Book excerpt: The childhood obesity epidemic continues to escalate, disproportionately impacting Mexican American children. It is unclear how Mexican American mothers, who are at high-risk of rearing obese children, perceive childhood obesity, prevention or their role in prevention. Three studies - a systematic literature review, a qualitative study focusing on Mexican American mothers' perceptions of childhood obesity, and a qualitative study examining Mexican American mothers' perceptions regarding childhood obesity prevention and their role in prevention - were used to address this research question. The first study is a systematic review of the literature regarding Mexican American mothers' perceptions of childhood obesity and their role in prevention. Four databases were searched for relevant articles and 22 studies met inclusion criteria and were included in the review. Social Cognitive Theory was used to sort similar findings across studies and identify scarce areas researched. Major findings included: (a) mothers felt inadequate to be role models for their children's healthy behaviors, (b) mothers did not identify short-term consequences of childhood obesity, (c) only 23% of studies explicitly used a theoretical framework to guide their study, and (d) most studies used heterogeneous groups (including all caretakers, including all Hispanics/Latinos) to discuss perceptions. The second study used naturalistic inquiry to examine mothers' perceptions regarding childhood obesity, its causes and its consequences. Using a Social Ecological Model adapted to childhood obesity, the study examined causes of childhood obesity at different levels of influence - intrapersonal, interpersonal, and community. Obesity was identified as an adult issue by the mothers. Mothers were more aware of the health risks associated with having underweight children rather than overweight children. Lastly, mothers identified overweight children as those who are suffering from consequences. The third study used a narrative inquiry approach to qualitatively investigate mothers' perceptions. Mothers suggested several ways parents could prevent childhood obesity and overweight among their children. However, fathers, grandparents and schools could unintentionally counter mothers' efforts to encourage healthful behaviors. Mothers identified a lack of ability to speak English, feelings of guilt associated with limiting food intake, and a lack of knowledge and skills as impediments in carrying out obesity preventive behaviors within the home.

In comparison with WHO growth standards, 30% children were obese, but 69% mothers wrongly perceived their child's weight. Mothers of overweight sons had more wrong perception than those with overweight daughters. Perception that children were underweight was associated with maternal BMI. Child's weight was found unrelated to feeding practices followed by mothers. But, there were associations between different subscales of feeding practices & family characteristics. Despite mounting public health concern about childhood obesity in India, most of surveyed mothers were not concerned about their children s weight, and many did not perceive their overweight children any different from their peers. This may have implications for interventions that rely on acknowledging child's overweight as a first step to change.

In urban cities, the Black sub-group of Afro-Caribbeans has a high rate of childhood obesity and overweight. The purpose of this study was to analyze Afro-Caribbean mothers' cultural perceptions in London and New York regarding childhood weight and food practices in children age 6 to 12 years. This qualitative content analysis, guided by the Developmental Niche, used a purposive sample of 30 Afro-Caribbean mothers to illuminate cultural perceptions of food and weight in their children. Semi-structured in- depth interviews were conducted with 15 mothers in both London and New York City. Each participant completed a demographic questionnaire that also included two quantitative questions. The first measured mother's perception of weight using a visual image and the other assessed the written description of the child's weight. The eight themes that emerged were: perceptions of childhood obesity within the general population; parents' role in child obesity; physical activity (PA); weight of child; cooking techniques; types of food consumed; food is a social bond that connects child with others; and food preparation varies according to families. Additionally, there were 29 subthemes such as: extracurricular PA is expensive, lack of knowledge about what constitutes a healthy weight, health care provider's involvement, cultural techniques modified and eating as a way to maintain cultural rituals. More than one-quarter of the mother's (27%) in both London and New York had overweight or obese children. In general, mothers tended to select a visual image that showed their children at a lower weight than their actual size. Furthermore, most mothers of overweight and obese children did not perceive their children as such in their responses to the visual images. The implications for nursing practice and future research include increasing cultural competence for nurses, health care providers and students; increasing parent education regarding healthy food substitutes and weight recognition; developing policies to increase physical education for children; and providing weight and nutrition interventions to the extended family.

