Book excerpt: Chapter 1: Interregional Transfers through Public Pension in Canada- In this chapter, I build a regional computable general equilibrium model with an overlapping generations (OLG) structure of the Canadian economy to analyze population aging dynamics and public pensions. Canada is divided into three regions: Atlantic, Quebec and Rest of Canada (ROC). The impact of population aging is investigated on each of three regions' pension systems. The results confirm that as a result of aging all regions are affected negatively if they choose to have an independent pension system. Under a joint pension system most of the pressure of the provision of the pension system is on the ROC. Atlantic region benefits the most from a joint pension plan as the implicit funds ow from ROC to Atlantic region. Quebec benefits from having its own program, but the benefits disappear slowly in future years. Chapter 2: Age-Variable Rate of Time Preference in CGE-OLG Model- Contrary to the mainstream studies in the area of intertemporal optimization that assume a constant rate of time preference over individuals' life cycles, in this chapter I propose a new approach to measure the rate of time preference by assuming that the rate of time preference evolves by age. I construct an overlapping generations model (OLG) and calibrate rate of time preference. The age-variable rate of time preference would permit to capture many other elements that affect the life cycle profile of consumption as observed in the data. The results show that rate of time preference exhibits three phases and is different for young versus old. Chapter 3: Computing Demographic Change Simulation under Constant and Age-variable Rate of Time Preference - This chapter simulates the impact of an aging population on various macroeconomic variables and calculates the cohort welfare as well as social welfare. The outcomes from simulations are dependent on the choice of rate of time preference as well as the structure of the model. The results in this chapter provide a new approach to determining the impact of aging population. The choice of a realistic rate of time preference, which allows its variability by age, affects the cohort welfare noticeably.

Book excerpt: Chapter 1: A Population Aging Analysis for Canada Using the National Transfer Accounts Approach: This analysis develops a new data set for Canada using an accounting methodology called National Transfer Accounts (NTA). NTA permits building an accounting system that introduces age into national accounts. NTA is consistent with the conventional national accounts and allows the estimation of lifecycle patterns for labour income and consumption from private and public sources. It also allows the calculation of per capita and aggregate lifecycle deficits (LCD) or surpluses (LCS) in an economy. In this chapter we calculate Canada's per capita and aggregate LCD for 2006. Using demographic projections for the next five decades, we present the aggregate LCD in Canada for the period 2006 to 2056 assuming a constant per capital LCD during this time horizon. The projection results show that labour income needs to increase rapidly or consumption needs to be cut significantly to compensate for the pressure on the aggregate lifecycle deficit as a result of population aging. Chapter 2: An Overlapping Generations Computable General Equilibrium (OLG-CGE) Model with Age-dependent Rates of Time Preference: This analysis develops a methodology to introduce an age-dependent rate of time preference to calibrate an OLG-CGE model with per capita age profile consumption path derived from the NTA framework of Chapter 1. The results show that the economic impact from an aging population will be significant. The living standard will decline by 15% from the present to 2050 and decline by 20% by the year 2105. To reduce such a decline, the Canadian government may introduce policies that could encourage labour force participation. We analyze the impact of: a) an increase in the general labour force participation rate for age 20 to 64; b) an increase in the labour force participation rate for workers aged 50 to 64; c) late retirement. Our results suggest that maintaining the current standard of living will be extremely difficult after 2026. Nevertheless, this would help reduce the economic pressure from population aging. Chapter 3: 9/11 Security Measures and North American Security Perimeter: A Computable General Equilibrium Analysis: The objective of this analysis is to assess the impact of the North American Security Perimeter (NASP) on both the Canadian and the U.S. economies. The NASP is a change in Canada-U.S. security paradigm that would allow the liberalization of the post 9/11 security measures at the Canada-U.S. border. This study applies a multi-sector and multi-region general equilibrium model together with econometric analysis. After simulating the NASP, Canada's capital market was found to become more attractive. Under the NASP, the welfare of Canada is estimated to increase by $19 billion or 1.8% of GDP, and that of the U.S. is estimated to increase by $32 billion or 0.3% of GDP.

