Book excerpt: This ECVP is a new technical product that aims to anticipate and collectively delineate the clinical trial, observational and other data likely to be required by global and national policy-makers, and to do so when most valuable – during development of the late-stage clinical plan, including the design of the pivotal licensure trial. Proactively generating this evidence as part of or in parallel to clinical development – where feasible – rather than reactively post-licensure, creates an opportunity to accelerate vaccine policy formulation, adoption and impact, particularly in LMICs.

Book excerpt: WHO seeks to advance the development and deployment of new TB vaccines to prevent TB disease, with a focus on adolescents and adults in whom 90% of TB disease occurs. With new TB vaccines in the development phase, their effectiveness and the strategy for their integration will need to be determined in the context of in-country epidemiology and existing national TB control interventions. Anticipating and addressing the opportunities and challenges of vaccine delivery in different country contexts must start now in parallel to assure the implementation of successful vaccines as rapidly as possible. The ‘WHO framework to prepare for Country Introduction of New TB Vaccines for Adults and Adolescents’ intends to facilitate the planning, investments, and preparation for the rapid introduction and scale-up of TB vaccines for adults and adolescents so that they are available such that new vaccines are manufactured with sufficient, sustainable and timely supply; accessible, with equitable delivery aimed at all who could benefit and accepted, so that end-users, healthcare workers, and policy makers ́ requirements are met. This framework has been developed by an expert advisory group and through public consultation, and intentionally focuses on activities at a national level while also highlighting the global and regional activities that need to occur in tandem to accelerate equitable introduction and scale-up.

Book excerpt: The update of the Guidelines for Treatment of Drug-susceptible Tuberculosis and Patient Care is important in the context of the End TB Strategy, which recommends treatment and patient support for all people with TB. This update aims to use the best available evidence on the treatment of drug-susceptible TB and interventions to ensure adequate patient care and support in order to inform policy decisions made in these technical areas by national TB control program managers, national policy-makers, and medical practitioners in a variety of geographical, economic, and social settings. The objectives of these updated guidelines are to provide updated recommendations based on newly emerged evidence on the treatment of drug-susceptible TB and patient care as well as provide a summary of changes in the new guidelines with all the existing and valid WHO recommendations on the treatment of drug-susceptible TB and TB patient care. The key audience for these guidelines are policy-makers in ministries of health or managers of national TB programs who formulate country-specific TB treatment guidelines or who plan TB treatment programs. In addition, health professionals - including doctors, nurses, and educators working both in government services and nongovernmental organizations, such as technical agencies that are treating patients and organizing treatment services - will find these guidelines to be useful.

Available in PDF, EPUB and Kindle. Book excerpt: Vaccines have made it possible to eradicate the scourge of smallpox, promise the same for polio, and have profoundly reduced the threat posed by other diseases such as whooping cough, measles, and meningitis. What is next? There are many pathogens, autoimmune diseases, and cancers that may be promising targets for vaccine research and development. This volume provides an analytic framework and quantitative model for evaluating disease conditions that can be applied by those setting priorities for vaccine development over the coming decades. The committee describes an approach for comparing potential new vaccines based on their impact on morbidity and mortality and on the costs of both health care and vaccine development. The book examines: Lessons to be learned from the polio experience. Scientific advances that set the stage for new vaccines. Factors that affect how vaccines are used in the population. Value judgments and ethical questions raised by comparison of health needs and benefits. The committee provides a way to compare different forms of illness and set vaccine priorities without assigning a monetary value to lives. Their recommendations will be important to anyone involved in science policy and public health planning: policymakers, regulators, health care providers, vaccine manufacturers, and researchers.

Book excerpt: This document describes the main elements that World Health Organization (WHO) technical departments should elaborate on when providing guidance on evidence generation to research and development (R&D) specialists who are developing health products to address critical unmet needs in public health. While WHO’s TPPs, preferred product characteristics (PPCs) and target regimen profiles (TRPs) provide high-level information on the desired characteristics of the product, the target population, and the key endpoints for safety and efficacy evaluation, more detailed guidance is needed to enable the development of suitable trial/study designs aimed at producing relevant evidence for WHO’s normative products such as guidelines. The objective of the Guidance for evidence generation (GEG) process is to produce a guidance document on how best to perform studies for the development of novel products (diagnostics, drugs, vaccines, vector control) and thus how to enable the generation of robust data and evidence to inform a WHO evidence review process.
