendu ou une efficacité douteuse par rapport à d'autres alternatives thérapeutiques. Laroche et al. ont défini en 2008 une liste de MPI applicable à la pratique médicale française regroupant les médicaments à éviter chez les personnes âgées. L'objectif de l'étude était de décrire l'association entre la consommation de MPI selon la liste de Laroche et la mortalité des personnes âgées institutionnalisées. Méthode : étude de cohorte, épidémiologique, observationnelle, descriptive, rétrospective au sein de 9 établissements d'hébergement pour personnes âgées dépendantes des départements du Nord et du Pas-de-Calais entre le 01/05/16 et le 30/04/17. Les traitements de fond et leurs posologies ont été relevés en date du 01/05/16. Le critère de jugement principal était la survenue d'un décès pendant la période d'étude. Résultats : 886 patients ont été analysés dont 20,4 % d'hommes (n=181) et 79,6 % de femmes (n=705). 200 patients sont décédés au cours du suivi, soit 22,6 % de l'échantillon. Il existait une augmentation de 3,3 % du risque de décès chez les patients ayant consommé au moins un MPI par rapport à ceux n'en ayant pas consommé. Cependant cette association n'était pas statistiquement significative, ni avant ajustement sur les facteurs de risque de mortalité (HR=1,033 ; IC95 % [0,78-1,38] ; p=0,82) ni après ajustement (HR=1,029 ; IC95 % [0,77-1,38] ; p=0,85). Il existait une augmentation de 2,8 % du risque de mortalité lorsqu'on augmentait d'une unité le nombre de MPI consommé avant ajustement, mais ce résultat n'était pas significatif (HR=1,028 ; IC95 % [0,73-1,45] ; p=0,87). Après ajustement, le risque de mortalité était de 30 %, mais il restait non significatif (HR=1,3 ; IC95 % [0,89-1,89] ; p=0,17). Conclusion : les résultats de l'étude ne retrouvent pas d'augmentation significative du risque de mortalité chez les personnes âgées de 75 ans et plus consommant des MPI. Bien qu'inattendus, ces résultats restent toutefois concordants avec les données de la littérature. La révision régulière des prescriptions doit néanmoins rester systématique chez les personnes âgées, les MPI représentant un surcoût de dépense pour l'assurance maladie et un risque que leurs consommateurs les privilégient au détriment de traitements plus pertinents.

Book excerpt: Depuis quelques décennies, la population âgée française ne cesse de croître, puisque l'espérance de vie s'allonge, en lien avec les progrès médicaux. Le vieillissement de l'organisme provoque l'apparition de nombreuses pathologies, dont il découle une polymédication majorant le risque iatrogène chez les personnes âgées. Toutefois, ce risque est en partie évitable en limitant notamment la prescription de médicaments potentiellement inappropriés (MPI), qui sont associés à une augmentation de la morbi-mortalité de cette population. L'objectif de ce travail était de décrire l'impact d'une hospitalisation sur la prescription de MPI, dans une population âgée de 65 ans ou plus. Les objectifs secondaires étaient i) d'évaluer pour les patients chez qui une diminution du nombre de MPI avait été observée à l'issue de leur hospitalisation, l'évolution de la prescription de MPI un an après la sortie de l'hôpital et ii) d'identifier les classes médicamenteuses les plus fréquemment impliquées dans la prescription de MPI. Notre étude, qui a inclus 191 patients âgés en moyenne de 81 ans admis en médecine, chirurgie ou gériatrie, montre que l'hospitalisation entraîne une augmentation de la polymédication (75,4% à l'entrée vs. 86,9% à la sortie), sans pour autant augmenter la prévalence des MPI, une discrète diminution de celle-ci, non significative, étant même observée (64,4% à l'entrée vs. 62,3% à la sortie). Pour les patients chez qui une diminution du nombre de MPI avait été observée en sortie d'hospitalisation, la prévalence des MPI augmente de nouveau 1 an après la sortie d'hospitalisation (73,7% vs. 57,9%). Enfin, les médicaments les plus fréquemment retrouvés parmi les MPI sont les médicaments de la sphère neuropsychiatrique, digestive et cardiovasculaire. La principale limite de cette étude est son design rétrospectif limitant l'évaluation de certains critères STOPP et ne permettant pas les échanges avec les prescripteurs, indispensables au jugement clinique. Notre étude n'a par ailleurs pas intégré les omissions thérapeutiques, qui méritent aussi d'être considérées dans la stratégie d'optimisation de la prescription médicamenteuse chez la personne âgée.

