Book excerpt: Impairments in mentalizing - the capacity to utilize mental state information to understand oneself and others - have consistently been identified across the developmental continuum of psychosis expression, from the premorbid and prodromal stages to its clinical forms. Mentalizing difficulties in psychosis have been investigated using an array of different methodologies, including novel experimental tasks, narrative assessments, self-report measures, as well as neuroscientific and computational methods. These studies have primarily examined how mentalizing disturbances relate to symptom dimensions and functional outcomes in clinical samples, as well as the transition to clinical psychosis among those who are at increased risk. More recently, clinical adaptations of mentalization-based treatments (MBT) and other psychotherapeutic approaches with a focus on supporting people suffering with psychosis reflect on their own and others’ mental states, such as Metacognitive Reflection and Insight Therapy (MERIT), have been reported in the literature.

Pretende ser un instrumento de trabajo riguroso que responde a la necesidad de facilitar la realización de planes individuales de cuidados que sirvan como guía, y en ella se recogen la mayoría de los diagnósticos enfermeros que pueden darse en personas afectadas por un trastorno psicótico. Todo ello fundamentado en los conocimientos de la disciplina enfermera, unido a la revisión bibliográfica realizada sobre los trastornos psicóticos y a la experiencia profesional en la atención de las personas afectadas por esta patología. El trabajo del enfermero especialista es un pilar fundamental en el abordaje de los trastornos psicóticos, ya que estos, por sus características (entre otras, la escasa conciencia de enfermedad), presentan dificultad para su adherencia al tratamiento, lo que consiguientemente empeora su pronóstico. Precisa un trabajo conjunto de psicoeducación, apoyo, coordinación, rehabilitación y de supervisión. Estas intervenciones son necesarias en su abordaje integral y deben ser realizadas por personal preparado para ello. La concepción y la definición de estos trastornos, siguiendo el sistema de clasificación nosológica de la DSM-IV y la actualización de su tratamiento, así como la revisión de aspectos tan importantes como el suicidio, la agresividad y la violencia y los temas legales y éticos, aportarán a los lectores de esta guía una base teórica actualizada y fundamentada sobre la que sustentar las actuaciones asistenciales. El otro punto fuerte de esta guía es el capítulo de los cuidados de enfermería en los trastornos psicóticos, ya que, basándose en diferentes modelos teóricos y tras realizar una valoración inicial, se detectan una serie de necesidades de cuidados que los pacientes tienen, y esto permite posteriormente establecer diagnósticos de enfermería que complementen los clásicos diagnósticos psiquiátricos. Esta estandarización de los cuidados enfermeros consigue unificar criterios y permite también realizar estudios de investigación y, por supuesto, facilitar la tan importante continuidad de cuidados. Guía de cuidados de enfermería que ofrece los diagnósticos de los trastornos psicóticos con las intervenciones y los criterios de resultados relacionados con ellas. El objetivo de esta obra es la formación y la capacitación de los profesionales de enfermería que pueden atender a personas con trastornos psicóticos.
