En effet, beaucoup d'articles cliniques relèvent le fait, que l'anesthésie locale serait plus difficile chez les patients toxicomanes. Cependant l'analyse de la littérature montre une réelle carence en études scientifiques qui prouverait une baisse d'efficacité de l'anesthésie locale chez ces patients. (2) En effet les récepteurs CB1, CB2 mais également Vanilloïdes de type 1 (TRPV1) sont naturellement présents chez l'homme et sont la cible des endocannabinoïdes produits par l'homme tel que l'Anandamide qui ont un rôle dans la régulation de la douleur. Ces récepteurs sont aussi les cibles des principales molécules présentes dans le cannabis, nommés les cannabinoïdes dont les deux principaux sont le Delta-9 TétraHydroCannabinol (THC) et le Cannabidiol (CBD). Ces cannabinoïdes activent via les canaux ioniques voltages-dépendants et les protéines G trans-membranaires des voies de transduction qui peuvent être responsables des signaux douloureux. Au sein du système nerveux pulpaire on retrouve ces récepteurs (CB1, CB2 et TRPV1), qui jouent un rôle dans la douleur dentaire. Sachant que le mode d'action de l'anesthésie locale dentaire se fait via les mêmes types de canaux ioniques voltages dépendants, on comprend qu'une interaction peut avoir lieu entre les deux. Les objectifs de cet article sont de faire le point des connaissances sur le sujet et de réaliser un protocole d'étude afin de comparer l'efficacité de l'anesthésie dentaire chez les consommateurs de cannabis selon le contexte de soin : en urgence et en suivi de soins.

Le chirurgien-dentiste devra décider s'il est préférable de réaliser des prothèses amovibles après des extractions multiples ou si la solution fixe est envisageable. Connaitre l'addiction du patient permet aussi d'être préparé aux éventuels risques des soins réalisés. Nous avons choisi d'étudier uniquement la toxicomanie psychoactive, à savoir héroïne, cocaïne et leurs substituts médicamenteux.
