Mais aujourd'hui, ce geste est de plus en plus remis en question lorsque la naissance se passe bien, à terme. En plus d'aller à l'encontre des recommandations de l'OMS, plusieurs études lui reprochent des effets délétères, des peturbations chez le nouveau-né. Nous avons réalisé une enquête auprès des sages-femmes et de pédiatres dans 13 maternités de France afin de discuter de cette pratique. Pendant la réalisation des soins aux nouveau-nés, des gestes invasifs peuvent être évités. L'aspiration gastrique n'est pas utile lors d'une naissance normale ; c'est le test de la seringue qui permet de faire le diagnostic d'atrésie de l'oesophage. L'atrésie des choanes peut être recherchée par des moyens non traumatiques comme les tests au coton, au miroir ou au sérum physiologique. Quant au dégagement des voies aériennes supérieures, il est difficile de savoir si tel enfant réussira à éliminer toutes ses glaires et ne sera pas gêner pour s'alimenter. Des sages-femmes se contentent d'essuyer le nez et la bouche du nouveau-né. Il a été constaté une amélioration du bien-être des enfants non aspirés à la naissance ; mais ceci est également dû au contact peau à peau précoce. Il est donc important de laisser la mère et son enfant ensemble le plus longtemps possible sans les perturber et remettre à plus tard ce qui n'est pas primordial.

Book excerpt: Objectifs L'objectif principal de cette étude est d'évaluer les pratiques professionnelles des sages-femmes en ce qui concerne le geste d'aspiration des voies aériennes supérieures sur les nouveau-nés bien portants nés à terme et par voie basse. L'objectif secondaire est d'évaluer la nécessité de la mise en place d'un protocole pour encadrer cette pratique. Matériel et méthodes L'étude, de type quantitatif, a été menée du 20 janvier 2015 au 17 avril 2015 à l'aide d'un questionnaire auprès des sages-femmes effectuant des gardes en salle de naissance au sein du CHI de Poissy/Saint-Germain-en-Laye (centre de niveau III). Résultats Nous avons reçu 45 réponses, ce qui représente 53,6% de la population cible totale. En regard des recommandations actuelles, les résultats montrent que l'aspiration est effectuée trop fréquemment sur des enfants dont l'état ne le nécessite pas. 53,3% de la population étudiée effectue une aspiration en systématique et 46,7% le fait en fonction de l'état de l'enfant. Par ailleurs, seulement 24% des sages-femmes interrogées aspirent uniquement les voies aériennes supérieures. 76% vont au-delà du pharynx jusque dans l'œsophage et/ou l'estomac, ce qui présente un risque important de bradycardie. L'étude souligne la nécessité de mettre en place un protocole concernant l'aspiration des voies aériennes supérieures sur le nouveau-né bien portant. 89% des sages-femmes souhaitent la mise en place d'un tel protocole.

Cette pratique est en constante évolution et a connu des changements au cours de ces dernières années. Les professionnels de santé sont donc amenés à modifier continuellement leurs habitudes en accord avec les nouvelles recommandations. L'objectif principal de cette étude était d'analyser la manière dont les professionnels effectuent l'aspiration et les raisons pour lesquelles il la pratique à l'heure actuelle. L'autre objectif de cette étude était d'étudier les situations dans lesquelles l'enfant avait eu ce geste et d'évaluer l'impact sur le nouveau-né. Matériel et méthode : Une étude descriptive, prospective, monocentrique a été réalisée au sein de la maternité du CHU de Dijon auprès des professionnels de santé à l'aide d'un questionnaire et des dossiers médicaux informatisés sur le logiciel AGOPRA afin de recueillir les informations complémentaires. Toutes les données ont été anonymisées. Résultats : 305 patients ont été inclus dans notre étude. Les professionnels de santé au CHU de Dijon n'aspirent plus systématiquement les nouveau-nés. L'aspiration est systématiquement réalisée de manière justifiée et conforme aux recommandations. En revanche, ce geste est peut-être effectué trop précocement et de manière trop invasive en allant jusqu'à l'estomac. Il semble exister des difficultés alimentaires significativement plus importantes lors de la première tétée chez les enfants aspirés et ces difficultés semblent persister par la suite. Conclusion : Une sensibilisation auprès des praticiens et un protocole plus actualisé dans le service serait à envisager pour permettre une amélioration des pratiques. En effet l'AVAS semble impacter la 1ère tétée et l'alimentation en suites de couches. Une étude prospective multicentrique avec une cohorte plus importante permettrait de mieux apprécier l'impact de l'AVAS sur l'alimentation.
