Available in PDF, EPUB and Kindle. Book excerpt: Developmental biology of normal bone and cartilage including histogenesis, molecular/gene and biomechanical aspects is updated and expanded. The book outlines the biology of: bone repair with differing mechanical environments; cartilage repair at articular and physeal sites; and distraction osteogenesis. The generously illustrated text provides an in-depth presentation of the interplay between normal developmental biology, abnormal pathologic states and the influence of operative and non-operative orthopedic interventions on childhood orthopedic deformity. Thirty-four principles underlying the development, progression and management of skeletal deformity in the growing child are defined. Orthopedic management including surgical treatment is discussed for: skeletal dysplasias; epiphyseal growth plate fracture-separations; lower extremity length discrepancies; and deformities of joints and epiphyses due to metabolic, inflammatory, infectious, hematologic, and neoplastic disorders. Treatments are related to extent of deformity, remodeling post-surgery and possible recurrence. This 2nd edition of Pediatric Orthopedic Deformities has been expanded to cover more regions and disorders and is being presented in 3 volumes.

Book excerpt: Bone: A Treatise is a series of seven volumes devoted to providing the most up-to-date, comprehensive, and authoritative coverage of all aspects of bone. Seventy-four chapters have been written by 127 respected authors, all of whom are actively engaged in basic, applied, and clinical bone research. The seven volumes in this unique series are organized thematically; each volume integrates structure, function, biochemistry, metabolism, and the molecular and clinical aspects of a particular aspect of the biology of bone. Bone-forming cells are treated in Volume I. Bone resorbing cells are covered in Volume II. Volumes III and IV examine the extracellular matrix of bone, with Volume III concentrating on the structure of bone matrix and on bone-specific proteins, while Volume IV focuses on the mineralization (calcification) of that matrix. Volume V addresses the repair and regeneration of bone. Volumes VI and VII explore bone growth and development.

Available in PDF, EPUB and Kindle. Book excerpt: Legg-Calvé-Perthes Disease (LCPD) remains a challenging hip condition to treat. More than a century after its identification, the etiology of LCPD remains unclear and treatment continues to be controversial. The main goal of treatment is to prevent femoral head deformation. Preservation of femoral head sphericity is known to minimize the risk of premature hip pain, stiffness and arthritis. The best ways to achieve these goals remain elusive. This book is replete with up-to-date discussions of history, epidemiology, radiology and classification systems for LCPD. It takes a well-balanced, evidence-based approach to various methods of treatment, from conventional containment femoral and pelvic surgery, to more cutting-edge procedures such as transfemoral head-neck tunneling, hip distraction, hip arthroscopy and intraarticular femoral head reshaping osteotomies, ending with a discussion of the latest biologic and pharmacologic treatment strategies. It is a well-illustrated tome for the various surgical techniques presented. The chapter authors, all experts in LCPD, share their practical, innovative and comprehensive approach to this challenging condition. Legg-Calvé-Perthes Disease will be the definitive reference work on this condition for many years to come.

Book excerpt: There are many textbooks on fractures in adults, but few deal with those in children and young persons. Publications on fractures in children are usually written by orthope dic surgeons who are familiar with the problems presented by the growing skeleton. The principles taught in the best known works on fractures in children by BLOUNT (1954), RETTIG (1957), CHIGOT and ESTEVE (1957), EHALT (1960), POLLEN (1973), and RANG (1974), differ from each other only slightly. The controversy which has sur rounded the treatment of fractures in adults in recent years has never extended to chi1drens' fractures. Skillful nonoperative treatment of the latter leads to uneventful healing in the majority of cases, which cannot always be said of similar injuries in adults. "Considerable skill" is required to induce nonunion in a child, and its occur rence always results from a serious error of management. In the adult, prolonged immobilization frequently leads to muscle atrophy, joint stiffness, and disturbances of the peripheral innervation and circulation. Fracture treatment by internal fixation in the manner originally described by LANE (1894), LAMBOTTE (1892, 1907, 1913), KONIG (1905, 1931), and DANIS (1932, 1949) has gained wide acceptance only in the last 15 years. Since 1958, MULLER, ALLGOWER, WIL LENEGGER, and the other members of the Swiss Association for the Study of Internal Fixation (ASIF) have been developing techniques and instruments for the operative treatment of fractures.

Book excerpt: This book provides the reader with the best available evidence on the most pressing issues relating to reconstruction of the anterior cruciate ligament (ACL) with the goal of supporting surgical reconstruction of the ACL and improving outcomes for patients. Key topics for which evidence-based information is presented include selection of graft material and source, the use of different surgical techniques, graft rupture in relation to surgical technique, and progression to osteoarthritis. The book will aid the surgeon in making decisions with respect to fixation devices and tensioning, the bundles to be reconstructed, and whether to preserve remnants or partial bundle ruptures. An evidence-based stance is taken on evolving topics such as the anatomy of the tibial insertion site of the ACL and the role of the anterolateral capsule and posteromedial corner in high-grade rotatory instability. Furthermore, novel technical developments for measurement of knee laxity and soft tissue navigation are discussed. The reader will also find useful information on general issues concerning physical examination, arthroscopic setup, timing of reconstruction, anesthesia, and anticoagulation.
