Book excerpt: Regenerative medicine is an interdisciplinary field which has advanced with the use of biotechnologies related to biomaterials, growth factors, and stem cells to replace or restore damaged cells, tissues, and organs. Among various therapeutic approaches, cell-based therapy is most challenging and exciting for both scientists and clinicians pursuing regenerative medicine. Specifically, stem cells, including mesenchymal stem cells and adipose-derived stem cells, are promising candidate cell types for cell-based therapy because they can differentiate into multiple cell types for tissue regeneration and stimulate other cells through neovascularization or paracrine signaling. Also, for effective treatment using stem cells, the tissue engineered constructs, such as bioactive degradable scaffolds, that provide the physical and chemical cues to guide their differentiation are incorporated with stem cells before implantation. Also, it was previously demonstrated that tissue-engineered matrices can promote tubulogenesis and differentiation of stem cells to vascular cell phenotypes. Hence, during tissue regeneration after stem cell therapy, there are numerous factors that need to be monitored. As a result, imaging-based stem cell tracking is essential to evaluate the distribution of stem cells as well as to monitor proliferation, differentiation, and interaction with the microenvironment. Therefore, there is a need for a stem cell imaging technique that is not only noninvasive, sensitive, and easy to operate, but also capable of quantitatively assessing stem cell behaviors in the long term with high spatial resolution. Therefore, the overall goal of this research is to demonstrate a novel imaging method capable of continuous in vitro assessment of stem cells as prepared with tissue engineered constructs and noninvasive longitudinal in vivo monitoring of stem cell behaviors and tissue regeneration after stem cell implantation. In order to accomplish this, gold nanoparticles are demonstrated as photoacoustic imaging contrasts to label stem cells. In addition, ultrasound and photoacoustic imaging was utilized to monitor stem cells and neovascularization in the injured rat tissue. Therefore, using these methods, tissue regeneration can be promoted and noninvasively monitored, resulting in a better understanding of the tissue repair mechanisms following tissue injury.

However, the process of vascularization from progenitor cells contributing vascular growth is not well understood. Hence, an imaging modality, which is easily applicable, reliable, and capable of quantitative assessment of vascular growth or regression in a three-dimensional environment, is required. In this study, the combined ultrasound and photoacoustic imaging method was applied to assess microvasculature in tissue engineered constructs. Hydrogels that spontaneously promote tube formation from implanted mesenchymal stem cells (MSCs) were imaged. PEGylated fibrin gels, supporting the development of capillary growth were implanted in a Lewis rat. After one week, the rat was euthanized and the gel implants were excised and positioned in water cuvettes for imaging. Ultrasound and photoacoustic images were acquired with single element 25 MHz and 48 MHz focused ultrasound transducers interfaced with a nanosecond pulsed laser source operating at 532 nm wavelength. The 3D ultrasound and photoacoustic image was reconstructed based on hundreds of 2D ultrasound images obtained by mechanically scanning the transducer. The ultrasound and photoacoustic images had good agreement with the vascular structure in the gel samples indicating that the combined ultrasound and photoacoustic imaging is capable of assessing growth of micro blood vessels.

Book excerpt: Photoacoustic imaging is an emerging imaging modality whereby pulsed laser illumination generates pressure transients that are detectable using conventional ultrasound. Plasmonic nanoparticles such as gold nanorods and spheres are commonly employed exogenous contrast agents for both sensing and therapeutic applications. This dissertation first presents a detailed review of engineering plasmonic nanoparticles for enhanced photoacoustic imaging followed by the design of iodide and chalcogenide doped silver-coated gold nanorods for oxidative stress sensing and enhanced photoacoustics. Clinically, photoacoustics leverages hemoglobin and melanin as endogenous contrast agents for breast, vascular, and wound imaging. The rest of this dissertation focuses on the use of ultrasound and photoacoustic imaging to monitor and predict chronic wound healing in human subjects. Chronic wounds are a major health problem that cause the medical infrastructure billions of dollars every year. Chronic wounds are often difficult to heal and cause significant discomfort. Although wound specialists have numerous therapeutic modalities at their disposal, tools that could 3D-map wound bed physiology and guide therapy do not exist. Visual cues are the current standard but are limited to surface assessment; clinicians rely on experience to predict response to therapy. Photoacoustic imaging can solve these major limitations. Chronic wound patients often receive skin grafts to promote tissue regeneration, but grafting makes it impossible to monitor the underlying wound bed. Ultrasound imaging can be used to size wounds in three dimensions, monitor, and predict tissue loss or regeneration under a skin graft. The addition of photoacoustic imaging allows functional imaging of angiogenesis and perfusion into the wound bed, both essential for wound healing. An increase in photoacoustic signal corresponds to higher perfusion and the rate of photoacoustic signal increase can predict if a wound is likely to heal or not. Clinical photoacoustic imaging can be limited by melanin, a major optical absorber giving skin its characteristic color. Subjects with darker skin tones and hence high melanin content absorb more photons leaving less light to probe deeper tissues of interest. The bias against darker skin tones subjects can be easily minimized by designing tools that account for variable melanin concentrations.
