L'esquisse de ces techniques sera dessinée au fil de ce travail, avec un focus particulier sur l'hémodialyse, méthode à ce jour encore la plus utilisée. À ses débuts, les résultats proposés par cette procédure, assortis d'une lourde mortalité, étaient peu concluants. Jusque dans les années 1960, l'hémodialyse n'était utilisée que brièvement pour le traitement de l'insuffisance rénale aigue (IRA). Le besoin de purifier l'eau de dialyse n'était alors, pas évident. Ce n'est que plus tard, que l'amélioration de la technique permit de soigner efficacement les signes associés à la maladie. Avec les années 1970 et l'expansion de la technique dans des centres spécialisés, les premiers signes de bactériémie et de réactions pyrogèniques post-dialyse ont été observés. L'exposition chronique de patients dialysés à un dialysat de qualité insuffisante fut responsable de l'apparition de nouveaux maux. La dialyse a amélioré le pronostic et la qualité de vie des patients insuffisants rénaux chroniques (IRC), mais au prix de nouvelles complications. Les professionnels de santé rattachés au monde de la dialyse se sont appliqués au quotidien à l'amélioration de ces techniques, de manière à en diminuer les répercussions sanitaires potentielles. Notre travail ne portera que sur les fluides pour hémodialyse et leur qualité, puisque leur fabrication extemporanée est sous la responsabilité seule du pharmacien en charge du traitement de l'eau. À l'inverse des poches de dialysat en dialyse péritonéale qui sont d'origine industrielle garantissant ainsi une reproductibilité de produits de haute qualité, les liquides de dialyse sont fabriqués extemporanément et nécessitent la mise en place d'un système d'assurance de qualité pour garantir une sécurité sanitaire optimale.Cette thèse débutera par une approche globale de l'anatomie, de la physiologie rénale, puis d'un rappel de la définition de l'insuffisance rénale chronique (IRC), de manière à resituer l'intérêt de la dialyse dans le traitement du stade terminal de la maladie. À partir d'ouvrages de référence et de normes internationales sera ensuite détaillée la place de l'eau dans la fabrication extemporanée du dialysat. La question de la recherche de la contamination bactérienne du dialysat, grâce aux techniques issues des travaux de biologie moléculaire, sera également abordée. La philosophie du traitement de l'eau de dialyse sera enfin développée afin de laisser place, pour finir, aux répercussions cliniques pouvant s'observer en cas d'une contamination du liquide de dialyse.

Long overlooked as a milder form of pain, itching has rapidly gained a new appreciation in both research and clinical communities because of its complexity and its negative effects on the quality of life of the distressed patients. Like pain, not all itches are the same. Unlike pain, there are no standard drugs equivalent to aspirin and morphine. Epidemiological studies emphasize the high incidence and economic costs of itch (pruritus). It is the most prevalent symptom of a wide variety of allergic and inflammatory skin conditions (e.g., psoriasis, atopic dermatitis), is associated with several systemic diseases (e.g., chronic kidney and liver disease), and occurs in patients undergoing hemodialysis, spinal administration of opioids, and in those suffering from AIDS. The reader will learn about the multiple pathways for itch and their interactions with pain. The relationship between these closely related, yet distinct sensory phenomena, will be emphasized. Both itch and pain use several common molecules to send signals to the brain. Thus, drugs that have been, and are being, developed as analgesics may also attenuate intractable itch. This has been an exciting and very necessary turn of events since traditional H-1 receptor antagonists are ineffective in blocking the pruritus associated with kidney failure and cholestasis. The clinical chapters will provide insights into contemporary treatment regimens for pruritus in different human scenarios.
