Book excerpt: Objective: This systematic review aims to comprehensively analyze the different themes of barriers that contribute to the underutilization of mental health services among Asian Americans living in the United States. Design: Systematic literature review. Methods: Nine databases were used to collect peer-reviewed articles to analyze mental health service utilization among Asian Americans. Inclusion criteria: (a) peer-reviewed articles that were published between 2001 and 2021; (b) studies that recognized the different barriers to mental health utilization among Asian Americans living in the United States; and (c) articles must be written and published in the English language. Results: Three main themes were identified as the barriers to the utilization of mental health services among Asian Americans, namely: (1) Cultural stigma, (2) different values and beliefs in the community, and (3) lack of knowledge regarding mental health services. Conclusion: Most findings reveal that the Asian American community responds to mental health issues depending on individual cultural beliefs. Although mental health services usage among Asian Americans is slowly increasing, it is essential to continue educating about mental health awareness, seeking professional services, and changing cultural perceptions.

Book excerpt: This dissertation reports three studies on mental health and service utilization among Asian Americans, using nationally representative data from National Latino and Asian American Study (NLAAS). The first study reported in Chapter 2 examines the prevalence and pattern of past year mental health-related service use, including specialty mental health services, general health services, human or alternative services, and any type of mental health-related services, among Asian Americans. Guided by Andersen's health behavioral model, this study explores influencing factors to Asian Americans' use and choice of mental health services. Findings from this study indicate that Asian Americans significantly underuse mental health services and that they tend to choose specialty mental health services and human or alternative services when seeking treatments. Marital status, age at immigration, and past year psychiatric disorder are found significantly correlated to Asian Americans' use of each and any type of mental health services. Second study reported in Chapter 3 investigates the influence of immigration and perceived social status on lifetime and 12-month psychiatric disorder occurrence among Asian immigrants. Results of the study suggest that Asian immigrants' age at immigration and perceived social status in the U.S. are significantly associated with lifetime and 12-month psychiatric disorder occurrence. Respondents who immigrate to the U.S. during childhood and those who report lower perceived social status in the U.S. are more likely to experience lifetime and 12-month psychiatric disorders. Chapter 4 reports a study on the impacts of family cohesion and family conflict to past year mental health-related service utilization among Asian Americans. Primary findings from the study suggest the critical role of family cohesion and family conflict in influencing Asian Americans' mental health service use. Specifically, family cohesion is found to have significant correlation to Asian Americans' receipt of general health services and any type of mental health-related services. Family conflict is found as a significant predictor to the use of each and any type of mental health-related services, except specialty mental health services. The final chapter, Chapter 5, concludes this dissertation by providing summary of findings, discussion of limitations, and implications pertinent to social work practice, policy, and research.

Book excerpt: "Asian Americans and Pacific Islanders (AAPIs) underutilize mental health services (Matsuoka, Breaux, & Ryujin, 1997; Zhang, Snowden, & Sue, 1998). However, reasons for their underutilization of mental health services are largely unknown. Large-scale epidemiological studies have included only aggregate data of AAPIs. Ethnic-specific information about the mental health problems and barriers to seeking mental health services is needed. The purpose of this study was to identify barriers to seeking mental health services among Korean immigrant women using a design that included focus groups and a survey questionnaire that was completed by 237 midlife to older married Korean immigrant women living in the Chicago metropolitan area. The survey included measures on cultural, predisposing, enabling, and psychosocial factors affecting intention to seek mental health services. Bivariate regression analyses were conducted to identify key predictor variables of perceived need for mental health services. All of the significant predictors were entered into a multiple linear regression to determine the best fitting model for perceived need for mental health services. Next, bivariate linear regressions were conducted to determine if perceived need for mental health services predicted psychosocial barriers and intention to seek mental health services. A final regression model for intention to seek mental health services was computed. Korean identification, Korean language proficiency, previous experiences with mental health services, perceived mental health status, and perceived need for mental health services were significant multivariate predictors of intention to seek mental health services. Concern about shame was a significant mediator of the relationship between perceived need and intention to seek mental health services. Findings suggest a need for outreach among Korean immigrant women who may have few financial and social resources. Practitioners and researchers need to be sensitive to the report of perceived stress, even if depression and somatization are not apparent. Korean immigrant women who identify more with Korean culture may not consider mental health services to be helpful in solving the mental health problems. Future research is needed about the appropriateness of mental health services for addressing problems and how Korean immigrant women might be encouraged to seek needed services." -Summary.

Book excerpt: Although most individuals in psychological distress do not seek and receive mental health services, underutilization of services continues to be the major mental health disparity affecting certain racial/ethnic minority populations, such as Asian Americans (Sue, Cheng, Saad, & Chu, 2012). Unfortunately, this problem has persisted over the past few decades, and there is limited theoretical understanding of why this occurs. The goal of the current study was to apply a social psychological theoretical framework--the Health Belief Model (Rosenstock, 1966)--to understand why people in distress do not seek help, and moreover, why this problem is especially pronounced among Asian Americans. This study examined how levels of functioning, perceived susceptibility to mental health problems, perceived benefits of treatment, and perceived barriers to treatment influenced intentions to seek help among a sample of 395 Asian American and 261 White American individuals experiencing elevated levels of psychological distress. Analyses using structural equation modeling indicated that perceived benefits partially explained why Asian Americans had lower intentions to seek help relative to White Americans. For Asian Americans, functioning was negatively related to help-seeking, but this effect was not present for White Americans. As this study is one of few studies to concurrently assess barriers and facilitators of help-seeking among those experiencing current psychological distress, clinical implications of these results are discussed.

While the author only briefly mentions the special problems of Asian American refugees, the mental health issues discussed in the book are applicable to all Asian American immigrants, including refugees.

This study aimed to evaluate the efficacy of a media-outreach intervention and the notion of ethnic match in relationship to reducing public stigma among Asian Americans. The outreach prevention resource, the video "Overcoming Adversity: Stories of Hope and Courage," created by Asian Americans for Community Involvement (AACI), aspires to reduce stigma by sharing experiences of mental illness specific to Asian Americans. Analyses showed that public stigma was lower after watching the video intervention, suggesting that the intervention may be effective in reducing public stigma. AACI' s outreach intervention integrates the notion of ethnic match, as participants watch individuals of similar or the same ethnic background speak about their experiences with mental illness. Results showed no statistical significance between participants' perceived ethnic identification with a consumer in the video and change in public stigma, suggesting that the video may be effective as a general Asian American outreach intervention without a need for specific Asian subgroup match. Second, multiple regression suggested that age and ethnicity did not moderate change in public stigma whereas lower acculturation was a statistically significant predictor of greater change in public stigma. Results of the study suggest that a media-outreach intervention may be effective in reducing public stigma among Asian Americans, particularly for those who are born outside of the U.S
