Book excerpt: This work describes the development of an optical probe for measuring movement of tissue particles deep inside which are loaded by an ultrasound remote palpation device. The principle of the method is that ultrasound force which can be applied inside the tissue makes the tissue particles vibrate and this vibration phase-modulates the light intercepting the insoniified region which results in a modulated speckle intensity on detection outside the object. This speckle intensity modulation detected through the measured intensity autocorrelation is a measure of the vibration amplitude. Since the vibration amplitude is related to the local elastic properties of the medium, the measured modulation depth in intensity autocorrelation can be used to map the elastic property in the insonified region. In this work, first the ultrasound induced force is calculated for both plane and focused ultrasound beams, and converted to amplitude of vibration and refractive index modulation, solving the forward elastography equation. Light propagation inside an insonified object is modelled using Monte Carlo simulation and the amplitude and intensity correlations are computed. The modulation depth on the autocorrelation is estimated and shown that it is inversely correlated to the local elastic modulus and optical absorption coefficient. It is further shown that whereas the variation in modulation depth is linear with respect to absorption coefficient, the same variation with elastic property is nonlinear. These results are verified experimentally in a tissue mimicking phantom. The phantom was constructed out of poly vinyl alcohol(PVA) whose optical, mechanical and acoustic properties are independently controlled. It is also shown that for loading with focused ultrasound beam the displacement is almost along the ultrasound transducer axis and therefore the contribution from refractive index modulation alone can be ascertained by probing the insonified perpendicular to the transducer axis.

Book excerpt: Skeletal muscle injuries are frequent in sports and exercise, accounting for between 2055% of all sports injuries, with the vast majority of these injuries being muscle strains or contusions. Although muscle injuries are primarily diagnosed through symptomatology and examination of the injury mechanism, medical imaging techniques such as magnetic resonance imaging (MRI) and ultrasound imaging (US) are gaining popularity for assistance with both diagnoses and prognoses. These imaging techniques provide valuable structural and physiological information that is provides more insight into the nature and extent of the damage to guide image treatment. Currently, MRI is the gold standard for use with musculoskeletal applications, but US is becoming more widespread due to its affordability, portability, and real-time imaging capabilities. Ultrasound elastography (USE) is a family of techniques that are used to allow the visual analysis and quantification of the mechanical properties of soft tissue. Injury induced changes in muscle structure affect the mechanical properties of the tissue, and can be detected using USE. Since the echogenicity of a tissue is not directly related to its mechanical properties, ultrasound elastography can extract important information about tissue stiffness and deformability that might not otherwise be attainable. Such mechanical information is especially useful in distinguishing between fibrotic tissue and fatty infiltration. The goal of this thesis is to develop USE as an affordable, fast, and readily available alternative to MRI for early stage diagnosis of acute muscle injury. In pursuit of this goal, we first developed a technique to obtain repeatable and reproducible strain images using USE. We found that a simple averaging procedure performed on 4-8 repeated USE compression cycles significantly improved both the reproducibility and repeatability of the resulting strain images compared to strain images generated using an automated USE system. We then showed that USE combined with principal component analysis can be used to quantify and locate muscle injury in a finite element model, and can feasibly be applied to strain images generated using USE. Finally, we attempted to use the procedures developed in the first two studies to detect and monitor contusion injury in a rat model. However, due to equipment limitations, the results of this study were inconclusive. This thesis is concluded by a summary of the overall findings and discussion of future work.

Third, of ophthalmological and dermatological applications, optical coherence passive elastography was tested in a phantom and a cornea of healthy mouse. Also, a fully optical setup was established to image surface wave for elastography applications. Finally, the resolution limit of elastography was measured using and ultrasound ultrafast scanner.

Book excerpt: Abstract : Ultrasonic elastography (UE) is a set of ultrasonic imaging techniques used to noninvasively quantify material properties of soft tissues. UE comes in two major forms, strain elastography (SE), in which compressional force is used to produce strain images of human tissues, and shear wave elastography (SWE) in which dynamic excitation is used to induce shear waves and (typically group) shear wave speeds (SWS) are collected from their propagation. Clinical interest in UE lies in its potential for noninvasive screening for pathologies with associated biomechanical property changes, such as breast lesion classification and staging of liver fibrosis. The diagnostic utility of UE hinges on its ability to consistently detect and characterize the underlying mechanical properties of diseased and healthy tissues. A fair amount of analysis and effort has been invested in investigating signal processing and measurement related uncertainty in SWE and SE. Comparatively less attention has been paid to investigating the uncertainty associated with the complex and multifaceted continuum biomechanics that underlies the response of tissue to an UE measurement. This dissertation contains my contributions toward delineating the role of several of these biomechanical facets on UE measurements. Its chapters are composed of a collection of five articles. In the first article, we investigated the contrast transfer characteristics of an advanced form of SE called viscoelastic imaging through mechanical simulations, which was found to be influenced by the non-viscous material characteristics. This motivated a follow up simulation study were we applied a systematic design-of-experiments based approach to determine the material and user dependent factors that most strongly influence traditional SE contrast. We found that along with the initial elasticity of the tissue, elastic nonlinearity and precompression seem to play substantial roles in SE contrast. The other three articles contain my contributions to SWE. Here, a robust approach to SWS estimation based on the method-of-characteristics solution to the 1D elastic wave equation was developed. I also contribute to the theoretical development of acoustoelastic analysis in SWE. From these efforts, it was found that only certain material models could accurately represent SWS along multiple principle axis of deformation and that acoustoelastic analysis can be applied to understanding SWE measurements in the presence of fluid-induced deformation.