Book excerpt: This dissertation is a collection of three essays that use economic tools to address policy-relevant issues related to ageing, population health, and education. The use of economic modelling and econometric analyses has the potential to provide information on the consequences and effectiveness of policy interventions in these areas and enables policymakers to make better informed decisions. Chapter 1 provides an introduction to these topics and is followed by the three essays. In Chapter 2, I analyze how providing informal care to an elderly parent affects the caregiver's labour market outcomes, cognitive ability, and health; and study the influence of the institutional background on the caregiving decision and the effects of caregiving. My results show that negative effects on labour market outcomes can be avoided by the provision of formal care alternatives, but negative effects for caregivers' mental health persist. These findings give useful insights into the optimal provision of formal care in today's ageing societies. Self-reported health measures are commonly collected in numerous surveys but might be influenced by respondents' definitions and frames of reference of health. In Chapter 3, I address the issue of response bias in population surveys by constructing an objective measure of health. I find that using a common definition of health nearly eliminates the reported health differences between the U.S. and Canada. Socioeconomic differences in health are stronger in the U.S., but remain an issue in Canada. Chapter 4 studies the effect of post-secondary education on the continued development of reading proficiency during adolescence and young adulthood. Reading proficiency is essential for labour market success in a knowledge-based economy, but little is known about how advanced reading skills such as text interpretation and text evaluation are developed. The results show that university graduation increases students' reading proficiency relative to high school graduation, which demonstrates the importance of cognitive skill investments later in the life cycle.

Book excerpt: This dissertation consists of three essays on the impact of publicly funded programs and how they can affect the well-being of the low-income population. The first essay focuses on Broad-based Categorical Eligibility (BBCE) expansion in Supplemental Nutrition Assistance Program (SNAP) eligibility in the United States. Applying the difference-in-difference model, I find that BBCE expansion in SNAP eligibility increases SNAP participation and weekly food expenditure and decreases food insecurity. The event-study model suggests that there is a lag between the effect of BBCE expansion on SNAP participation and food insecurity and the expansion and the effects are long-lasting. Using BBCE expansion as an instrumental variable for SNAP participation, I show that the effect of BBCE expansion on weekly food expenditure is through increased SNAP participation; however, the reduction in food insecurity cannot be explained by increased SNAP participation. The second essay focuses on maternity leave policies in the United States. Using the propensity score matching method, I find that a mother’s maternity leave-taking is positively associated with the probability of children graduating from high school and the probability of children ever attending college. Subsample analyses also show that the effect is slightly larger for low-income mothers. The third essay examines the effect of New Rural Pension Schemes (NRPS) on the health conditions of older adults in rural China. Both regression discontinuity and triple-difference methods show that NRPS significantly improves self-reported health for the elderly in rural China and reduces reported symptoms of headaches, stomachaches, and asthma. In all, public transfer programs targeting the low-income population not only serve as a safety net to low-income individuals but also benefit the low-income population by improving their diet, health, and their children's outcomes.

