Book excerpt: The inflammatory bowel diseases, of unknown etiology and for which there are no cures, continue to attract the attention and interest of gastroenterologists, internists and surgeons. International symposia are common and it is safe to say that there is at least one major symposium held somewhere in the world each year. This book encompasses the proceedings of two recent symposia held in Victoria, British Columbia, Canada. The symposia were the fifth and sixth international meetings focused on inflammatory bowel disease in Canada in the last eight years. Once again they were sponsored by Axcan Pharma, Inc. (formerly Interfalk Canada, Inc.) and endorsed by the Canadian Association of Gastroenterology. As has become traditional at such meetings the faculty was drawn from an international roster ofleaders in the field of inflammatory bowel disease and gastroenterology. The chapters of the proceedings provide a timely, up-to-date review of the major issues, including those within the realm of basic science and others dealing with clinical problems. The first symposium, 'Basic Research and Clinical Implications', was co-ordinated by John Wallace in association with Stephen Collins and Stephan Targan. The themes of this section were organized under the general topics of predisposing factors (genetics, animal models, infection, permeability, and immune deficits) and the pathophysiology of intestinal inflammation. The second symposium, Trends in Therapy' was organized by Lloyd Sutherland along with Franc;:ois Martin, Robin McLeod and Noel Williams.

Book excerpt: Now published in two volumes to accommodate new chapters on the many advances in understanding and treatment options, this volume represents the definitive reference on inflammatory bowel disease, a spectrum of diseases that is receiving increasing attention as our understanding of the etiological factors increases and diagnostic tools are refined. Basic research accelerated at the beginning of this decade and is now yielding new, more targeted treatments than were available just a few years ago. Volume 1 is on IBD and Ulcerative Colitis, and Volume 2 is on IBD and Crohn's Disease. All areas that were covered in the second edition have been expanded and updated. New sections include one addressing etiology and pathophysiology of IBD, including environmental influences and effect of age at onset, and one with a focus on aggravating factors in the IBD patient such as infectious agents, non-steroidal anti-inflammatory drugs, and pregnancy.

Book excerpt: Inflammatory bowel disease (IBD) comprises a group of idiopathic diseases of the intestine characterized by chronic inflammation of the bowel with periods of exacerbation and remission. Although the exact cause of IBD remains undetermined, the condition appears to be related to a combination of genetic and environmental factors resulting in an aberrant activation of the mucosal immune system. This book contains a series of interdisciplinary discussions between clinical and basic scientists focusing on key issues such as: Epithelial cell and molecular biology, including apoptosis, necrosis and cell survival The role of bacterial milieu and mucosal bacteria in the IBD and of prebiotic and probiotic therapy The progress towards the identification of susceptibility genes and phenotype-determining genes The pharmacogenetics of IBD Mucosal immunology and therapeutic strategies stemming therefrom

Book excerpt: As the number of patients with colitis-associated cancer (CAC) is on the increase, the purpose of this book is to review the latest topics concerning management of the disease. In recent years, the diagnostic power of endoscopy and molecular pathology has also grown tremendously, as a result of which they now have a far greater influence on the treatment of CAC. At the moment, appropriate monitoring programs for ulcerative colitis and Crohn’s disease remain uncertain. At the same time, the latest findings on DNA methylation and microRNAs hold the promise of making revolutionary changes in these areas. Moreover, recent drug advances in the treatment of inflammatory bowel diseases have changed surgical indications. On the other hand, the indication of mucosectomy on colorectal cancer in ulcerative colitis and prophylactic abdominoperineal resection for Crohn’s disease remain controversial. This book provides the latest information on the remaining issues of CAC from the point of view of expert surgeons.

Available in PDF, EPUB and Kindle. Book excerpt: One of the most vivid memories from my medical school training was seeing my first surgical operation on a patient with Crohn’s disease. The senior surgeon at Mount Sinai Hospital in New York City, the same institution at which Burrill Crohn, Leon Ginzburg, and Gordon Oppenheimer had first described the disease “terminal ileitis,” had - doubtedly done countless operations on patients with inflammatory bowel disease in the past. Yet as we both gazed down into the patient’s open abdomen, at the “creeping fat” that seemed to be wrapping its sticky fingers around the young man’s intestines, he stated, “this is the m- tery of Crohn’s disease—no two patients are ever the same. ” What is it about the inflammatory bowel diseases, Crohn’s disease, and ulcerative colitis, that we find so intriguing? Is it the young age of the patients, many who are younger than even the medical students - tending to them? Or is it the elusive etiology, the theory of a “mystery organism” that has yet to be identified? Perhaps it is the familial pattern of disease, where many patients have relatives with similar diseases, yet in some instances only one of a pair of identical twins is affected. Regardless of the cause, these chronic diseases with a typically early age of onset, result in a long-term commitment of the patient, their fa- lies, friends, health care providers, researchers, employers, and even health care insurers and other health-related industries.

