Book excerpt: Despite the wide prevalence of urogynaecological problems, in clinical practice, there is a paucity of specialists that are skilled in the management of these conditions. The recognition of the need for a specialist to deal with these specific problems has led to the recent evolution of urogynaecology as a subspecialty. This book, Principles and Practice of Urogynaecology aims to equip the practicing professionals - Gynaecologists, Urogynaecologists and Urologists, with up-to-date information on the principles that guide the evaluation and management of pelvic organ prolapse and other common urogynaecological problems. With an emphasis on evidence based medicine, the book aims to deliver guidance on management of common urogynaecological problems and provides information on the latest cutting-edge surgical techniques. Written by global experts in the field of urogynaecology, the book focuses initially on pelvic floor anatomy and function, moving seamlessly to the evaluation and management of clinically relevant pelvic floor problems. A detailed discussion on management of mesh related complications is a highlight.

Many sufferers never seek help; many who do are treated inappropriately or inadequately due to a lack of knowledge or understanding. This is a clear, practical, and basic guide to the problem of urinary incontinence in women, aimed at doctors, nurses, and other involved professionals. It describes the wide range of investigations now available to provide an accurate diagnosis, and explains how the correct treatment may be planned on the basis of this diagnosis. It discusses management options ranging from simple pelvic floor exercises and bladder drill to surgery and drug therapy, and covers specific problems such as incontinence in the elderly and in pregnant women. Not everybody with urinary problems can be cured, but the vast majority can be helped to lead a normal life. This book will be of value to all members of the health care team who are concerned with this important area of women's well-being including physicians, nurses, residents, continence advisers, and family practitioners.

Available in PDF, EPUB and Kindle. Book excerpt: According to the International Association for Ambulatory Surgery (IAAS), ambulatory surgery should be defined as 'an operation/procedure, excluding an office or outpatient operation/procedure, where the patient is discharged on the same working day'. The rise of ambulatory surgery has been driven by technological advances that reduce the need for overnight hospital stays, enhanced recovery programmes that advocate early mobilisation, and the need for economic efficiency. Recent experience has shown that redistributing surgical procedures from the inpatient setting to ambulatory centres can be done without impacting quality. The majority of people requiring urologic surgery are now treated as day/outpatients thus requiring a different level of care from inpatients. Ambulatory Urology and Urogynaecology is the only book that combines urology and urogynaecology focuses on outpatient management. Packed with learning points, practical hints and tips, and boasting an international group of contributing authors, this book is co-edited by world-leading pioneers in urologic and urogynaecological surgery. Ambulatory Urology and Urogynaecology is ideal for urologists, urogynaecologists and gynaecologists, as well as specialist urology nurses and surgeons. With more and outpatient procedures being performed, this book is the perfect step-by-step guide to consult time and time again.

Book excerpt: While the upper renal tract remains the sole preserve of the urologist, gynaecologists have traditionally been involved in the assessment and treatment of female lower urinary tract infection disorders. Childbirth is a major causal factor in urogynaecology, and women often present with a combination of menstrual, genital, urinary and colorectal symptoms which require a holistic approach. This book provides a comprehensive overview of the assessment and treatment of a range of urogynaecological problems: urinary incontinence; lower urinary tract symptoms; overactive bladder; recurrent urinary tract infection; haematuria; painful bladder syndrome and interstitial cystitis; pregnancy and the renal tract; ageing and urogenital symptoms; fistulae and urinary tract injuries; pelvic organ prolapse; colorectal disorders; obstetric anal sphincter injuries. It has been comprehensively updated to reflect current clinical practice and new research since publication of the original edition.

Book excerpt: Laparoscopic surgery for the treatment of disorders such as urinary incontinence and pelvic organ prolapse is evolving rapidly with few resources available for clinicians. This text will act as a gold standard reference in the field of laparoscopic urogynaecological surgery. The introductory section covers the basics of laparoscopy, including patient selection, surgical set up and the prevention and management of complications. Further sections focus on different “gold standard” techniques and the procedural steps needed to perform the surgery, including chapters on colposuspension, paravaginal repair, laparoscopic hysterectomy as well as apical suspensory surgery such as sacrocolpopexy and sacrohysteropexy. The final section includes debates and opinion pieces on newer techniques as well as discussion on the use of mesh in treating pelvic organ prolapse. There is also a section addressing the current rise in robotic surgery. The editors and contributors are all experts in the field, providing an authoritative and global view on techniques. Highly illustrated, with videos demonstrating the techniques, this is an eminently practical guide to the use of laparoscopy in urogynaecology.

Book excerpt: This lavishly illustrated operative atlas consists of detailed, step-by-step descriptions of the procedures used in reconstruction of the female urinary tract from the kidney to the urethra. It is based on the extensive operative experience of two very experienced reconstructive urologists. The procedures described have been devised compeletely afresh or considerably developed during the course of many thousands of operations and thus represent a unique collection. Many of the procedures and "tips" have not been published before. The approach taken throughout the book is a functional one. The basic principles underlying reconstrucive urology are outlined and emphasis is placed on the fact that the logic of each procedure is dictated by a thorough understanding of the particular functional abnormality involved. In addition there is a wealth of practical tips, observations and clinical pearls built up over many years of surgical experience which help to reduce complications and make operations easier. Each operative procedure is described by a sequence of colour photographs with running captions explaining the pitfalls and points of particular importance at each stage. It will be an indispensible guide to reconstructive urology for urologists and gynaecologists alike.

Book excerpt: This is a unique book in that it brings together the two key investigative techniques in Gynaecology and Obstetrics, namely ultrasound and endoscopy. So often in the past they were regarded by their exponents as rival techniques but it is now recognised that they are complementary to each other. Consequently future trainees in endoscopy should become efficient in transvaginal sonography and vice versa. Ultrasound can be used to study the morphology of the pelvic organs such as the endometrium, myometrium and ovaries, and being non-invasive, safe and convenient can be repeated as often as is deemed necessary to monitor changes over time; for example in the investigation of the infertile woman the development of the dominant follicle, the maturation of the endometrium and the formation of the corpus luteum can be documented throughout the menstrual cycle while growth of ovarian cysts can be precisely measured to determine the need for surgery. A seldom-mentioned strength of the ultrasound examination is its interactive quality, for example by performing abdominal palpation during the scan, the mobility of the uterus and ovaries can be assessed. Also the images as they appear can be shown to the patient to aid understanding. Ultrasound can also determine function and the use of Doppler has been used for example to access endometrial receptivity, follicular maturity and the likelihood of malignancy in the endometrium or ovary.
