Book excerpt: Introduction : Tout comme les terriens, les marins peuvent développer des pathologies au niveau de la sphère bucco-dentaire, cependant une contrainte majeure apparaît chez ce type de population : l'isolement. La prise en charge d'une urgence dentaire en mer va donc devoir s'adapter au milieu dans lequel évolue le marin, mais également prendre en compte différentes composantes, à savoir le matériel disponible à bord mais également les compétences du patient. Dans ce contexte, nous nous sommes demandés si les modalités de prise en charge des urgences dentaires lors de navigation hauturière étaient connues et appliquées par le public amateur et professionnel de la navigation à la voile. Méthodes : Nous avons réalisé une étude transversale rétrospective dans le but d'évaluer les pratiques des professionnels et amateurs de la voile en diffusant un questionnaire au sein du Club nautique de la marine de Toulon et du Pôle Finistère de course au large sur la période du 25/09/2020 au 28/12/2020. Résultats : 77 participations furent traitées et analysées. Discussion : Nous avons pu constater que certains moyens de prévention n'étaient pas suffisamment employés comme la consultation chez un dentiste dans les 6 mois précédant un départ, ou encore qu'il restait des progrès à faire afin d'homogénéiser le contenu des trousses à pharmacie par exemple. La prise en charge médicale est limitée du fait de l'impossibilité de consulter un chirurgien-dentiste directement au moment de l'urgence, cependant le développement technologique que nous connaissons corrélé à l'essor de la télémédecine permettent tous deux une nette amélioration des entretiens. Conclusion : Il serait intéressant de plus sensibiliser la population des marins afin de diminuer le nombre d'urgences dentaires en mer en utilisant à bon escient les méthodes préventives que nous connaissons mais également la sensibiliser afin de maîtriser les outils et protocoles disponibles pour contacter les secours.

Book excerpt: Countries that have made the most progress toward universal coverage have public expenditures in health equivalent to at least 6% of their gross domestic product (GDP), which is the percentage established in PAHO's universal health strategy as the benchmark for the countries. However, while higher expenditure is a prerequisite, it is not enough to combat inequities and advance toward universal health. In addition to greater resources, the quality of the expenditure must be improved, reducing health system inefficiencies. Moreover, public expenditure in health should be sustainably increased in a fiscally responsible manner. The concept of fiscal space for health refers to the ability of governments to provide additional budgetary resources for the health system without affecting the financial position of the public sector or supplanting other socially necessary expenditures. Any analysis of fiscal space, therefore, will attempt to identify the prospects for increasing health expenditure in the short and medium term to address a series of clearly established health needs. These efforts are under way at a critical time in the Region of the Americas, particularly in the countries of Latin America and the Caribbean, which are engaged in a singular health system reform process. For the first time in history, these countries have formalized their intention of increasing public expenditure in health, putting themselves firmly on the path to real and effective access to health care through the universal health strategy. Without achieving basic well-being at this level, it will be impossible to improve social cohesion and social development in the countries of the Region. This publication brings together and summarizes PAHO's studies on fiscal space for universal health in the Americas and draws on the contributions of the regional forum held in Washington, D.C. on 7-8 December 2015. With this publication, whose target audience is the technical personnel responsible for policy development, decision-makers, and authorities, PAHO hopes to contribute to the analysis and discussion of health financing policies on the path toward universal health.
