Book excerpt: The brilliant yet simple idea of introducing a catheter percutaneously into an artery, without first dissecting it free, using a flexible guide wire, has led to a truly revolutionary breakthrough in abdominal x-ray diag nosis (SELDINGER, 1953). In the meantime, methods and techniques for injecting contrast media into various vessels have become largely standardized; innumerable publications have appeared which deal with every conceivable aspect of angiographic technique and interpretation. This volume is designed to present our experience with abdominal angiography. We deliberately refrained from any systematic discussion of the genitourinary tract, which has been adequately dealt with in the literature, also with respect to angiographic findings. Our interest in the retroperitoneal region is based mainly on its significance in differential diagnosis. In ten years of angiographic activity, our Department had made successful use of a simple technique which appears suitable also for smaller hospitals. We wish to point out its diagnostic potential and, at the same time, to outline its limitations. Our experience embraces 2804 abdominal angiograms, which we have classified according to clinical and morphologic anatomical criteria. Their diagnostic interpretation has been compared with the surgical or histopathological results. This may help others to avoid errors of the type which we discovered in our own work. Angiographic diagnosis requires not only familiarity with normal radiographic anatomy, but also specific knowledge of angiographic patho morphology. We have tried to identify those features which typify the individual findings and to derive therefrom valid generalizations with the aid of simple sketches.

Book excerpt: The present anthology seeks to give an overview of the different approaches to establish a relation between phenomenology and psychoanalysis, primarily from the viewpoint of current phenomenological research. Already during the lifetimes of the two disciplines' founders, Edmund Husserl (1859 - 1938) and Sigmund Freud (1856 – 1939), phenomenological and phenomenologically inspired authors were advancing psychoanalytic theses. For both traditions, the Second World War presented a painful and devastating disruption of their development and mutual exchange. During the postwar period, phenomenologists, especially in France, revisited psychoanalytic topics. Thus, in the so-called second generation of phenomenology there developed an intensive reception of the psychoanalytic tradition, one that finds its expression even today in current hermeneutic, postmodern and poststructuralist conceptions. But also in more recent phenomenological research we find projects concentrated systematically on psychoanalysis and its theses. In this context, the status of psychoanalysis as a science of human experience is discussed anew, now approached on the ‘first person’ basis of a phenomenological understanding of subjective experience. In such approaches, phenomena like incorporation, phantasy, emotion and the unconscious are discussed afresh. These topics, important for modern phenomenology as well as for psychoanalysis, are examined in the context of the constitution of the human person as well as of our intersubjective world. The analyses are also interdisciplinary, making use of connections with modern medicine, psychiatry and psychotherapy. The systematic investigations are enriched by historical analysis and research in the internal development of the disciplines involved. The volume presents recent work of internationally recognized researchers – phenomenologically oriented philosophers, psychoanalysts and psychotherapists – who work in the common field of the two disciplines. Book excerpt: Since the early days of medicine one concern of doctors has been the removal of kidney stones and prevention of recurrence. Owing to the hesitancy of progress in the prevention of initial stone formation and of relapse, however, removal of stones from the kidney and ureter were developed to highly refined techniques and they formerly accounted for a major proportion of the urological operations performed. In the last few years developments in the treatment of kidney stones have taken a completely different turn. In the majority of cases suit able methods are available to bring about spontaneous passage of the stones, while in a smaller proportion drug-induced litholysis is pos sible. Stones that cannot be passed are now treated mainly with extra corporeal shockwave lithotripsy, percutaneous litholapaxy or uretero renoscopy. These methods are often used in combination and comple ment each other. Nonetheless, despite the accumulating experience with the new methods there will still be situations in which stones can not be removed except by open surgery. "Our skill as surgeons and the management of the brilliantly designed equipment would amount to nothing more than highly skilled mechanical work if they did not go hand in hand with enhanced insight into the cause of lithiasis and thus into ways of preventing it - or at least of preventing the relapse that is the lot of most patients.
