Book excerpt: "This book in three sections uses formally written poems--rhymed quatrains, sonnets, terza rima, blank verse--to link the relationship between the poet and his mother with the wider world derived from a rare definition of umbilical: "descended through the female line." "Motherlands" deals with the connection of physical and mental landscapes. "Mother Tongues" focuses on language, spoken and unspoken. And "Blood Mothers" concentrates on flesh-and-blood mothers, whose support and inspiration help to shape our first thoughts. The poems move among scenes as varied as World War II, divorce, the Vietnam War, 9/11, and the imagined end of the world. The book concludes with a crown of sonnets about the poet's mother and his childhood in the South, showing in its way how all human life begins as female. Umbilical is the fifteenth winner of the annual New Criterion Poetry Prize. The New Criterion is recognized as one of the foremost contemporary venues for poetry that pays close attention to form. Building upon its commitment to serious poetry, The New Criterion established this annual prize in 2000"--Provided by publisher.

Book excerpt: This comprehensive volume discusses the current scope of umbilical cord blood transplantation (UCBT), including recent controversies and future developments for improving clinical outcomes. Its twenty chapters introduce new applications in regenerative medicine and discuss the latest scientific, regulatory, clinical and investigational aspects of cord blood banking. Physicians from around the world provide a global collaboration which explores strategies for umbilical cord blood expansion, homing, unit selection, and combining of graft sources to improve patient outcomes. Umbilical Cord Blood Banking and Transplantation also reviews advances in pediatric UCBT for hematologic and non-hematologic disorders as well as immune recovery, which is critical to preventing infection. Finally, it compares UCBT with other graft sources in an attempt to understand the optimal graft source for the individual patient. UCBT is an important option for many patients who need a transplant but do not have a family donor or a matched unrelated donor. The collective and timely knowledge presented here is essential reading for any regenerative medicine investigator, cord blood banker, transplant laboratory scientist or clinical physician interested in improving and expanding the applications of umbilical cord blood.

Available in PDF, EPUB and Kindle. Book excerpt: With an estimated 8,000 deaths per year in the United States from complications of UCA, an initial goal of 50% reduction of loss is possible. To achieve this goal requires the recognition by the obstetrical community of the issue. Recent research into circadian rhythms may help explain why UCA stillbirth is an event between 2:00 a.m. and 4:00 a.m. Melatonin has been described as stimulating uterine contractions through the M2 receptor. Melatonin secretion from the pineal gland begins around 10:00 p.m. and peaks to 60 pg at 3:00 a.m. Serum levels decline to below 10 pg by 6:00 a.m. Uterine stimulation intensifies during maternal sleep, which can be overwhelming to a compromised fetus, especially one experiencing intermittent umbilical cord compression due to UCA. It is now time for the focus to be on screening for UCA, managing UCA prenatally, and delivery of the baby in distress defined by the American College of Obstetricians and Gynecologists as a heart rate of 90 beats per minute for 1 minute on a recorded nonstress test. The ability of ultrasound and magnetic resonance imaging (MRI) to visualize UCA is well documented. The 18–20 week ultrasound review should include the umbilical cord, its characteristics, and description of its placental and fetal attachment. The American Association of Ultrasound Technologists has defined these parameters for umbilical cord abnormalities: B.1.4• Abnormal insertion B.1.5• Vasa previa B.1.6• Abnormal composition B.1.7• Cysts, hematomas, and masses B.1.8• Umbilical cord thrombosis B.1.9• Coiling, collapse, knotting, and prolapse B.1.10• Umbilical cord evaluation with sonography includes the appearance, composition, location, and size of the cord Cord Events: Although many stillbirths are attributed to a cord accident, this diagnosis should be made with caution. Cord abnormalities, including a Nuchal Cord, are found in approximately 30% of normal births and may be an incidental finding. (American College of Obstetrics and Gynecology Practice Bulletin 2009) According to NICHD’s recent stillbirth study, UCA is a significant cause of mortality (10%). This finding is in agreement with other international UCA studies. (Bukowski et al. 2011) These histologic criteria identify cases of cord accident as a cause of stillbirth with very high specificity. (Dilated fetal vessels, thrombosis in fetal vessels, avascular placental villi.) (Pediatr Dev Pathol 2012) Finally, defining the morbidity (injury) of cord compression, such as fetal neurologic injury or heart injury identified with umbilical cord blood troponin T levels or pulmonary injury, is the next major area of investigation.

