Many valuable steps have been taken in the first year of this grant, including the development of a pilot questionnaire, administration of the pilot questionnaire, development of the protocol for the study and development of patient, spouse and physician questionnaires. Enrollment for the study began in November, 2000.

Book excerpt: This study evaluated the efficacy of a patient-spouse centered cognitive-affective counseling session (CARE: cognitive-affective reactions and expectations), to facilitate treatment decision-making for localized prostate cancer (PrCa). Methods: CARE identified treatment-related values and goals and focused on improving knowledge about treatment options. A time and attention matched General Health Information (GHI) session provided nutrition information. Couples (N=300) were enrolled after diagnosis with PrCa, but before a definite treatment decision was made. Data were assessed separately for patients and spouses at baseline, at 6-months, and at 12 months post intervention. Results: Both sessions were well accepted among participants. The goal of CARE to facilitate treatment decision making was best achieved for spouses and among those patient/partners who chose a non-invasive treatment option. Partners were more distressed about the treatment decision compared to patients, especially in the CARE condition and when considering invasive procedures. We interpret this result as an indication that participants in CARE processed the relevant information and that momentarily higher levels of distress were the cost of such processing. At 6-mo this effect disappears, underscoring its temporal nature. Conclusions: The results demonstrate the usefulness of integrating a brief counseling session into the decision-making process, and that increases in perceived difficulty of decision-making are short-term.

Book excerpt: This study involves patients and their spouses/partners who are attending two cancer centers: the Deane Prostate Health and Research Center in the Department of Urology at Mount Sinai Hospital New York City and the Department of Medical Oncology at Fox Chase Cancer Center Philadelphia. Patients will have been diagnosed with rising PSA but have no clinical evidence of cancer. Eligible patients and their spouse/partners who have agreed to participate (n = 191) will take part in a 12-months long assessment study. During this time they will be interviewed via questionnaires four times: at the beginning (baseline) at 6-months at 12-months and when they have made a treatment decision. Our research focus and the measures used for data collection are guided by our cognitive-social health information processing (C-SHIP) theoretical framework. This framework incorporates individuals' experiences expectations beliefs values and emotional responses to a health threat and sees these components as influential factors in treatment decision making. Both the patient and the spouse/partner will be assessed. Due to the limited patient enrollment no significant findings can be reported at this time.

An estimated 2.3 million prostate cancer survivors reside in the United States, 50,000–70,000 of which are gay men. Fearing discrimination, stigma, or receipt of substandard care, gay men may not reveal their sexual orientation potentially affecting provider communication, treatment decision-making and post-treatment quality of life (QoL). Sexual orientation and other social factors may play an important role in men's QoL. Research suggests that functional impairments following treatment diminish QoL more for younger (i.e., age 50–64 years) than older men, while supportive partners can positively influence how men adapt to prostate cancer. Yet data are currently limited to heterosexual populations; it is unknown whether these effects are similar among gay prostate cancer survivors. Aims. To revise recent measures of prostate cancer QoL to include the experiences of gay men, assess the role of age and partnership status among gay men and compare QoL between gay and straight men. Methods. We administered a national QoL survey to gay prostate cancer survivors, including questions about sexual orientation disclosure and community rejection derived from qualitative interviews. Survey data were analyzed to assess the role of age and partnership status among gay men for four post-treatment prostate cancer QoL outcomes (masculine self-esteem, health worry, informed treatment decision and treatment regret) and QoL was then compared to straight prostate cancer survivors using data from a previous study to assess for differences based on sexual orientation. Results. Younger gay men experienced poorer QoL outcomes following treatment than older men; no association with partnership status was found. Comfort in revealing one's sexual orientation to a provider may result in better outcomes. Gay men reported more stigma, lower masculine self-esteem and more treatment regret following prostate cancer treatment than straight men. Conclusions. For gay men, greater sensitivity to stigma as a sexual minority may result in poorer QoL compared to straight men. While both gay and straight men struggle with similar QoL issues following localized prostate cancer treatment, gay men may have more difficulty due to sexual orientation. Further research is needed to better understand the role that sexual orientation and stigma play in prostate cancer QoL.

