Book excerpt: Few subjects in medicine are so intimately connected with the history and philosophy of the human mind as insanity. Derangement of the understanding is generally considered as an effect of an organic lesion of the brain, consequently as incurable; a supposition that is, in a great number of instances, contrary to anatomical fact. The present essay will commence with an historical exposition of periodical insanity. The leading principles of our moral treatment will then be developed. Attention to these principles along will, frequently, not only lay the foundation of, but complete a cure. This successful application of moral regimen exclusively gives great weight to the supposition that, in the majority of circumstances, there is no organic lesion of the brain. By autopsy, interviews, and observational means, I have been enabled to introduce a method into the services of this hospital [Asylum de Bicetre, France], and to class my patients in a great measure according to the varieties and inveteracy of their complaints. An account of our system of interior police will finish this part of the enquiry. The last section will comprehend the principles of our medical treatment. Book excerpt: Excerpt from A Treatise on the Medical Jurisprudence of Insanity Mental disease, or disease Of the brain. Powerfully affects, or may affect, the knowledge by which our actions are guided, the feelings by which our actions are prompted, the will by which our actions are performed, whether the word will is taken to mean volition or a settled judgment Of the reason acting as a standing con trol on such actions as relate to it. The means by which these effects are produced are unnatural feelings; delu sions or false opinions as to facts; hallucinations or deceptions Of the senses; impulses to particular acts or classes of acts; and in some cases, it is asserted, a specific physical inability to recognize the difference between moral good and evil as a motive for doing good and avoiding evil. Book excerpt: Excerpt from The Medical Jurisprudence of Insanity The last words of a book are those which meet the reader's eye first. As that is the case, a preface generally says what the book is meant to be, and in how far the writer's intentions have been realised in the work. Most prefaces are apologies. This work on mental defects and diseases in their legal relations is intended as a practical treatise more complete and systematic than the chapters devoted to this subject in medico-legal text-books, or than those monographs on limited portions of it which have from time to time appeared. Book excerpt: Excerpt from Insanity in Its Medico-Legal Relations Even in this enlightened day there are those who sneer at the idea of calling a physician to decide the question of insanity. A person in order to be insane, in their estima tion, must either be a raging maniac, or, at least, be labor ing under an outrageous delusion. They know nothing of the secret workings of the human mind, either in health or disease, and too often think the physician makes crime an evidence of insanity, Simply because they cannot appreciate the unseen evolutions of the mind. Book excerpt: Excerpt from The Question of Insanity: And Its Medico-Legal Relations Considered Upon General Principles I ask to be allowed to inscribe these pages to you, not because I believe them worthy of such high and distinguished consideration, but simply as a slight testimonial of respect for the talents and patriotism which induced you, "a gallant few," to step forward in the work of medical reform in North Carolina; of esteem for those steadfast virtues, courage and industry, which have caused you to press forward amid the greatest of discouragements, and almost of defeat in fact, "to perform the tasks of hope in the midst of despair;" and of acknowledgment for numerous instances of disinterested friendship, and for honors conferred, that were wholly unsolicited on my part. "The labors of the faithful seldom go unrequited; and the work of the wise endureth as a monument." Such are your labors; may your work be a monument of wisdom imperishable as the pyramids. Book excerpt: Excerpt from The Curability of Insanity: A Series of Studies It is now nearly ten years since my essay - "The Curability of Insanity," enlarged from the original in the annual report for the official year 1875-76 of the Northampton Lunatic Hospital - was read before the New England Pyschological Society, and published, by direction of that association, in pamphlet form. Meanwhile, seven other articles upon the same subject have been embodied in my annual reports to the trustees of the aforesaid institution. Those papers have been favorably received by a large number of readers, both at home and abroad. It is not presumptuous to claim that they have greatly modified the aspect of insanity, as a curable mental condition, in the view of a large proportion of the persons most interested in the subject. They have thus been an important agent in stimulating the minds of philanthropists to seek-and in several notable instances to adopt-other methods for the custody and care of a large part of the insane than that of collecting them in expensive and unwieldy curative institutions.