Book excerpt: Umbilical cord blood (UCB) and, more recently, umbilical cord tissue (UCT) have been stored cryopreserved in private and public cord blood and tissue banks worldwide, since the umbilical cord blood was used for the first time in a child with Fanconi anemia with his HLA-identical sibling, following strict guidelines that imply high-quality standards and total rastreability of these units. The hematopoietic stem cells (HSCs) are clinically used in hematopoietic treatments for blood disorders and hemato-oncological diseases. Also, the mesenchymal stem cells (MSCs) isolated from the UCT and UCB, nowadays, can be used as coadjuvants of hematopoietic transplants. In the near future, these stem cells will have a crucial role in regenerative medicine. For this reason, these cells have been tested in several clinical trials and compassive treatments in children and adults, concerning a wide range of pathologies and diseases, for instance, for the treatment of cerebral paralysis. Considering the worldwide availability of UCB and UCT units and the absence of ethical concerns will probably become the best sources for cell-based therapies for hematological and nonhematological pathologies. The UCB will also have a crucial role in neonatology-predictive analysis in the near future.

Available in PDF, EPUB and Kindle. Book excerpt: Umbilical cord blood, previously discarded after birth, has emerged over recent years as an alternative source of hematopoietic stem cells for hematological reconstitution, mainly for leukemia patients, as well as for some hematological deficiencies and bone marrow failures. In recent years, it has become increasingly clear that cord blood, as well as the surrounding tissue of the umbilical cord, contain additional stem cells which have been shown to be of great potential for regenerative medicine. Importantly, cord blood is abundant, it can be banked and shipped with ease, and thus has an indisputable potential for future medicines and regenerative therapies. Driven by a massive interest for regenerative medicine and alternative yet ethically acceptable stem cell sources, the scientific literature on umbilical cord and cord blood stem cells has increased tremendously. This book provides a consolidated, up-to-date overview of basic research on hematopoietic and mesenchymal stem cells contained within umbilical cord tissue, as well as other more recently described stem and precursor cells of not yet fully elucidated potential. It also takes an in-depth look at basic and translational research efforts with stem cells from the umbilical cord in academic institutions and biotech companies. Suitable for use as a primer and reference book by medical fellows and researchers entering the research fields of stem cell biology and regenerative medicine, it can also be used by students (undergraduate and graduate) as a starting point for read-up on the literature on stem cells and their potential and applications, or as a teaching tool in graduate schools for biologists, particularly for students wanting to enter the emerging field of stem cell biology.

Available in PDF, EPUB and Kindle. Book excerpt: Since the therapeutic value of umbilical cord blood (UCB) stem cells was first recognised in the late 1980s, there has been a proliferation of both public and private UCB banks worldwide. However, the ability to utilise such a potentially valuable resource has provoked a number of controversies. In a distinctly accessible style, this book unpacks the socio-legal implications of the UCB collection process and constructs a detailed analysis of the law and ethics that surrounds UCB banking in the UK, including ownership of the cells. Its enquiry is located within the theoretical framework of altruism versus self-interest and explores the notions of risk and choice associated with this distinctive blend of public/private healthcare provision. The book evaluates the impact of the Human Tissue Act 2004 and the European Union Tissues and Cells Directive (2004/23/EC) on the UCB industry and provides a unique insight into the effect that the law may have on the NHS whose maternity staff and premises are used to collect UCB. This book would be of interest primarily to a UK readership in addition to expectant families, health professionals, students, academics, practitioners and the UCB industry elsewhere in the world.