Une revue de littérature a été réalisée afin d'évaluer le lien entre la prescription de MPI et la survenue d'événements cliniques indésirables. Au total, 37 études ont été retenues. La prescription de MPI a été associée à un impact péjoratif en termes de survenue d'événements indésirables, de passages aux urgences, de nombre d'hospitalisations, d'augmentation de la durée d'hospitalisation, de fractures du col du fémur, de blessures consécutives à une chute, de consommation de soins, de mortalité et de diminution de la qualité de vie liée à la santé. Nous avons ensuite étudié la prescription de MPI sur les ordonnances de sortie d'hospitalisation (d'après LAROCHE, STOPP 2008 et EU(7)-PIM list) dans une population de patients inclus dans le programme pluridisciplinaire OPMED (Optimisation de la Prescription MEDicamenteuse). Sur les 2 095 patients, 364 (17,4 %) ont au moins un MPI selon LAROCHE, 425 (20,3 %) selon STOPP et 1608 (76,8 %) selon EU(7)-PIM list. Au total, 3 186 MPI selon EU(7)-PIM list ont été identifiés. Les plus fréquents sont : les Inhibiteurs de la Pompe à Proton prescrits pendant plus de 8 semaines (786 soit 24,7 % du total) et le tramadol (202 - 6,3 %). Regroupés par classes ATC, ce sont les MPI appartenant à la classe N. Système Nerveux qui sont les plus nombreux (1201 - 37,7 %). Des analyses univariées et multivariées ont permis d'évaluer l'association entre la prescription de MPI et le devenir du patient 12 mois après sa sortie. Avec la EU(7)-PIM list il existe une association significative entre la prescription de 2 MPI ou plus et le décès (OR = 2,26 [IC 95% = 1,23-4,16]) et entre la prescription de 3 MPI ou plus et le décès (OR = 4,85 [IC 95% = 1,38-17,06]). Ces associations restent significatives après ajustement sur les facteurs suivants : âge, sexe, nombre de médicaments prescrits, certains antécédents et comorbidités : OR = 2,68 [IC 95% = 1,28-5,61] pour le groupe 2 MPI ou plus et OR = 6,01 [IC 95% = 1,49-24,3] pour le groupe 3 MPI ou plus. Notre étude démontre la pertinence d'agir dans le sens d'une diminution de la prescription de MPI selon la EU(7) PIM list.

