Book excerpt: Optical coherence tomography (OCT) is a non-contact, non-invasive, micron-scale optical imaging technology that has become a standard clinical tool in ophthalmology. Fourier domain OCT detection methods have enabled higher sensitivity and imaging speeds compared to previous generation timedomain OCT. Spectral / Fourier domain OCT (SD-OCT) detects the interference spectrum using a broadband light source and spectrometer. Swept-source / Fourier domain OCT (SS-OCT) detects the interference spectrum over time using a wavelength-swept laser. Current standard commercial ophthalmic clinical systems based on SD-OCT technology have imaging speeds of 20,000 - 40,000 axial scans per second and axial resolutions of 5 - 7 ptm. In this thesis, ultrahigh speed OCT for ophthalmic imaging applications are presented. SD-OCT systems using high speed CMOS camera technology can achieve imaging speeds over 70,000 axial scans per second. Axial resolutions better than 3 ptm can be achieved with multiplexed broadband superluminescent diodes. A novel registration motion-correction algorithm for volumetric OCT datasets reducing motion artifacts and improving signal quality is investigated. Ultrahigh speed, ultrahigh resolution SD-OCT ophthalmic imaging applications including small animal retinal imaging and clinical imaging of age-related macular degeneration (AMD) are illustrated. SS-OCT systems using short cavity wavelength-swept laser light sources can achieve imaging speeds over 100,000 axial scans per second with 6 pm axial resolution for small animal and clinical imaging. The high sensitivity of SS-OCT enables enhanced vitreous imaging, visualizing features in the vitreous and vitreoretinal interface. Finally, a new vertical cavity surface-emitting (VCSEL) technology based wavelength-swept laser light source SS-OCT system with tunable speed and wavelength range as well as long coherence length enabling ultrahigh speed and ultralong range OCT imaging applications is demonstrated. In addition to comprehensive structural imaging, the emergence of functional OCT imaging of retinal blood flow using ultrahigh speed OCT may also improve the understanding of ocular disease pathogenesis. Therefore, ultrahigh speed OCT is a promising tool for the diagnosis and management of diseases in ophthalmology.

Ophthalmologists now use commercial OCT systems as a standard diagnostic instrument for imaging the retina to detect or monitor pathologies. However, prototype OCT research instruments exceed commercial systems in terms of faster imaging speeds and higher resolutions. Finding applications for these improvements will improve clinical utility for future OCT systems. This thesis describes the design and use of an ultrahigh resolution spectral domain OCT system for detecting the photoreceptor changes during flash stimulus and an ultrahigh resolution swept source OCT system for use in eye surgery. The ultrahigh axial resolution of our system enabled visualization of thickness changes in the outer retinal layers after flash stimulus and subsequent dark adaptation. This finding could be used as a marker for photoreceptor health in retinal diseases that influence dark adaptation such as age-related macular degeneration. In the operating room, the ultrahigh speed system attaches to the operating microscope to share the surgeon's view and provide depth-resolved information that is not possible with the standard surgeon's stereoscopic view. This allows for imaging during surgical procedures and the ultrahigh speed enables acquisition of dense, widefield data sets as well as rapid volume acquisition to generate 3D visualizations in time. These data sets will enable 3D planning of procedures, assessment of outcomes before leaving the operating room, and feedback for surgical procedures.

Although current commercial Fourier domain OCT systems have high imaging speeds of 20-100kHz A-scan rates, these imaging speeds are not sufficient for more advanced structural and functional imaging techniques. Current state-of-the-art spectral domain and swept source OCT provide ultrahigh imaging speeds of >200kHz A-scan rates. These speeds enable functional imaging of retinal blood flow, OCT angiography of the retinal and choroidal microvasculature, and wide field volumetric structural imaging of the retina and choroid. In this thesis, advances in structural and functional ophthalmic imaging techniques for the human and small animal eyes are investigated using ultrahigh speed Fourier domain OCT. The following topics are discussed: (1) a method for numerically extracting and compensating dispersion mismatch in ultrahigh resolution spectral domain OCT, (2) ultrahigh speed spectral domain imaging in the small animal eye for measuring total retinal blood flow, (3) development of ultrahigh speed phase stable swept source OCT system for human retinal imaging, (4) OCT angiography of the choriocapillaris in the human eye, (5) clinical applications of OCT angiography in retinal diseases, including diabetic retinopathy and age-related macular degeneration, (6) small animal anesthesia protocol for functional hemodynamic imaging, and (7) imaging of neurovascular coupling in small animals using ultrahigh speed OCT.

In a previous bachelor's thesis prior to this project, we described a highly optimized Graphics Processing Unit (GPU) implementation of the Fourier Domain (FD)OCT processing and visualization pipeline that was capable of real-time volume rendering at video rate.In this thesis, we describe three applications that build upon the GPU-based processing software: Speckle Variance (sv)OCT, Compressive Sample (CS)OCT, and Wavefront Sensorless Adaptive Optics (WSAO)OCT. We first demonstrate that svOCT can be a powerful fundus imaging technique for visualizing the retinal vasculature network, which may be comparable to the gold-standard technique called Fluorescein Angiography (FA). The strongest attribute of svOCT is that it bypasses the use of intravenous fluorescein for vascular contrast enhancement, and can be highly suitable for longitudinal monitoring of patients with vasculature-related pathologies in the retina. In our second application, we present a GPU-accelerated CS-OCT processing pipeline. The principle behind CS-OCT is to take advantage of the redundancy in biological structures, such as the retina, for reconstructing volumes acquired at a sampling density below the Nyquist criterion; the purpose is to justify decreasing volume acquisition time by significantly subsampling the datasets. In our final application, we present a WSAO-OCT system as a novel technique for cellular resolution imaging of the human photoreceptor layer. This technique leverages on the ultrahigh speed processing rates in our GPU-based processing software in order to produce a real-time intensity-based merit function for en face image quality optimization.