Book excerpt: Abstract: The aim of this thesis is to describe how men with localised prostate cancer make decisions about their medical treatments, to describe the psychological and decision-related adjustment of these men over time, and to identify what variables predict decision-related adjustment. Chapter 1 reviews the medical context of localised prostate cancer and factors that influence men's decision making in this context. It is concluded that owing to ongoing uncertainty about the optimal medical treatment for this cancer and the substantial negative quality of life effects of treatments, how men make decisions in this context is an important research question. Further, although men with prostate cancer are high seekers of medical treatment information, knowledge about how men use such information and actually make this treatment decision is limited. -- Chapter 2 discusses research approaches currently applied to patient decision making: first, a social interaction approach encompassing the interaction between the patient and their physician and the social context influencing this interaction (Charles, Gafni, & Whelan, 1999); and second, normative decision theory (Shafir & Tversky, 1992; von Neumann & Morgenstern, 1947). The Heuristic-Systematic Processing model (Chaiken, 1980) is then proposed as a theoretical framework for investigating patient decision making that includes both systematic and non-systematic decision strategies. -- Chapter 3 reviews applied decision research in cancer, and presents an overview of research findings regarding patients' preferences for involvement in decision making, the relationship between decisional involvement and psychological adjustment, and decisional support interventions. Research on adjustment to cancer is discussed and the need for further research about men's psychological and decision-related adjustment after localised prostate cancer is identified. Finally, a multivariate analysis of decision-related adjustment for men with localised prostate cancer based on the stress and coping framework of Lazarus and Folkman (1984) is proposed. -- Chapter 4 describes Study 1 that was an experiment to investigate the utility of the Heuristic-Systematic Processing model (HSM) in explaining low desire for involvement in decision making about prostate cancer treatments as an example of use of the expert opinion heuristic. Using a hypothetical decision scenario about localised prostate cancer it was found that a low desire for involvement in decision making by men was predicted by a high belief in powerful others controlling health, a low belief in the self being responsible for good health, a high preference for black and white thinking, and a lower level of education. This study provides preliminary support for use of the HSM in this context and for the conceptualisation of decision deferral as the expert opinion heuristic. -- Chapter 5 introduces and describes the method of Study 2 that was a descriptive, prospective study of men's decision making after an actual diagnosis of localised prostate cancer. This method allowed for an analysis of men's decision making that includes both systematic and non-systematic processes, and for further investigation of the utility of the HSM in explaining decision behaviour. In addition, a multivariate approach was used to describe men's physical, psychological and decision-related adjustment over time, and to identify psychological predictors of decision-related adjustment. -- Chapter 6 describes men's use of systematic processing as limited and the use of non-systematic processes, such as lay beliefs and heuristics, as pervasive. It is concluded that patients do not utilise information about medical treatments in a comprehensive or systematic way when making treatment decisions and that patients' decision making is biased by their prior beliefs about cancer and health. A framework is outlined to demonstrate how the results of Study 1 and 2 support the application of the HSM to decision making about prostate cancer with particular reference to the role of beliefs about the physician, health locus of control, and uncertainty about the treatment decision in influencing decision strategies. -- Chapter 7 describes men's physical, psychological and decision-related adjustment over time, and concludes that decision-related distress is high but psychological distress in general is low. Decisional conflict at diagnosis and at twelve months concurrently, and at two months prospectively, was predicted by dispositional optimism; and this effect was mediated by the man's proximal cognitive appraisal of the impact of the cancer. It is concluded that decisional conflict is a person's cognitive judgment of the treatment decision that is generated by similar processes to that of the psychological distress that follows a cancer diagnosis. Conclusions and implications of these studies for future research in this area are summarised in Chapter 8.

Book excerpt: This study aims to examine a method of patient education for men who are seeking treatment for management of localized prostate cancer. The primary goal is to evaluate a recently developed computer-based educational tool (CD-ROM) that is designed to provide treatment-related information and to assist men in making an informed treatment decision. Men are accrued pre-biopsy for a baseline interview, and those who are diagnosed with localized cancer are randomized to receive a CD-ROM with a decision aid, or a CD-ROM with information only. To date, we have accessioned 97 patients. Of the 97, 36 were ineligible and 15 are pending (either pending initial contact or biopsy result). Of the remaining 46, 28 either declined or were unreachable, 2 dropped out and 16 are active with a CD-ROM. This results in a 35% participation rate (16/46). Because of the number of men who are ineligible, and who were unreachable prior to biopsy, we are seeking to expand the study to invite those already diagnosed with prostate cancer to consider participation. To that end, we aim to contact the Georgetown University Radiation Department and Georgetown University Medical Center Referral Center for accrual expansion.