The aim of this study was to look at maternal perceptions of her childu2019s weight status in 5-year-old children and to investigate factors influencing this.Materials and Methods:339 mother and child pairs were followed up from the ROLO Kids study in the National Maternity Hospital, Dublin, Ireland. Height and weight of the mother and the child were measured, and BMI was calculated. Demographic details were collected and questionnaires asked mothers to assign their child to a weight category and to give the reason they put their child in this particular category. KAPPA analysis and student t tests were used to analyse the data.Results:In this cohort over 75% of the children were normal weight, 14.7% were in the overweight category, and 8.5% in the obese category. The KAPPA score for maternal-perceived category and actual weight status was 0.037 which is a low level of agreement. Mothers were more likely to underestimate their childu2019s weight, with 28.5% underestimating the appropriate weight category for their child compared with 2.4% who overestimated. This dramatic increase greatly impacts adult health as childhood obesity has been linked to markers for cardiovascular disease, such as high blood pressure, type-2 diabetes, elevated cholesterol and an early onset of obesity-related morbidity and mortality. This issue of childhood obesity is particularly problematic in the Latino community. Children have an elevated prevalence of obesity relative to other racial groups and Latinos are amongst the fastest growing ethnic groups in the country. Niños Sanos, Familia Sana (NSFS) is a completed five-year intervention study aimed at improving the health of Mexican-origin children in the rural agricultural communities of California’s Central Valley. This dissertation utilized data collected during the NSFS study to focus on various characteristics related to childhood obesity. The first study explored maternal perceptions of child weight and how they might be impacted by an intervention aimed at reducing BMI gains. Comparisons from baseline to follow-up showed dramatic improvement on the accuracy of maternal perception of their overweight and obese children. Further analysis, however, revealed no evidence of an association between the intervention and changing perception. The second study used mathematical models to estimate the timing of the adiposity rebound (AR) for children in the NSFS study and further examined associations between timing of the AR with exclusive breastfeeding practices. The AR was found to have occurred earliest in the heaviest children (mean age (standard deviation): normal weight: 5.58(0.55), overweight: 4.65(0.48), obese: 3.55(0.65)). No significant association was shown between exclusivity of breastfeeding and age at rebound. The third study aimed to determine the effect of an intervention targeting reduction in BMI gain on the growth of subcutaneous fat tissue in Mexican-origin preadolescent children. Outcomes of the models included a log-transformed sum of the raw measures and z-scores for triceps and subscapular skinfold thickness. For all measures and across age and gender, no significant reduction in skinfold thickness due to intervention was observed.

Participants were 120 mothers who completed survey measures of sociocultural appearance norms, health efficacy, and physical activity and health eating behaviors. The results of this thesis found sociocultural appearance norms to predict physical activity and health eating through the mediator of health efficacy among women.

Book excerpt: "Health disparities are prevalent in the U.S, with low-income African American children suffering from higher rates of obesity and chronic diseases compared to White children. Currently, little is known about parental perceptions of healthy eating, and concerns related to child health and weight in this at-risk population. The main purpose of this study was to examine perceptions, motivating factors and barriers to healthier eating in a sample of low-income African American parents of 3-5 year old children. The second purpose was to assess parental concerns about health and/or weight and to examine the accuracy of parental perceptions of child weight status. Participants were recruited from Head Start Programs in North Carolina. The Social Cognitive Theory constructs guided focus groups and a survey administered to the participants. Content analysis of 8 focus groups generated the following themes: 1) lack of nutrition knowledge and misconceptions; 2) healthy meals are home-cooked, include meat and starch; 3) family members, lack of maternal modeling, and child pickiness are main barriers to healthier eating; 4) strong awareness of family history of chronic disease, including obesity; 5) lack of concern about child's current weight. Over 25% of mothers underestimated their child's weight status. Our findings highlight important maternal perspectives influencing children's diet quality and long-term health outcomes among low-income at-risk preschoolers. Nutrition educators should be aware that low-income African American mothers may be aware of chronic disease risks, but do not perceive food choices in early childhood as having strong impact on the child's future health."--Abstract from author supplied metadata.
