Book excerpt: This dissertation reports three studies on mental health and service utilization among Asian Americans, using nationally representative data from National Latino and Asian American Study (NLAAS). The first study reported in Chapter 2 examines the prevalence and pattern of past year mental health-related service use, including specialty mental health services, general health services, human or alternative services, and any type of mental health-related services, among Asian Americans. Guided by Andersen's health behavioral model, this study explores influencing factors to Asian Americans' use and choice of mental health services. Findings from this study indicate that Asian Americans significantly underuse mental health services and that they tend to choose specialty mental health services and human or alternative services when seeking treatments. Marital status, age at immigration, and past year psychiatric disorder are found significantly correlated to Asian Americans' use of each and any type of mental health services. Second study reported in Chapter 3 investigates the influence of immigration and perceived social status on lifetime and 12-month psychiatric disorder occurrence among Asian immigrants. Results of the study suggest that Asian immigrants' age at immigration and perceived social status in the U.S. are significantly associated with lifetime and 12-month psychiatric disorder occurrence. Respondents who immigrate to the U.S. during childhood and those who report lower perceived social status in the U.S. are more likely to experience lifetime and 12-month psychiatric disorders. Chapter 4 reports a study on the impacts of family cohesion and family conflict to past year mental health-related service utilization among Asian Americans. Primary findings from the study suggest the critical role of family cohesion and family conflict in influencing Asian Americans' mental health service use. Specifically, family cohesion is found to have significant correlation to Asian Americans' receipt of general health services and any type of mental health-related services. Family conflict is found as a significant predictor to the use of each and any type of mental health-related services, except specialty mental health services. The final chapter, Chapter 5, concludes this dissertation by providing summary of findings, discussion of limitations, and implications pertinent to social work practice, policy, and research.

The author's conclusions include: 1) the rate of mental disorders among Asian Americans has been underestimated; 2) the expression of symptoms is influenced by culture; and 3) the Asian American family has both positive and negative effects on mental health. While the author only briefly mentions the special problems of Asian American refugees, the mental health issues discussed in the book are applicable to all Asian American immigrants, including refugees.

Design: Systematic literature review. Methods: Nine databases were used to collect peer-reviewed articles to analyze mental health service utilization among Asian Americans. Inclusion criteria: (a) peer-reviewed articles that were published between 2001 and 2021; (b) studies that recognized the different barriers to mental health utilization among Asian Americans living in the United States; and (c) articles must be written and published in the English language. Results: Three main themes were identified as the barriers to the utilization of mental health services among Asian Americans, namely: (1) Cultural stigma, (2) different values and beliefs in the community, and (3) lack of knowledge regarding mental health services. Conclusion: Most findings reveal that the Asian American community responds to mental health issues depending on individual cultural beliefs. Although mental health services usage among Asian Americans is slowly increasing, it is essential to continue educating about mental health awareness, seeking professional services, and changing cultural perceptions.

Using California Health Interview Survey (CHIS), secondary data analysis was used to further understand mental health status and utilization of mental health services amongst Asian-Americans in California. Specific ethnic subgroups of the study were Japanese, Chinese, Vietnamese, Filipino and Korean. Participants of the secondary data analysis identified as adults, ages 18 and up. The specific aims of the study were to understand overall mental health issues, if mental health had an influence on daily activities, and to know the insurance rate coverage for mental health treatment amongst Asian-Americans. Results of the study have found significant differences amongst ethnic subgroups for overall mental health issues as well as interference of mental health in daily activities. However, no statistical significance was found for insurance rate coverage for mental health treatment amongst subgroups. This study concludes with the need to further understand mental health in Asian-Americans and tailor programs to better suit the needs for utilization of services.

The striking growth and intra-group diversity of OAA underline the urgency for an improved understanding of ethnic Asian groups' health care access and service utilization patterns. This dissertation therefore examines health seeking and utilization behaviors among OAAs. It consists of two studies. The first study used secondary data from the 2011-2012 California Health Interview Survey (CHIS) to explore health service utilization (HSU) patterns among OAAs. Bivariate and multivariate regression analyses were conducted to examine the significant factors associated with physician visits, emergency department (ED) visits, and hospitalization, focusing on psychological distress as the key independent variable. The study findings suggest that psychological distress was associated with the utilization of ED services among older Korean and Chinese Americans. Other significant factors associated with HSU included self-reported health, length of residence in the U.S., and English proficiency. ED encounters may therefore represent opportunities to identify older Korean and Chinese Americans with psychological distress and initiate appropriate treatment. The second study is a qualitative study that examined culturally-informed perceptions of dementia and barriers/facilitators in accessing/providing dementia-related services from the perspectives of older Korean and Chinese immigrants, their family caregivers, and home care providers. In-depth face-to-face interviewers were conducted with 12 older Asian immigrants and 12 family caregivers. Three semi-structured focus group sessions were conducted with home care professionals and ethnic home health aides. Data were analyzed using the conventional content analysis approach. Six themes emerged: perceptions and attributions of dementia, delayed help-seeking, perceived barriers in accessing care, caregiving burdens, barriers in providing care, and service needs. The findings underscore the importance of culture in shaping perceptions of dementia and related help-seeking behaviors. Culturally and linguistically appropriate training and educational programs should be developed to enhance awareness and knowledge of dementia in the Asian community to facilitate timely help-seeking. Culturally and linguistically appropriate social services are imperative to address the emotional and social needs of older Asian immigrants and their family caregivers.
