The Patient Protection and Affordable Care Act (PPACA) offered ways for states to increase insurance levels including by expanding their Medicaid programs. In anticipation of the expected decline in the uninsured and uncompensated hospital costs, PPACA also reduced federal support for hospitals serving a disproportionate share of low-income and uninsured individuals and redirected some support to Medicare UC payments for hospital uncompensated care costs. GAO was asked to examine federal support for hospital uncompensated care. GAO examined (1) the key sources and amounts of federal support for hospital uncompensated care costs; (2) the basis for determining hospital uncompensated care payments made under Medicaid and Medicare; and (3) the extent to which Medicare UC payments align with hospital uncompensated care costs. GAO analyzed federal payment data for fiscal years 2013 and 2014, the most recent available, reviewed relevant laws and regulations, and interviewed CMS officials. "

Yet existing measures of uncompensated care are flawed because they compare uninsured payments to list prices, not to the prices actually paid by the insured. We address this issue using a novel source of data from a vendor that processes financial data for almost 4000 physicians. We measure uncompensated care as the net amount that physicians lose by lower payments from the uninsured than from the insured. Our best estimate is that physicians provide negative uncompensated care to the uninsured, earning more on uninsured patients than on insured patients with comparable treatments. Even our most conservative estimates suggest that uncompensated care amounts to only 0.8% of revenues, or at most $3.2 billion nationally. These results highlight the important distinction between charges and payments, and point to the need for a re-definition of uncompensated care in the health sector going forward.

Book excerpt: Abstract: Hospitals provide significantly more uncompensated care compared to other providers due to their history, mission statements, and the higher cost associated with inpatient services. The purpose of this study was to examine the relationship between payer source, insurance type, and hospital care type in the provision of uncompensated care. A sample of 342 Californian hospitals, using 2010 Office of Statewide Health Planning and Development data, was tested using Pearson correlation coefficient and an independent sample t test. It was found that hospitals with more Medicare and Medicaid program patients had higher rates of uncompensated care. Hospitals with more managed care insurance patients and specialty hospitals had lower rates of uncompensated care. As hospitals seek solutions to this challenging financial situation, community benefit and financial viability has to be balanced. With these findings, hospitals can potentially make financially responsible business decisions related to payment, negotiated contracts, and types of services offered.

Book excerpt: This report summarizes research on the effect of the major health insurance coverage expansion under the Affordable Care Act (ACA) on the drivers of uncompensated care (UCC) and on hospital UCC costs. Hospital UCC costs totaled between $46 and $51 billion in 2012, according to the American Hospital Association (AHA) as well as data provided to the Center for Medicare and Medicaid Services (CMS) via Hospital Cost Reports. The Urban Institute reports that each individual uninsured for the full year of 2013 received $1,005 in implicitly subsidized uncompensated care, and that these costs totaled $49 billion nationwide in 2013. Though specific definitions differ, UCC is generally considered to be the unreimbursed cost of the care provided by hospitals to people who are uninsured, underinsured, or in some cases publicly insured. For the purposes of this report, we define hospital UCC costs as the combined total of bad debt and charity care. Bad debt refers to an amount hospitals anticipated receiving for services but in fact never received. Charity care is the value of services rendered for which hospitals never anticipated receiving payment, because the patient's inability to pay was determined early in the course of care; both of these sources of UCC are in large part generated by medically indigent or uninsured patients, though unrealized co-pays or deductibles for insured patients are also included in this calculation.