L'étiologie des troubles cognitifs chez le sujetâgé est le plus souvent plurifactorielle, mais la consommation de médicaments est souvent suspectée. En 1997, Beers et col. ont publié une liste de 28 critères permettant l'identification de médicaments potentiellement inappropriés chez le sujet âgé. L'objectif de ce travail était de répondre aux questions suivantes : 1/ Comment utiliser les tests neuropsychologiques pour l'évaluation des performances cognitives chez le sujet âgé ? Existe-t-il un "effet époque" et quelle est son importance ? 2/ Quelle est la fréquence de la consommation de médicaments potentiellement inappropriés chez le sujet âgé en France, et quels sont les facteurs associés à cette consommation ? 3/ Quel est le retentissement de la consommation de ces médicaments sur les performances cognitives du sujet âgé ? Différentes analyses ont été réalisées à partir des données actuelles au cours des dix premières années de suivi de la cohorte PAQUID et à l'inclusion dans la cohorte 3C. Dans un premier temps, des normes ont été établies pour différents tests neuropsychologiques, à partir d'un échantillon représentatif des personnes âgées de 70 ans et plus. Elles devront être néammoins actualisées régulièrement de manière à tenir compte de "l'effet époque". Comme cela est montré dans une seconde étude, les générations les plus récentes présentent en effet de meilleures performances cognitives que les générations plus anciennes, du fait notamment d'un allongement de la durée des études. Dans un second temps, une étude transversale a été réalisée pour décrire la consommation de médicaments potentiellement inappropriés chez le sujet âgé en France, selon les critères de Beers de 1997. Les résultats montrent que cette consommation est fréquente (près de 40 %). Elle est surtout liée à la consommation de vasodilatateurs cérébraux (23,4 %), de benzodiazépines à demi-vie longue (9,2 %), et de médicaments anticholinergiques (6,4 %). Enfin, la troisièmpe partie de ce travail s'intéresse aux médicaments pouvant avoir un effet délétère sur la cognition chez le sujet âgé. Deux classes de médicaments ont été étudiées plus spécifiquement : les médicaments anticholinergiques et les vasodilatateurs. Concernant les médicaments anticholinergiques, les résultats d'une étude transversale montrent que leur consommation est associée à de faibles performances cognitives chez le sujet âgé. Concernant la problématique française des vasodilatateurs, les résultats d'une étude longitudinale montrent que la consommation de ces médicaments a diminué de manière significative chez le sujet âgé en France entre 1988-89 et 1998-99 et qu'elle n'est pas associée au statut cognitif. Ce travail apporte un éclairage sur la consommation de médicaments potentiellement inappropriés chez le sujet âgé en France, et plus particulièrement sur l'effet de certains de ces médicaments sur les performances cognitives. Il souligne la nécessité d'une bonne information des professionnels de santé et des personnes âgées quant aux risques associés à l'utilisation des médicaments. Aussi, une attention particulière devrait être apportée aux sujets polymédiqués ou présentant déjà un déficit cognitif.

Celle-ci est souvent légitime mais elle augmente le risque iatrogène et diminue l'observance des traitements. Elle peut également être inappropriée, exposant à des risques iatrogènes. L'objectif était d'étudier l'évolution de la consommation de médicaments potentiellement inappropriés (MPI), sur une période de 6 mois, chez des personnes de 70 ans et plus, après une hospitalisation en gériatrie. Méthodes : une étude rétrospective a été réalisée auprès de 84 malades admis dans le service de gériatrie du CHU de Limoges en 2005. La consommation médicamenteuse était déterminée 3 mois avant l'hospitalisation [3M]; 1 mois [M1], 3 mois [M3] et 6 mois [M6] après la sortie d'hospitalisation. Les données ont été extraites de la base de données de l'Assurance Maladie. La fréquence de l'utilisation de MPI était établie à partir des critères de Beers adaptés à la pratique française par un groupe d'experts. Résultats : le nombre de médicaments remboursés était de : 9,0 +/- 4,5 avant l'hospitalisation, 10,3 +/- 4,1 à M1, 8,4 +/- 3,6 à M3 et 9,4 +/- 4,4 à M6. Cette évolution était non linéaire. Plus de 80% des sujets étaient polymédiqués. Avant l'hospitalisation, 59,5 des malades avaient au moins un MPI et 49,4% à M1, 37,7% à M3 et 55,3% à M6. Les médicaments les plus prescrits étaient les vasodilatateurs cérébraux (28,6%) dont le nombre était divisé par trois pendant le séjour. Une réduction significative de la prévalence de la consommation de MPI a été retrouvée 3 mois après la sortie d'hospitalisation suivie d'une hausse de 3 mois après. Conclusion : il n'a pas été observé une nette baisse de la consommation des MPI à M1. On observe ici plus les fluctuations des remboursements des médicaments. Toutefois, la prévalence des MPI est trop élévée ; des actions de prévention doivent être menées.
