Nous avons étudié le rôle pronostique de la déformation régionale et globale du ventricule droit par la technique du «Speckle Tracking Imaging». Méthodes: Nous avons réalisé une étude de cohorte longitudinale prospective bicentrique entre 2011 et 2014 incluant des patients adultes porteurs d'une HTP à l'exclusion du groupe 2. Tous les patients ont bénéficié à l'inclusion d'une échocardiographie, d'un dosage de BNP, d'un test de marche de 6 minutes (TM6). Les critères d'exclusion étaient le refus de participation, un rythme non sinusal et une qualité d'image échographique insuffisante. Le suivi a été réalisé de façon régulière jusqu'à la fin de l'étude. Résultats: 112 patients ont été inclus, 70% du groupe 1 (dont 34,8% de cardiopathie congénitale), 18% du groupe 3 et 10% du groupe 4. A l'inclusion, 78 patients (70%) étaient en classe NYHA III-IV et 74 (66%) étaient sous thérapies spécifiques. Au terme d'un suivi médian de 16,6 mois, 29 (n=25,9%) patients sont décédés. Les facteurs échographiques associés à la survie étaient la fonction diastolique VD et VG, l'index d'excentricité, la POD, le ratio OD/OG et la déformation radiale ventriculaire droite. Dans le groupe HTAP congénitale, une meilleure survie a été observée (p= 0,02) et associée à une déformation radiale VD augmenté ainsi qu'une déformation apicale prédominante. Conclusion: La déformation radiale VD est un facteur prédictif indépendant de survie dans l'HTAP et ce remodelage pourrait expliquer la meilleure survie des patients avec HTAP associée aux cardiopathies congénitales.
