Book excerpt: This report deals primarily with the analysis of the drinking-water and sanitation situation in the member countries of the Thematic Working Group on Water Sanitation and Hygiene (TWG WSH) based on statistics published by the World Health Organization (WHO) and United Nations Children s Fund (UNICEF) Joint Monitoring Programme for Water Supply and Sanitation (JMP) updated in 2015. This document also provides key information on selected health and development issues for TWG WSH member countries. The member countries covered in this report are: Brunei Darussalam Cambodia China Indonesia Japan the Lao People s Democratic Republic Malaysia Mongolia Myanmar the Philippines the Republic of Korea Singapore Thailand and Viet Nam. Previous documents prepared by WHO for the TWG WSH include extensive analyses of the drinking-water sanitation and hygiene sector as a whole based on country-level information. A key finding of this report is that the TWG WSH region has succeeded in meeting the Millennium Development Goal (MDG) target for drinking-water and sanitation which is to halve by 2015 the proportion of the population without sustainable access to safe drinking-water and basic sanitation (UN 2017). Individually nine TWG WSH countries achieved the MDG sanitation target but four countries (i.e. Cambodia Indonesia Mongolia and the Philippines) did not. All TWG WSH countries met the MDG drinking-water target except Mongolia which fell short of the target by 13 percentage points. Overall 77% of people in the TWG WSH region are using improved sanitation facilities whereas 94% are using improved drinking-water sources (UNICEF and WHO 2015). Despite an impressive effort made to provide drinking-water and sanitation infrastructure to the residents in this region about one quarter of the population still lacks access to improved sanitation and 7% lacks access to improved drinking-water. Water-related diseases including diarrhoeal diseases are significant causes of death among children under 5 years old in the region. Almost 30 000 people in the TWG WSH region especially children under 5 die each year due to water sanitation and hygiene related diseases (WHO 2014).

Book excerpt: This paper argues that there are many challenges to designing and implementing water and sanitation interventions that actually deliver economic benefits to the households in developing countries. Perhaps most critical to successful water and sanitation investments is to discover and implement forms of service and payment mechanisms that will render the improvements worthwhile for those who must pay for them. In this paper, we argue that, in many cases, the conventional network technologies of water supply and sanitation will fail this test, and that poor households need alternative, non-network technologies. However, it will not necessarily be the case that specific non-network improved water supply and/or sanitation technologies will always be seen as worthwhile by those who must pay for them. We argue that there is no easy panacea to resolve this situation. For any intervention, the outcome is likely to be context-dependent. An intervention that works well in one locality may fail miserably in another. For any given technology, the outcome will depend on economic and social conditions, including how it is implemented, by whom, and often on the extent to which complementary behavioral, institutional and organizational changes also occur. For this reason, we warn against excessive generalization: one cannot, in our view, say that one intervention yields a rate of return of x% while another yields a return of y%, because the economic returns are likely to vary with local circumstances. More important is to identify the circumstances under which an intervention is more or less likely to succeed. Also for this reason, when we analyze a few selected water and sanitation interventions, we employ a probabilistic rather than a deterministic analysis to emphasize that real world outcomes are likely to vary substantially.

An important step towards resolving this global crisis is to understand its magnitude: how many people lack access to drinking water and sanitation? To help answer this question household surveys and censuses are conducted every year throughout the world to assess drinking water sanitation and hygiene-related practices at the household level. Because of variations in survey tools attempts to compare the results of one survey with those of another have been fraught with diffi culties. Solving survey comparability problems is crucial if we are to establish accurate trends over time within a country and compare data between countries. For this reason the WHO/UNICEF Joint Monitoring Programme for Water Supply and Sanitation developed a set of harmonized survey questions that resolves the comparability problems of the past. Including the harmonized questions in national surveys and censuses will help countries gain more systematic information on the drinking water and sanitation needs of thei population. With popular use these harmonized questions will make data across international and national survey programmes more comparable which in turn will result in more accurate country regional and global estimates of unmet drinking water and sanitation needs. The core harmonized questions presented in this guide have been adopted by the Demographic and Health Surveys the Multiple Indicator Cluster Survey and the World Health Survey. The purpose of this guide is to encourage even more widespread use of these harmonized questions because accurate information is a valuable tool that can be used to assess progress towards international and national goals and targets. It also allows decision-makers and stakeholders to make evidence-based choices and direct efforts to where they are needed as well as promotes increased investment in the sector.