Available in PDF, EPUB and Kindle. Book excerpt: As in the first edition of Inflammatory Bowel Diseases this new edition continues to provide readers with a concise, yet in-depth review of many of the important areas in the history, epidemiology, pathogenesis, diagnosis, and treatment of the inflammatory bowel diseases (ulcerative colitis and Crohn's disease), as well as associated issues: extraintestinal manifestations, ostomy care, women's issues, economics, etc. This Second Edition provides the reders with up-to-date, state-of-the art approaches to these disease states, with expansion into the newer topics that have emerged in the past few years which include the expansion of biological agents for treatment, new studies in the epidemiology of IBD, the changing economics of IBD, breakthroughs in new endoscopic evaluations, major advances in novel radiographic techniques, the detection of colorectal cancer in patients with IBD and new findings in the genetics of IBD. New Trend / Developments (* These all emerged since our last edition) The most critical trend that has arisen over the past few years has been the introduction and expansion of biological agents in the treatment of inflammatory diseases worldwide. While only one agent, infliximab (Remicade), existed at the time of publication of my first edition, there have been multiple other agents either approved or pending approval for the treatments of these diseases, or in the upper-stages of development. The impact is not only upon the patients, but also on the health-care providers, policy makers, researchers, biotech and pharmacompanies, and the business community. Major changes worldwide in the epidemiology of IBD has spurred new studies in the demographics of who is getting these diseases and why. Previously characterized as diseases primarily affected Caucasians in industrialized countries, there has been an explosion of growth amongst the African American and Hispanic populations in the United States, and well as in pockets of these and other ethnic groups worldwide. The economics of IBD has been changing greatly, given the issues raised above, as well as major changes in the United States Medicare program, and other insurance changes. The growing proportion of the GNP that healthcare costs are consuming in the US and worldwide has shifted attention to decreasing utilization of health care services, and other cost-savings measures. Again, up to this point this has been an exclusive part of our IBD book. New findings in the genetics of IBD have been recently published. Our first edition had the finding of the first Crohn’s disease gene inserted just before going to press; so much has been published about the phenotypes and other genotypes that have emerged. Breakthroughs in new endoscopic evaluations of the bowel (ie. Capsule endoscopy, double-balloon enteroscopy, endoscopic ultrasound) and their use in the inflammatory bowel diseases will be a key highlight of this new book. Major advances in novel radiographic techniques employed in patients with inflammatory bowel disease (ie. CT enterography, MR enterography, advanced ultrasounds, etc.) will also an important component of our work. There have been numerous areas related to the development and detection of colorectal cancer in patients with IBD that have emerged in the past few years. Techniques of better detection (ie. Narrow band imaging; special mucosal staining; endoscopic histological evaluations) as well as research in the putative role of mesalamine and other agents for colorectal cancer prevention are all hot topics that will attract much interest from a variety of interest groups I described above. Identification of additional disease states that probably belong in the IBD family, such as microscopic colitis (including lymphocytic colitis), and collagenous colitis add to the diseases that will be discussed in this new edition.

It outlines the latest findings in targeting therapies to the individual patient with Crohn’s and colitis, management of chronic infections in the setting of immunomodulators and biologics, non-surgical therapy of dysplasia in colitis patients, and redefining and structuring the problematic pouch. In addition, this book features useful chapters dedicated to the economic aspects of IBD in an increasingly constrained healthcare system, as well as the patient experience and the role of subspecialist telemedicine care. Written by specialists and thought leaders in the field, Inflammatory Bowel Disease: Pathogenesis, Diagnosis and Management provides a concise but highly relevant account of the latest thinking and concepts in IBD.

Book excerpt: The first gene responsible for Crohn's disease has now been identified and the pathophysiological understanding of inflammatory bowel disease as well as its practical treatment have made much progress in recent years. Thus it is obviously important to consider the targets and the aims of treatment in context. The Falk Symposium No. 131 (Part II of the Gastroenterology Week Freiburg 2002), held in October 2002 in Freiburg, Germany, in its first part was dedicated to newly defined and partly alternative therapeutic targets. In addition, an attempt was made to learn from experts from other fields such as pneumology or dermatology. The second part was dedicated to the aims of treatment in daily practice. Aside from general treatment, aims such as amelioration of symptoms, pre-conditions of treatment, the view of the patients, and aspects of life quality were dealt with. This volume is the proceedings of the symposium and is important reading for both clinicians and basic scientists.

Book excerpt: In this monograph about gene therapy of autoimmune and inflammatory d- orders we have gathered international experts and leaders from different fields to review the state of the art advances on topics ranging from disease entities to vectors and engineered cells. The different approaches described in each chapter take into consideration the biomedical knowledge of these diseases and address the complexities of delivering long-term genetic interventions. Gene therapy also serves as a testing ground for new therapeutic entities and helps provide proof of principle for their potential therapeutic role in animal models of disease. Scaling up from mice to men still remains an important h- dle not only from the quantitative point of view, but also for currently unknown and unexpected secondary effects of the vector or the transgene. Some of these approaches have already been tested in the clinic, but much more needs to be done to understand the human conditions treated and the n- ural history of their pathology. We are indebted to the secretarial assistance of Ms. Lin Wells (Bone and Joint Research Unit, London, UK) and the help of Hans Detlef Klüber for his help in getting this book published. We hope this book will be of interest to c- nicians and scientists and inspiring to students of the subject who will use their own ingenuity and knowledge to further forward this discipline into clinical use.

Book excerpt: The amount of information on the pathogenesis of inflammatory bowel disease is growing rapidly. This is reflected by a continuous increase in the number of papers presented at international GI meetings. To make things more difficult for practicing physicians, there is also a large number of new clinical trials being published which require periodical critical reviews and recommendations. Faced with these issues, the scientific commitee of the Falk Symposium No. 140 decided to take a different approach and to apply a novel format that is reflected in the title of the Symposium: "Translation from basic research to clinical practice". This book contains the proceedings of that Symposium, held in Dubrovnik, Croatia, on May 7–8, 2004. The sections are designed so that they start with the information from basic sciences on different aspects of these complex diseases and further lead to their clinical implications. Special attention is paid to the mechanisms of actions of established drugs. The last two sections are clinically oriented and focus on the most difficult aspects of both Crohn's disease and ulcerative colitis. This format provides state-of-the-art chapters by leading experts in the field and at the same time up-to-date information on the clinical application of the new knowledge.