Data analytics, which has been seen as a significant power in other industries, is expected to contribute to the improvement of efficiency and effectiveness in healthcare. This thesis aims to identify and promote more effective and efficient strategic, operations and clinical policies in healthcare systems through descriptive, predictive and prescriptive analytics. To this end, this thesis focuses on three essays, i.e. three data-driven problems based on medium to large size of real life datasets, on: i) design of financial incentive systems for maternity care; ii) design of specialist response policies and modified triage coding to reduce waiting times in emergency departments (EDs), and iii) design of observation units for hearth failure patients. The first essay focuses on strategic level and aims to design a two-level financial incentive mechanisms to reimburse physicians, in order to reduce unnecessary C-sections while retain it for those who need it, resulting in enhanced birth quality with alleviated economic burden for overall health care system. Contributing to clinical decision-making, we first cluster the patients according to their pregnancy complexities, and characterize a threshold between spontaneous birth and medically necessary planned C-section by analyzing 12.7 million annual birth records from National Bureau of Economics Research through statistical learning methods. Then we compare payment systems analytically vis-\'{a}-vis a variety of performance measures within two-level hierarchy, (i) mainstream payment models and (ii) compensation on the top of mainstream payment, and provide insights about the effectiveness of alternative payment models in the context of maternity care. Finally, we propose optimal payment for physicians to maximize the value for patients under the principal and agent framework, from the strategic perspective.The second paper focuses on operational level and targets to reduce the length of stay in EDs by designing a systematic response policy for various specialists depending on ED clinical demands. This work is motivated by and verified with 40,000 ED visits to a local community hospital in Montreal. We first identify a class of patients who are more likely to require specialist consultation based on their clinical information available at the triage stage through statistical analysis. Then we analyze several alternative policies for specialists' response to consultation requests using queuing models with non-homogeneous Poisson arrival rates. Moreover, we examine an integrated ED decision-making by incorporating specialist consultation requests in the triage system. Finally, our proposed optimal specialist response policy and associated modified triage coding are verified through a comprehensive simulation model. We provide a feasible guideline of integrated patient streamlining to shorten length of stay and alleviate overcrowding in ED. The third paper focuses on clinical level and propose a framework to design a dedicated observation unit for acute decomposition heart failure patients, in order to provide proper treatment and reduce unnecessary hospitalization and chance of post-discharge events. To this end, we, first, use multiple analytical models to figure out the proper number of bed for this observation unit based on historical patient arrival data from a local community hospital. Based on the confined range of analytical capacity, we use simulation models to analyze different discharge and admission policies. We propose an optimal discharge-admission criteria for this dedicated observation unit to realize cost-saving and quality enhancement of treating acute decomposition heart failure patients." --

Book excerpt: This dissertation employed a psychological framework to investigate the saving behavior of older pre-retirees through three essays using data from the Health and Retirement Study (HRS). Understanding the connection between psychological characteristics and saving behavior is critical as this population attempts to bridge the retirement saving gap. Of these characteristics, financial self-efficacy beliefs (FSE) are theoretically vital to saving behavior. With the FSE beliefs of older adults weak and vulnerable to decline, more research is needed to understand how FSE beliefs affect saving behavior and how FSE beliefs can be supported. Essay one investigated the psychological characteristics associated with FSE beliefs according to the Meta-Theoretic Model of Motivation and Personality (3M). Using a sample of 2,070 pre-retirees aged 50 to 70, essay one revealed that FSE beliefs can be supported through the frequent experience of positive affect, reduced negative affect, a stronger perception of mastery, and a higher task orientation, holding all else constant. Essay two investigated the relationship between FSE beliefs and saving behavior (i.e., change in net worth from 2008 to 2012) through the Social Cognitive Theory of Self-Regulation. Using a sample of 844 pre-retirees aged 50 to 70, results revealed that FSE beliefs are significantly and positively related to saving behavior, after controlling for the financial ability and motivation to save. Essay three employed a structural equation model to investigate an integrated psychological approach to saving behavior based upon the 3M. Using a sample of 1,370 pre-retired and partially retired adults aged 50 to 70, essay three revealed that FSE beliefs facilitated the connection between elemental traits (i.e., openness, conscientiousness, extroversion, agreeableness, and neuroticism), compound traits (i.e., positive affect, negative affect, mastery, and task orientation), and saving behavior. Overall, significant evidence was generated supporting a psychological approach to the saving behavior of older pre-retirees. Financial and mental health professionals can utilize this framework to provide holistic retirement saving advice that acknowledges the psychological roots of behavior. Moreover, results established empirical support for the role FSE beliefs play in executing saving behavior. Lastly, results supported the importance of domain specific measurement for self-efficacy beliefs in future research.
