The continuous emergence and evolution of pathogenic E. coli strains associated with human and animal infections have demonstrated that (i) groups of related pathogenic E. coli are responsible for most infections caused by this bacterial species; (ii) diverse virulence phenotypes expressed during infection defined each one of these pathogroups; (iii) the geographical distribution of pathogroups in Latin America and the evolution of new isolates was defined by the dominant pathogroup and presence of distinct virulence strains; (iv) acquisition of mobile elements or accumulation of point mutations accelerate the development of antibiotic resistance in some of these strains. The Latin American Coalition for Escherichia coli Research (LACER), a multidisciplinary network of over seventy research groups in eleven Latin American countries and the USA, was established in 2009 to apply One Health principles in defining and combating this pathogen. It has long been recognized that E. coli exhibits quite extensive substructuring and that the relative abundance of the most common phylo-groups varies with respect to host age and sex, host species, as well as geographical location. Strains belonging to some phylo-groups are much more frequently encountered in water samples than strains belonging to other phylo-groups. The extent to which strain survival in the external environment has determined the evolutionary origins of the various diarrheal pathotypes is unknown. There is a growing body of evidence that indicates commensal E. coli, like the diarrheal pathotypes, exhibits at some tissue specificity within the gastrointestinal tract. However, there is little evidence to suggest that commensal E. coli exhibit extensive host specificity as is the case with many of the diarrheal pathotypes.

Book excerpt: In addition to the well-established and extensively studied categories of pathogenic E. coli, there are other groups of E. coli associated with diarrheal and/or inflammatory bowel diseases that have not been thoroughly characterized or which are considered newly emergent pathogens. This chapter focuses on three representatives of these groups of pathogenic E. coli and the current progress understanding their role in pathogenesis. Diffusely adherent E. coli (DAEC) are a collection of isolates that colonize the small bowel, considered potentially diarrheagenic, and that can cause childhood diarrhea. Adherent and invasive E. coli (AIEC) are a group of mucosa-associated bacteria with invasive properties in Crohn’s disease patients and can survive and replicate extensively in macrophages. Finally, E. coli O104:H4, a hybrid Shiga toxin-producing enteroaggregative E. coli, which caused a large outbreak in Europe in 2011, displays a unique combination of virulence traits resulting in the development of hemolytic uremic syndrome in an unusually large proportion of infected patients. This chapter summarizes recent progress in understanding the pathogenesis and the clinical outcome of these types of E. coli infections.
