The goal of this project was to provide undergraduate trainees with exposure to areas of breast cancer research that focus on the role of microenvironment in mammary gland biology and in the development of neoplasia. Trainees in this project benefited from working in a program that investigates the intersection of hormones, growth factors, and extracellular matrix (ECM) signaling and remodeling during mammary gland morphogenesis, differentiation, and carcinogenesis. The program was advertised through several undergraduate research forums on the UC Berkeley campus, and more than forty applications were received. From these, eight applicants were selected to represent a balance of interests and approaches, with broad levels of expertise ranging from laboratory novices to students with many years of laboratory experience. During the research portion of the program, undergraduate trainees had frequent interaction with mentors and with advanced postdoctoral fellows, and reports were presented in organized, biweekly meetings structured to reflect the organization of a research paper. At the first meeting, the students presented the introduction to their research project; at the second, the materials and methods; at the third, the results. For the final meeting of the program, the students presented their work in complete form, including conclusions and interpretations. While the success at obtaining experimental results within the allotted time of the research program varies, all the participants (both students and preceptors) agreed that the overall experience was successful.

New developments in the methodology of cell biology and the explosive growth of molecular biology had begun to impact upon our understanding of mammary gland growth and function. It seemed a propitious time for summarizing the current status of knowledge of the cell and molecular biology of mammary cancer and for attempting to outline future areas of concern and interest. The reviews presented here were completed by the Fall of 1986. Although new insights will surely continue to emerge, it is hoped that the material in this volume will form not only a current update but a basic core of information for future experiments. We have not attempted to cover all areas of mammary gland transformation. Those areas where recent detailed reviews are already available have been omitted. Also, the areas of normal gland development, cell ultrastructure, hormone responsiveness, chemotherapy and clinical aspects of mammary cancer have not been included. Instead, we have selected those areas where the development of new methodology, reagents and results have led to new ideas about mammary gland function and development as they are related to neoplasia.

The response to this volume was superb, indicating a tremendous interest in basic growth regulatory mechanisms governing breast cancer and controlling its malignant progres sion. In the two years since its publication, much new and exciting in formation has been published and the full interplay of regulatory mechanisms is now beginning to emerge. We have divided this book into four sections that we hope will unify important concepts and help to crystallize areas of consensus and/or disagreement among a diverse group of basic and clinical scientists working on the disease. The first section is devoted to studies on oncogenes, antioncogenes, proliferation, and tumor prognosis. The first chapter, by Sunderland and McGuire, introduces the characteristics of breast cancer as studied by patho logists to establish prognostic outcome. Of particular interest is a new proto oncogene called HER-2 (or neu), which is rapidly becoming accepted as a valuable new tumor marker of poor prognosis. The second chapter, by Lee Bookstein and Lee, introduces the best known antioncogene, the retinoblas toma antioncogene, whose expression is sometimes lost in breast cancer. Malignant progression appears to be influenced by the balance of proto oncogene and antioncogene expression.

The authors are based on the epidemiological evidence indicating that early first full term pregnancy is a protective factor in human against breast cancer and they have used this paradigm and developed experimental systems in both in vivo and in vitro that have demonstrated mechanistically how the differentiation at the organ and cellular level takes place. This knowledge has provided the blueprint for developing better understanding of the basis of cancer prevention. The transcriptoma analysis of the breast of pre and post-menopausal women has established a genomic signature imprinted in the breast that differs according to the reproductive history of the woman showing that early first full term pregnancy reprogram the organ. This reprogramming takes place at the chromatin level by changing the transcriptional process. The modification of the transcriptional control is due to the expression of non coding RNA sequences and posttranscriptional control driven by the splicesome. The plasticity of the genome of the human breast make possible this reprogramming that is not only induced by the physiological process of pregnancy but by the use of hormones that mimic pregnancy without pregnancy. The author have established the basis of clinical trials for prevention and the discovery that short 15aa peptides of the chorionic gonadotropin hormone can be used in human breast cancer prevention based on preclinical and clinical data.

There are two main reasons why such an initiative was considered necessary. Firstly, the teaching of oncology requires a rigorously multidisciplinary approach which is difficult for the Univer sities to put into practice since their system is mainly disciplinary orientated. Secondly, the rate of technological development that impinges on the diagnosis and treatment of cancer has been so rapid that it is not an easy task for medical faculties to adapt their curricula flexibly. With its residential courses for organ pathologies and the seminars on new techniques (laser, monoclonal antibodies, imaging techniques etc.) or on the principal therapeutic controversies (conservative or mutilating surgery, primary or adjuvant chemotherapy, radiotherapy alone or integrated), it is the ambition of the European School of Oncology to fill a cultural and scientific gap and, thereby, create a bridge between the University and Industry and between these two and daily medical practice. One of the more recent initiatives of ESO has been the institution of permanent study groups, also called task forces, where a limited number of leading experts are invited to meet once a year with the aim of defining the state of the art and possibly reaching a consensus on future developments in specific fields of oncology.
