Book excerpt: Performing hazards analyses and fire risk assessments on oxygen systems is critical to ensure that a system design has a low probability of ignition and a low consequence of ignition. Historically several different methods have been used to identify and evaluate fire hazards in oxygen systems. Most of these methods, however, are designed for detailed materials or component analyses, and can be impractical for large-scale oxygen systems with many components and even multiple end processes. Wendell Hull & Associates, Inc. (WHA), in conjunction with Oxygen Safety Consultants, Inc. and INCO Ltd., has developed an efficient approach to identify and evaluate hazards in large-scale oxygen systems. The WHA oxygen hazards and fire risk assessment process considers the important aspects of oxygen system safety as they relate to design, cleaning, operations and maintenance, and uses ASTM Standard Guide G 88 as a basis for quickly identifying and ranking system-level oxygen hazards. ASTM G 88 is a standard guide for oxygen system design containing many system-level factors known to contribute to ignition and propagation of fires. The WHA analysis process applies ASTM G 88 at the system level, using both schematic analysis and visual inspection to systematically assign a hazard severity ranking to all system components, allowing components that possess the greatest risk of ignition and fire propagation to be quickly identified and further evaluated. This paper describes the WHA oxygen hazards and risk assessment method, using ASTM G 88 to quickly identify and assign a hazards severity ranking to components in large-scale oxygen systems.

The NASA Johnson Space Center White Sands Test Facility developed a method for analyzing the hazards of oxygen systems and components. Using this method, material flammability and the risks associated with various ignition modes were addressed. In addition, secondary hazards, such as those caused by seal leakage and the formation of oxygen-enriched environments exterior to pressure-retaining components were addressed. This hazards analysis method is presented as a systematic investigation tool and as a means of training individuals on the nature and level of severity of oxygen-enriched combustion. Three case studies drawn from work performed at the NASA Johnson Space Center White Sands Test Facility are presented. These case studies were conducted on a space flight component, an aircraft flight system, and a facility installation.

Cleanliness limits for nonvolatile residues, ranging from 10 to 500 mg/m2, have been established for various industries and types of oxygen systems to reduce the risk of ignition of flammable organic films. Particulate cleanliness limits used for oxygen systems vary considerably. Maximum allowed particle size, quantity limits, and allocations for fibers or metallic particles are all variables seen in aerospace cleanliness limits. Particles are known to have the potential to ignite within oxygen systems and must be limited to prevent fires. Particulate contamination may also pose risks to the performance of oxygen systems that may or may not be related to ignition hazards. An extensive literature search was performed to better understand the relative importance of particle ignition mechanisms versus other deleterious effects of particles on oxygen systems and to identify rationale for derivation of particulate cleanliness limits for specific systems. The identified risks of different types and sizes of particles and fibers were analyzed. This paper summarizes the risks identified and rationale that may be used to derive particulate cleanliness limits for specific oxygen systems.
