Book excerpt: Bien que l'HAS (Haute Autorité de Santé) ait défini les indications d'extraction des troisièmes molaires pour raisons pathologiques, leurs avulsions prophylactiques ont été réalisées sans aucune recommandation officielle durant de nombreuses années. L'encombrement antérieur secondaire et les accidents infectieux à répétition étant mise en avant malgré l'absence de preuve scientifique. L'objectif de cette revue de littérature est de réaliser une synthèse des différentes méthodes pronostiques de l'évolution des troisièmes molaires maxillaires et mandibulaires, à travers les examens cliniques et radiologiques établissant le dossier orthodontique des patients. Les résultats montrent que les facteurs influençant le plus l'éruption des troisièmes molaires sont : l'espace rétro molaire, la croissance squelettiques, les dysmorphoses en présence ainsi que l'angulation, la taille, la forme et la maturité du germe. L'étude pronostique doit donc porter sur l'analyse de l'espace disponible et de l'espace nécessaire. Par conséquent, cette étude nous montre la nécessité de réaliser une évaluation des pratiques professionnelles actuelles, et propose donc un questionnaire s'adressant aux omnipraticiens. La décision d'avulsion des troisièmes molaires doit être globale et prendre en compte leur pronostic d'évolution sur l'arcade ainsi que l'objectif du traitement orthodontique, ceci afin de préserver le capital dentaire du patient et ainsi d'éviter l'avulsion de huit dents définitives. La surveillance radiologique et clinique par évaluation périodique individuelle sera la marche à suivre en cas de doute pronostic et d'absence de symptomatologie.
