Book excerpt: De récentes études neurophysiologiques en imagerie cérébrale menées par Zilbovicius et al. en 2000-2003 (in Zilbovicius, 2006) ont montré une inactivation de "l’aire de la voix" chez les personnes autistes à l’écoute de la voix humaine. En revanche, il a été observé chez cette même population une sensibilité significative à la musique et à la voix chantée. Notons que le chant comme la prosodie tiennent un rôle majeur dans le développement de la communication chez tout enfant. Pourquoi ne pas utiliser ces supports en rééducation orthophonique de l’enfant autiste? Telle est la démarche de ce mémoire réalisé au sein de l’Institut Médico Educatif "Les Tilleuls" à Tours auprès de quatre enfants âgés de 9 ans à 12 ans et atteints d’un trouble autistique associé à un retard mental. Cette étude s’inspire de différentes techniques rééducatives déjà utilisées dans d’autres pathologies (surdité, aphasie, bégaiement, dyslexie) : la méthode verbo-tonale, la thérapie mélodique et rythmée, la rythmique orthophonique et les ateliers comptines. Se complexifiant progressivement, la rééducation débute par des activités non-verbales basées sur la symbolisation des paramètres acoustiques de la prosodie, un travail du rythme et de la mélodie, pour terminer par des activités verbales utilisant la voix chantée. Parallèlement, chaque enfant a été évalué en début, milieu et fin de rééducation afin d’apprécier son évolution sur le plan de la communication et du comportement. L’analyse des résultats a permis de mieux appréhender le potentiel de chaque enfant, de considérer l’impact de cette approche rééducative chez chacun et de commencer à cerner la population apte à en bénéficier. Par cette étude, nous espérons avoir démontré en quoi notre protocole répond aux objectifs d’une prise en charge orthophonique de l’enfant autiste et nous souhaitons ainsi participer au développement de stratégies rééducatives de cette pathologie.

Méthode : 10 sujets masculins adultes avec TSA SDI et 10 sujets masculins adultes témoins ont été enregistrés lors de tâches vocales et d'expression prosodique, incluant les versants linguistique et émotionnel de la prosodie, en lecture, répétition, production et récit d'événements personnels. Résultats : L'étude montre que plusieurs paramètres prosodiques sont différents entre les sujets avec TSA SDI et les sujets témoins, traduisant une désorganisation prosodique représentée par une fréquence fondamentale haute, des intonations et un volume réduit, un rythme ralenti et confus, ainsi qu'une accentuation atypique. Conclusions : Les résultats mettent en évidence la question d'un entraînement prosodique spécifique en intervention précoce chez les enfants présentant un TSA et la nécessité de la prise en soin de la population adulte présentant ce même trouble.

After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? Do dreams equal understanding and what about the generalizability of the co-created reverie? Busch concludes that it is primarily through the analyst’s own associations that the reverie’s potential is revealed, which further helps the analyst distinguish it from many other possibilities, including the analyst’s countertransference. He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation. The Analyst's Reveries will be of great interest to psychoanalysts and psychoanalytic psychotherapists.
