Book excerpt: Depuis 2013, le Centre Hospitalier de Bourg-en-Bresse forme ses professionnels de santé aux TAC (Thérapies par Activation de Conscience), un courant de l'hypnose thérapeutique. Quel est le vécu de la pratique de l'hypnose par les soignants en milieu hospitalier ? Etude qualitative par entretiens semi-dirigés menée auprès de 17 soignants de professions et spécialités différentes, formés et exerçant au Centre Hospitalier de Bourg-en-Bresse. Les professionnels soignants perçoivent un apport très bénéfique sur la prise en charge de la douleur, de l'angoisse, ainsi qu'une amélioration de la relation et de la communication avec le patient. Ils perçoivent pour eux-mêmes une amélioration de leurs conditions de travail, un moyen de mieux prendre soin d'eux, une amélioration de la cohésion dans l'équipe, ainsi que le développement d'une dynamique positive à l'échelle hospitalière. Bien que les participants de l'étude aient des avis très positifs sur les TAC, la plupart n'utilisent pas l'outil régulièrement en séances. C'est en partie à cause des freins tels qu'un manque de confiance chez les praticiens quant à leurs capacités à proposer l'outil, et les idées reçues chez les personnels non formés qui empêchent l'usage régulier des TAC en séance. Ces obstacles peuvent être surmontés avec des pistes d'amélioration concernant la formation et l'accompagnement à l'hôpital. Les TAC apportent de nombreux bénéfices à l'échelle du Centre Hospitalier de Bourg-en-Bresse, mais des nombreux freins empêchent leur pratique de façon régulière. Des améliorations dans la formation pourraient favoriser l'utilisation des TAC en pratique quotidienne conjointement aux pratiques de soins conventionnels.

Son efficacité est démontrée dans de nombreuses publications. L'objectif de l'étude est d'explorer comment la formation à l'hypnose peut impacter la pratique professionnelle des soignants. Cette étude qualitative a été menée par auto-questionnaire informatisé auprès des soignants formés à l'hypnose lors du diplôme universitaire d' « hypnose thérapeutique et médicale pour les professionnels de santé » ou grâce à l'institut Globalescence. Les participants devaient répondre à des questions selon trois thèmes : les raisons de leur formation, leur mode de pratique/non pratique de l'hypnose et les effets sur leur pratique. Le recueil des réponses s'est réalisé du 20 juin au 10 juillet 2017. Il a été recueilli 43 questionnaires. Ceux-ci montrent que l'hypnose était une pratique singulière et complémentaire au système de soin actuel. Elle influait sur la communication, améliorait le temps et la qualité d'écoute, le langage et l'attention portée à la communication non verbale. Il y avait un impact sur la place du patient, plus au centre de sa prise en charge, qui devenait co-acteur des soins. Il existait une remise à l'équilibre de la relation soignant - soigné. Enfin le soignant ne portait plus le soin mais accompagnait le patient, qui mobilisait ses propres ressources. Conclusion : La formation à l'hypnose propose de concevoir la place du patient et celle du soignant autrement, dans un souci de bienveillance, d'efficacité et d'égalité. Sa reconnaissance institutionnelle pourrait permettre d'encadrer son enseignement et sa pratique afin d'accompagner au mieux les patients.

Nous nous sommes intéressés aux patients qui ont eu recours à l'hypnose dans leur parcours de soins. A travers une étude qualitative prospective auprès de 30 patients dans 4 centres en Rhône Alpes. L'objectif principal était de décrire chez les patients, les représentations, les freins ou les motivations qu'ils auraient à une prise en charge en hypnose par leur médecin traitant. L'objectif secondaire était de décrire leurs attentes des modalités pratiques d'intégration de l'hypnose dans l'exercice de la médecine générale. Notre étude a démontré que le recours à hypnose se faisait majoritairement par défaut, par le biais d'un réseau informel. La présence du médecin traitant était faible, il était rarement prescripteur, parfois thérapeute mais souvent indifférent à l'hypnose. Les motifs de prise en charge étaient variés : biomédicaux, psychosociaux, obstétricaux. Les freins à l'utilisation de l'hypnose étaient liés aux représentations socioculturelles de l'hypnose, considérée parfois comme un phénomène surnaturel. L'hypnose semblait pour certains, incompatible avec l'esprit scientifique de leur médecin traitant et inadaptée à la médecine générale. Cependant les motivations des patients à l'utilisation de l'hypnose étaient souvent liées au professionnalisme de leur thérapeute avec une place importante accordée à leur formation de soignants, qui les rassurait. Tout au long de leur parcours, leur perception de l'hypnose a évolué favorablement. D'une part par l'effet de leurs expériences positives et l'efficacité de ce soin sur l'amélioration de leur état de santé. D'autre part par la qualité de ce soin centré patient, dont ils étaient acteurs. L'hypnose nous semble adaptée dans la prise en charge de pathologies variées rencontrées en médecine générale. Les patients étaient majoritairement favorables au suivi en hypnose par leur médecin traitant, notamment en raison de leur relation privilégiée préexistante. Ils étaient en demande d'une reconnaissance sociétale de l'hypnose et de son service médical rendu. Ils souhaitaient une évolution de la place de l'hypnose en médecine comme un soin à part entière s'intégrant dans leur prise en charge globale, de manière complémentaire. Ils demandaient l'encadrement de cette pratique et l'amélioration de son accessibilité. Ils souhaitaient une meilleure sensibilisation des professionnels à ce soin par des formations de qualité.

Fred Busch ponders if it has been fully recognized that some post-Bionions suggest a new, radical paradigm for what is curative in psychoanalysis. After searching for the roots of the analyst’s use of reverie in Bion’s work and questioning whether in this regard Bion was a Bionian, Busch carefully examines the work of some post-Bionians and finds both convincing ways to think about the usefulness and limitations of the analyst’s use of reverie. He explores questions including: From what part of the mind does a reverie emerge? How does its provenance inform its transformative possibilities? Do we over-generalize in conceptualizing what is unrepresented, with the corresponding problem of false positives? He believes in the importance of converting reveries into verbal interpretations, a controversial point amongst post-Bionians. Busch ends with the difficult task of classifying the analyst’s reveries based on their degree of representation.
