Book excerpt: Abstract: Because vitamin B-6 is vital to infant central nervous system growth and because of estrogen induction of vitamin B-6 requiring enzymes, increased amino acid turnover, and fetal demand, pregnant women may need more vitamin B-6 than nonpregnant women. Estrogen-based oral contraceptives (OCA) may also increase this requirement. A study incidated that women who had vitamin B-6 intakes at the Recommended Dietary Allowance had significantly lower levels in maternal and cord serum at delivery. Use of OCA for more than 30 months lowered levels in maternal serum at 5 months gestations, in cord serum, in delivery, and in milk at 14 days. Although intakes above 5 mg/day did not eliminate differences among users in maternal serum levels at delivery and in milk at 14 days, cord serum differences were eliminated. Levels in maternal serum at 5 months gestation correlated with levels in cord and in milk at 3 and 14 days. Infants whose mothers gained 4.4 kgor more than 18 kg had lower Apgar scores at 1 minute. The mothers of infants with scores below 7 had lower intake and levels in maternal serum at delivery and in milk at 3 and 14 days.

The new Sustainable Development Goals, adopted by countries of the United Nations in September 2015, include a commitment to end the AIDS epidemic by 2030. Considerable emphasis on prevention of new infections and treatment of those living with HIV will be needed to make this goal achievable. With nearly 37 million people now living with HIV, it is a communicable disease that behaves like a noncommunicable disease. Nutritional management is integral to comprehensive HIV care and treatment. Improved nutritional status and weight gain can increase recovery and strength of individuals living with HIV/AIDS, improve dietary diversity and caloric intake, and improve quality of life. This book highlights evidence-based research linking nutrition and HIV and identifies research gaps to inform the development of guidelines and policies for the United Nations’ Sustainable Development Goals. A comprehensive approach that includes nutritional interventions is likely to maximize the benefit of antiretroviral therapy in preventing HIV disease progression and other adverse outcomes in HIV-infected men and women. Modification of nutritional status has been shown to enhance the quality of life of those suffering HIV/AIDS, both physically in terms of improved body mass index and immunological markers, and psychologically, by improving symptoms of depression. While the primary focus for those infected should remain on antiretroviral treatment and increasing its availability and coverage, improvement of nutritional status plays a complementary role in the management of HIV infection.

As it is now understood that rickets is not only caused by vitamin D deficiency, it has to be kept in mind that vitamin D and calcium deficiency is prevalent in developing countries as well as in affluent societies, where children and their mothers are not exposed to as much sunlight as they need. The rapid growth in molecular biology has been exemplified in the application of subcellular technologies to study vitamin D in human and animal models.
